On December 16, 2021, I interviewed Child A's mother. She noticed that Child A had a mark above and to the side of his eye and had a mark under his nose around 6pm on December 9, 2022. Child A goes to Kidcare around 6am in the morning, is bussed from there to GSRP Preschool at Davison's Central Elementary around noon Monday through Thursday and after PM Preschool, is bussed back to Kidcare until he is picked up around 5:00pm — 5:30pm. She did not notice the marks until Child A's older sibling said, "look at his face." She had received an e-mail from Child A's GSRP teacher at 12:45pm with a picture of the mark above Child A's eye. Child A's GSRP teacher wanted to let her know that he arrived at GSRP with the mark above the eye. She did not receive an "ouch" report from the school regarding this. She denied that he had either of these injuries when dropped off at Kidcare that morning. I asked if Child A said anything regarding the marks and she said that Child A is not verbal. Child A's mother sent me a picture of Child A's face that was taken on December 9, 2022, by Child A's GSRP teacher and was sent to her. There is an abrasion on Child A's head, just above his right eyebrow. The abrasion was about inch in diameter. 1/2 She also sent two more pictures showing the mark below Child A's nose. This did not appear to be an injury but possible impetigo. Child A's mother did not take him for medical treatment regarding either of these possible injuries. On December 17, 2021, I made an unannounced onsite inspection to the child care center, along with MDHHS worker Erik Woodside. We interviewed owner/program director Lisa Kaiser. She saw the mark on Child A's head but did not recall seeing anything below Child A's nose. She said that Child A's mother came to the center and was very threatening to staff and went up to Child A and pretended she was going to "flick" him in the face with her finger when asked Child A if he had kicked his caregiver. 3 Ms. Kaiser said that Child A is in the Preschool Room. There are usually 16 children and three staff in this classroom. They have the third staff member in the classroom due to Child A's behavior as he will often try to take off out of the room. Accident reports are completed, and parents are notified if a child is injured at the child care. Child A's last day was December 14, 2021. On this day, he kicked and slapped a caregiver, and they contacted his mother to come pick him up. This is when Child A's mother arrived at the center and was threatening staff. She contacted her licensing consultant Lisa Gundry regarding this as Child A's mother was very angry that they would not let him staff at the child care. I confirmed this with Lisa Gundry, who was contacted by Ms. Kaiser who told her what had happened when Child A's mother came up to the center and was concerned that Child A's mother might try to cause trouble as they were going to discharge Child A due to his behavior. The center has cameras, but they do not do a continuous feed as they take still shots throughout the day. She reviewed this video feed for December 9, 2021, and the only thing she witnessed that might have caused an injury was Child A going under a table. A typical day for Child A would be him dropped off at the center between 6:45am and 7:00am and then catching the bus to go to GSRP around 12:00pm. Child A arrives back at the center around 4:30pm and is picked up around 5:00- 5:30pm. She confirmed that Child A is not verbal. While at the center, we interviewed child care staff member Kylie Wilson. Ms. Wilson confirmed that Child A was I her preschool classroom. She did not recall seeing any marks on Child A on Thursday, December 9, 2021, or Friday, December 10, 2021. She did recall seeing Child A with a scrape above his eye that following Monday (December 13, 2021). Discipline is usually time out in the preschool classroom. Child A's mother said that Child A would often have soiled diapers when picked up from child care. She confirmed that Child A is not potty trained and still wears pull ups. She had also received complaints from Davison Schools, where Child A attends GSRP, stating that Child A often arrived there for PM Preschool with a soiled pull up. Child A goes from Kidcare to GSRP at noon Monday through Thursday. Program director Lisa Kaiser said that diaper and pull ups are checked every two hours and changed if needed. Child A's pull up is usually checked and changed around 11a rr daily. They had been doing a "potty chart" for Child A's mother in the last few weeks as she would often complain that Child A's pull ups were not being checked and changed regularly. Child A usually arrives in the morning with a soaked diaper, and they change his pull up right away. Caregiver Kylie Wilson said that diapers and pull ups are checked and changed regularly at the center.
