On 512212023, I conducted an unannounced inspection at the child care center. I interviewed program director, Ahmad Hazime. Mr. Hazime said Child A has been enrolled at the center for approximately a year. Child A's Mother is very hard to please. Mr. Hazime stated Child A's Mother contacted staff after hours making up allegations. She threatened to call 911 the prior week because she said someone was hurting Child A. On Wednesday, May 17, 2023, or Thursday of last week, Mr. Hazime requested a meeting with Child A's Mother and his stepfather. Child A's Mother was upset that Child A had ,and no one informed her. Child A's Mother put on him, Mr. Hazime explained to her that if there were any incidents that prompts the center to call her, they would call. He explained that the injuries to Child A's knees did not occur at the center. He asked Child A's Mother to bring Child A in long pants so the knee patches could be covered so he does not pull them off. Child A's Mother brought Child A in on Friday with shorts on again. Mr. Hazime stated to her that he asked her not to bring him in shorts. She stated that she could not find any long pants for him to wear. The entire day, Child A picked at the patches until they would not stick any longer. Child A's Mother was left a daily report by CCSM 1 informing her that Child A pulled on his patches until they would no longer stick to his knees. Later that evening, Child A's Mother called CCSM 1 through someone they both know after hours. Child A's Mother stated that Child A told her that CCSM 1 took his patches off. CCSM I explained to her that she left her a note on the daily report regarding the patches. On Friday, May 19, 2023, Child A's Mother was contacted around 8:15 a.m. She was asked not to bring Child A to the center anymore. Child A's Mother showed up anyway and Mr. Hazime met her outside in the parking lot. He explained to her that Child A is no longer allowed to attend the center and she is trespassing. police were contacted, and she was asked to wait off the premises until they arrived. It was explained to her that she is not allowed on the premises. The police arrived and were informed of the situation. She was asked to leave the premises and not return. On 512212023, I interviewed CCSM 1. She stated that Child A's Mother asked for a job at the center about a month ago. She was informed they were not hiring then she asked about volunteering. There were two accidents Child A was involved in at the center. On 2124123, Child A was fighting over a toy with another child and was hi On 51012023, Child A and anotlr ~r child were running in on the class and burnpe flnto each other causing . Both accidents were reported to Child A's Mother and she was provided an accident report form to sign acknowledging she received it. Child A was not at the center on Thursday or Friday. Child A's Mother informed the on Child A in thr area. No one was aware of the onp iiipd A's Mother continued to make threats and post on social media. On 5/22/2023, I interviewed CCSM 2. Child A was in CCSM 2's room for six months before transitioning to CCSM 3's room. Child A's Mother would ask about CCSM 3 and CCSM 2 reassured her Child A would be fine. Child A's Mother was concerned that she did not know what was happening in his classroom and if CCSM 2 could check on him. Child A's Mother informed her that CCSM 3 told her about the incident when Child A was _ for taking someone's toy. She threatened to sue the center after she left the center. She was told by Mr. Hazime to stop calling his staff and harassing them. Child A's Mother asked CCSM 2 if Mr. Hazime asked CCSM 3 to remove Child A's patch so he could get hurt. CCSM 2 was told to block Child As Mother phone number and not engage with her. On 5/22/2023, 1 interviewed CCSM 3. CCSM 3 stated the children were going to large group. On 05/25/2022, I discussed the incident with Child A's Mother by telephone. Child A is doing well and exhibited no signs of a burn, redness or irritation from the incident. She is concerned about the supervision and will not be returning Child A to the facility. On 05/18/2022, she picked Child A up at approximately 05:45 p.m. and saw her clothing had been changed. She was told, "She had an accident" and the bag of soiled clothing was given to her. On 05/23/2022, while doing laundry she observed the clothes were soiled with coffee stains. Child A's Mother called the school and the person at the desk did not know what happened but would have someone call her back. On 05/25/2022, the licensee Mr. Ahmad Hazime called and explained a teacher had an open cup of cold coffee sitting within Child A's reach. The coffee was cold when purchased and was in the room since earlier in the day. He apologized for the incident and indicated he had spoken to his staff. On 05/11/2022, Child A was observed to have a red bump between her eyes. Child A's Mother had arrived home and noticed the bump when removing her from the car seat. It looked as if it was turning the color of a bruise. When she inquired about it the following day, the facility knew nothing about it and stated she could have injured herself. Child A did not receive medical attention for any of the incidents. On 06/02/2022, I conducted an unannounced inspection at the center. I interviewed the program director Mona Agemy, licensee Mr. Hazime and observed the space where the coffee incident took place. The room has no blind spots and there was nothing left out that could cause injury to a child. I also observed the two child care staff members (CCSMs) present providing appropriate care and supervision to the children present. Ms. Agemy and Mr. Hazime regret the coffee incident happened. They admit leaving the cup on the floor was a mistake which should not have happened and they are happy there was no injury to Child A. Coffee of any kind is no longer permitted in rooms where children are present. Mr. Hazime called Child A's Mother after being notified of the incident. He explained what happened, took full responsibility for the accident and thought the situation had been resolved since Child A's Mother was receptive to his explanation and apology. She never complained of any other incident, and he knew nothing about the red bump between her eyes prior to my mentioning it to him. The facility has a procedure of notifying parents immediately by phone and following up with incident reports should there be a serious incident or accident. Since parents are not allowed in the building, they do not have an opportunity to talk with their child's teacher. If anything happens, it's reported to the front desk who can communicate it to parents at pickup time. Ms. Agemy or Mr. Hazime knew nothing about a mark between Child A's eyes. Once they were made aware during this investigation, Mr. Hazime checked with staff who indicated there was no mark the day Child A left or when she returned the following morning. 3 On 06/15/2022, I interviewed CCSM Kawthar Aoude. On the day of the incident, she was the only staff present in the room with three children. It was close to departure time, and they were all in the book area of the classroom. She realized another child's diaper needed changing and went to the changing area. The area is adjacent to the classroom and not far from the book area. After changing the diaper, she returned and saw Child A's clothing was wet with the coffee. She was not crying or showing any signs of discomfort. There was little coffee in the cup, and it had been left over from earlier in the day. She immediately cleaned her up and changed her clothing. Ms. Aoude thought she was doing the right thing in changing the clothes and did not write anything up since there was no injury. On 04/25/2022, 1 conducted an unannounced investigation at the facility and inspected the classroom where Child A attended. The space has three distinct play areas but no blind spots in any of them. The child care staff members (CCSMs) assigned to the room were not present, but I was able to interview them at a later date. The licensee Ahmad Hazime was not present but immediately called me when he arrived. He denied Child A had any bruising when he left the center or when he returned the following morning. Child A's Mother never discussed the issue with him or any of the staff. Mr. Hazime found out about the allegation when she arrived at the facility with A's On 0510212022, 1 interviewed Child A's Mother. Child A is doing well and she is open to him returning to the facility. She had no major problems or concerns prior to the current incident. On 04/21/2022, she dropped him off at 09:00 a.m., returned at 06:00 p.m., and "I instantly saw the bruises on his forehead and face. It also looked like someone had pulled a clump of his hair outs The marks gave her the impression Child A had been in a fight with another child at the center for his pacifier. She admits to not mentioning anything to the center staff about the injuries but is not sure why. She called and reported the injuries to "my assistance worker" who told her the situation needed to be reported. Child A's Mother denied telling anyone there was bruising all over his body. 3 On 05/02/2022, l conducted a telephone interview with CCSMs Kawthar Aouda and Randa Harhara. They were working in the classroom the day Child A was picked up and dropped off the following morning. They did not notice any bruising on his face, or head and there was no evidence of any hair having been pulled out. On the day of the alleged incident, Child A and the other six children present had an uneventful day. According to Ms. Aouda, Child A's Mother `mentioned something about blood being on his face, which turned out to be sauce or ketchup." The exact day the comment was made cannot be recalled. On 0512612022, 1 interviewed the Complainant. She had no additional information regarding the bruising of Child A. She had not seen him but was told he had bruises all over his body when he was picked up from child care. Child As Mother requested and was provided assistance in locating another daycare facility.
