(ii) Condition of the child's health. (iii) Acknowledgement that the parent has been offered either a copy of the licensing rules for a child care home or has been given the website for an electronic copy of these rules. (iv) Agreement as to who will provide food for the child. (v) Acknowledgement that firearms are on the premises, if applicable. (vi) If the child care home was built prior to 1978, then the licensee shall inform the parents of each child in care and all personnel of the potential presence of lead-based paint or lead dust hazards, unless the licensee maintains documentation from a lead testing professional that the home is lead safe. On 06/28/2022, I completed a telephone interview with Child A and B's Mother. Child A and B's Mother enrolled her children with Ms. Charice Madison on 06/07/2022 and pulled her children out on 06/09/2022 for Child A and 06/10/2022 for Child B. Child A and B's Mother offered to provide copies of Child A and B's immunization waivers to Ms. Madison. Ms. Madison did not seem to care about obtaining the records. I asked Child A and B's Mother if she filled out any forms for enrollment and she stated that she did. On 06/30/2022, I completed an unannounced inspection at Charice Madison's home. Ms. Madison stated that she had 14 children enrolled in care. I reviewed 14 children's records and all children had a Child In Care Statement/Receipt (CCL- 3900) with the vaccination questions answered. The CCL-3900 is the only required proof of vaccinations required by the department for family and group child care homes. In addition to having a CCL-3900 for all enrolled children, Ms. Madison also had vaccination records for all enrolled children. Ms. Madison thought that she was required to have both the CCL-3900 and the children's vaccination records on file. Additionally, Ms. Madison had the CCL-3900 form for Child A and B, signed by their parent. Additionally, she had Child A's immunization wavier.

(1) A licensee shall ensure that all of the following requirements are met: (a) Each child shall be provided with nutritional and sufficient food pursuant to the minimum meal requirements of the child care food program, as administered by the Michigan department of education. On 06/28/2022, I completed a telephone interview with Child A's Mother. Child A's Mother stated that when she picked Child A up from Ms. Madison her first day attending, she was very hungry. Child A's Mother stated Child A was dropped off at 8:30 a.m. and picked up between 7:00 and 7:30 p.m. When Child A got home, she ate a large amount of food, more food than she ever eats during dinner. Child A told her that the only thing she ate that day was hot dogs. The next day she attended Child A's Mother told Ms. Madison she would have food delivered to Ms. Madison's home for Child A to eat. Child A's Mother also told Ms. Madison that she would have bulk snacks delivered to her home to keep for Child A. Ms. Madison informed her that she provides snacks however, Child A's Mother still wanted to have bulk snacks sent to the home. Child A went to Ms. Madison the following day and the same thing occurred; Child A was picked up very hungry. Child A's Mother enrolled the children with the agreement that Ms. Madison would supply all meals and snacks. Child A was dropped off a third and final day at 8:30 a.m. and picked up between 7:00 and 7:30 p.m. When picked up Child A was very hungry and informed her that she had only had chips to eat. Child A's Mother felt that if Ms. Madison saw Child A eating chips all day, then that was an indication that she was hungry and should be provided a meal or more food. Child A's Mother decided to not allow Child A to return to Ms. Madison. During the onsite inspection on 06/30/2022, Ms. Madison showed me a refrigerator in the basement fully stocked with bulk meats and foods. I viewed several cabinets in Ms. Madison's kitchen filled completely with bulk dry goods, ready-to-eat baked goods, and various ready-to-eat snacks. Additionally, the refrigerator in Ms. Madison's kitchen had a large volume of meats, milk, vegetables, and fruit. The kitchen also had bulk food items stored around the kitchen table such as muffins, chips, and granola bars. Ms. Madison stated that she is in the food program which reimburses her for meals provided to the children enrolled. Ms. Madison is to create a menu for each month and provided it to her food program sponsor along with attendance records detailing what child was provided with which meal. Ms. Madison provides all meals and snacks for the children enrolled. She offers breakfast, lunch, dinner, and two snacks. Meals and snacks are provided to children at scheduled times. However, the meal schedule can vary based on the time in which the children arrive and how long they are in care. Ms. Madison stated that all children enrolled generally arrive and are in care long enough to be provided at least one meal and two snacks. Ms. Madison states that children are offered more food if they finish 4 their food. If a child informs Ms. Madison that they are hungry she allows them to eat. The time of their arrival and the time of the next meal will determine when the next meal is served. It can be earlier the usual or she will just provide the child another snack until the next meal. Ms. Madison always gives a child a meal or snack if they tell her they are hungry or appear hungry. Ms. Madison stated that she offers the children more food until they do not want anymore and even asks them what they would like more of. Ms. Madison stated that she does have children who do not want to eat what she has planned and prepared on some days. She also has children who waste their meals and do not fully eat what was provided. Ms. Madison stated that she informs parents when the children do not eat the meal or snack that she provided. Ms. Madison stated that Child A ate what was provided on the menu for the dates the child attended. Ms. Madison stated that at snack time on the second day that Child A was in care, Child A wanted more bags of chips, Ms. Madison allowed her two bags, and told her that she could not have more because then she would not want to eat dinner. On 06/28/2022, I completed a telephone interview with Child A and B's Mother. Child A and B's Mother stated that on Child B's last day attending Ms. Madison's child care she noticed Child B had a welt on his leg and a scratch in his diaper area. Child A and B's Mother sent a message to Ms. Madison about this. Ms. Madison instantly called her and denied anything occurring on her end. Child A and B's Mother stated that it kind of looks as if Child B could have scratched himself but, he has never scratched himself in that area before. Child A and B's Mother also admitted that she does not cut Child B's nails. After this incident, she stopped sending Child B to Ms. Madison because she did not feel comfortable. On 06/30/2022 during my onsite inspection, Ms. Madison denied scratching Child B or him having a welt on his leg. Ms. Madison stated Child A and B's Mother texted her that evening and sent her a picture of the scratch. The scratch was scabbed over, and Ms. Madison felt that if it had happened while he was in care with her that day it would not have scabbed over that soon. Ms. Madison does not remember seeing or feeling any scratches on Child B while changing his diapers. I viewed the pictures that were texted to Ms. Madison. The pictures showed two very faint scratches across the front thigh. The pictures were the same that the complaint source sent to me to view. The complaint source included an additional picture that Ms. Madison did not. This picture was of a single scratch that was scabbed over. The scratch was along the side of the child's butt. The scratches did not look like bite marks. ..........
