On January 13, 2022, I interviewed the complaint source. The complaint source outlined several allegations that were beyond the scope of the child care licensing rules and regulations, including concerns related to the care of child by their parent. The complaint source was provided with information for centralized intake, and I discussed mandated reporting requirements with the complaint source. The complaint source also noted concerns regarding COVID-19 that are not covered in the child care licensing rules. I directed the complaint source to contact their local health department regarding COVID-19 concerns. These allegations were not investigated as they are not related to child care licensing rules and regulations. The complaint source indicated that child care staff and children are allowed to be present with communicable diseases, most recently in December of 2021. On January 13, 2022, I completed an unannounced onsite inspection at the facility. I interviewed Hannah Galbraith, child care staff member. Child care children and child care staff members are not allowed to be present at the facility with a communicable disease. Ashley Reif, licensee, sends a message on the Bright Wheel application to inform families when communicable diseases are present at the facility. Ms. Galbraith showed me a message recently sent on Bright Wheel to inform families of a communicable disease present at the facility. Ms. Galbraith does not have any concerns regarding communicable disease reporting or exclusion at the facility. On January 13, 2022, I interviewed Andrea McColman, child care staff member. Ms. McColman is not aware of any communicable diseases present at the facility not being reported to enrolled families. Parents are informed of communicable diseases through the Bright Wheel application. Child care staff have not been asked to stay and work while infected with a communicable disease. Ms. McColman does not have any concerns with communicable disease reporting or exclusion. On January 13, 2022, I interviewed Ashley Reif, licensee. I explained the allegations to Ms. Reif that were beyond the scope of the child care licensing rules and regulations. I advised Ms. Reif that as directed in the Guidelines for Safe Child Care Operations During COVID-19, that child care facilities should work with their local health department regarding regulation of COVID-19 guidelines. Ms. Reif reported that two child care staff members currently have a communicable disease and are not currently working. Parents of enrolled children are informed of communicable diseases present at the facility through the Bright Wheel application. Ms. Reif sent me several examples of notifications sent to parents of enrolled children through Bright Wheel from November of 2021, December of 2021, and January of 2022. Ms. Reif has not received any complaints regarding disease 3 reporting procedures or exclusions. On January 13, 2022, Ms. Reif provided me with a copy of the written policy for the facility detailing when children, staff, and volunteers will be excluded from the center due to illness. The facility has a written policy in place and is adhering to their written policy. On January 18, 2022, I interviewed Child A's Mother. Child A's Mother receives notification of communicable diseases in the Bright Wheel application. If there is a communicable disease you have to follow the exclusion policy. Child A's Mother does not have any concerns regarding communicable disease reporting. On January 25, 2021, I interviewed Tabitha Flitton, child care staff member. Communicable disease reporting is done in the Bright Wheel application. If parents do not read the notifications, Ms. Reif will call parents of enrolled children with the information. Ms. Flitton does not have any concerns regarding communicable disease reporting or exclusion. As of the date of this report, Child B's Mother and Jennifer Lopez-Suess, child care staff member, have not returned telephone contact.
The child care facility is not following their written policy for communicable disease reporting.

On July 16, 2021, a complaint was received regarding Wiggles and Giggles Early Development Center. A child who presented with a physical injury including bruising, disclosed physical abuse to a child care staff member. When the child care staff member told the supervisor and attempted to contact centralized intake, the child care staff member was directed not to report the concerns to centralized intake. Information regarding which specific child, child care staff member and supervisor were involved was not provided in the intake document. On Ju C3 Can July 16, 20.21, I received information from the complaint source regarding the face- to-face visit with Child A's Mother and ■ on July 15, 2021. Child A was dropped off at the facility on July 12, 2021, around . Child A's Mother denied that Child A had a bruise on ■ face when vas dropped off at the facility. Child A's Mother figured it was a scratch and did not touch it until she was able to use water to soften the adhesive at bath time. Child A's Mother contacted Ms. Reif about the injury around 7:50 pm. Child A's Mother told Ms. Reif that Child A said another child hit with a toy. Ms. Reif told Child A's Mother that Child A stated, "smacked Child A would never use the word smacked, she would say "hit." Child A's Mot er denied that the bruise was obtained at home. July , 2021 I com leted an unannounced onsite inspection at the facility with I interviewed Ashley Reif, Licensee :with ~ 0S.. as no present a e facility on July 12, 2021, when Child A arrived. Tabitha Flitton and Hannah Galbraith, child care staff members, were present and providing care. Child A told Ms. Galbraith that" •," Child A had a bruise on ■ smacked left temple near cheek bone. it was described as a small purple bruise and was present on Child A's face at drop off. Child A reported the information on autopilot, it was fast, and never said again on that day. Ms. Reif took a photograph of the injury on July 12, 2021 and provided it to the de artment on July 23, 2021. The bruise is visible on the left side of Child A's face, near temple and cheek bone. The bruise has a round shape and has a darker purple Inse toward the bottom. Ms. Reif had contact with Child A's Mother on the evening of July 12, 2021, at approximatel 7:50 pm, Ms. Reif reported to Child A's Mother that Child A stated, smacked ' . Child A told Child A's Mother was hit with a toy. Child A had a band-aid on face when went home on Ju y 2, 2021. During the day child care children were going throug r a band-aid phase. Multiple children went home with band-aids on their faces. Child A also stated that" hit " on June 21, 2021. The bruise was beginning to fade when Ms. Reif observed the bruise. Child A's Mother told Ms. Reif that Child A's hit with a toy. Ms. Reif denied that she would ever tell anyone not to call centralized inta a to report child abuse and neglect. Child care staff members are trained regarding mandated reporting during orientation. Ms. Reif did not call central intake regarding the statements made by Child A because she did not feel there was enough information to make a report. Ms. Reif is not aware if any other child care staff members called centralized intake. Child B age has never disclosed any concerns to child care staff members. On July 19, 202 and I discussed with Ms. Reif that mandated reporters are not responsible for investigating statements made by child care children regarding abuse and neglect. If there is reason to suspect child abuse and neglect, the child care staff members are mandated reporters and required by law to immediately report child abuse and neglect allegations. On July 19, 2021, I interviewed Tabitha Flitton with Ms.. Flitton was present at the facility on July 12, 2021, and provided care to Child A. She was present when nI Child A was dopped off, but Child A was received at the door by Hannah Galbraith. Child A told Ms. Galbraith that caused the injury.
