A complaint was received stating that on May 16, 2023, Child A had sustained a laceration to his cheek after Child B fell over onto him while sitting in his chair. Child A sustained an injury to his head, including a laceration on his right cheek and back of his head. One of his arms was stuck in the chair after it fell. It is unclear what kind of supervision was going on in the home when it happened. On May 17, 2023, I interviewed Child A's Father. He stated that he was contacted by Mrs. Ledbetter yesterday due to an injury his son had received when another child's chair fell over on him. She contacted him and the mother immediately and explained what happened. It was around 9:00am and it was breakfast time there. He wasn't able to pick Child A up until after 11:00am, so he did after that. He felt that Mrs. Ledbetter was totally transparent about what happened, and she felt terrible about it. She told them it was an accident, and she couldn't have stopped it, even though she was nearby. 2 At first the wound didn't look that bad, but they decided to take him for medical treatment because it was a cut on his face. They did a scan, and everything looked fine. There were no other injuries besides the laceration. They didn't need to do anything for the cut, not even a bandage. They put an ointment on it and said to keep an eye on it. The medical personnel told them that by law, they had to contact the Department of Health and Human Services to report the injury. He also talked to someone from DHHS already and told them (name unknown) that he had no concerns about the incident itself or the home. He explained to them that it was an accident, and that Mrs. Ledbetter has always provided appropriate care and supervision in the home. They have an older child who also attended the home and there have never been any issues with the quality of care provided by Mrs. Ledbetter. On May 18, 2023, I completed an unannounced onsite inspection at the child care home from 1:00pm to 2:30pm. Mrs. Ledbetter was present and working in the home. Child B was observed playing during the interview. Child A was also present but was sleeping. Mrs. Ledbetter stated that on May 16, 2023, it was around breakfast time, and she was in the kitchen making breakfast and preparing to serve the children. There were five children there when it happened. The older ones were already seated at the table, including Child B. Child A was toddling around the table area. Child B pushed his chair back and it toppled over backwards just as Child A was standing behind the chair. The kitchen is open to the dining area, so she was in the vicinity and within line of sight when it happened. The dining set has a large rug under it, with hardwood floors underneath. Immediately she ran over to check on them both. Child A's arm had gone through the middle slat of the chair after it fell. Fortunately, Child B did not fall on top of Child A. He fell to the side and only the chair fell onto Child A. Child A had fallen down and the chair was on top of him. He cried for a minute and then stopped. He appeared more scared than hurt. She noticed a small amount of blood dripping down from a small cut by the crease of his cheek and his nose. She did not see any injury to the rest of his head. Child B did not have any visible injuries. She immediately cleaned the wound and gave first aid to Child A. She then texted both of Child A's parents to let them know what happened and sent a picture of the injury. She didn't get an immediate answer, so she called Child A's Father. She explained to him what had happened and her concern for the small amount of blood by his nose. After looking at the picture Child A's Father indicated that he seemed ok, and they discussed her keeping an eye on it. She agreed to do so but encouraged them to come see it if they felt they needed to. She felt that they should come get him, but he didn't seem too worried.
