Out of the nine records reviewed, eight child information cards were incomplete. Four child information cards were missing employer information, two were missing work telephone numbers for the employment that was listed, five were missing allergy information and one was missing permission for the center to secure emergency medical treatment.

A child care staff member shall give or apply medication, prescription or nonprescription, only with p rior written permission from a parent.

The center did not have medical permission slips on file for Child A, Child B, Child C or Child D's inhalers or epi pens.

Based on my interview with Licensee Designee Bianca Blue-Thomas and Child A, Child B, Child C and Child D's Mother, Child A, Child B, Child C and Child D epi pens were not returned after being disenrolled and Child B, Child C and Child D's inhalers were not returned after being disenrolled. Licensee Designee Bianca Blue-Thomas admitted to possibly throwing the medication out without Child A, Child B, Child C and Child D permission.
