On 09/02/2022, I interviewed Child A and B's Mother by telephone. She said Child A is in the Pedal toddler classroom. She has an egg allergy and cannot eat foods that contain egg. On 09/02/2022, she noticed on the Nurture app, the center's app which is used to provide parents with the written daily record, that Child A had eaten 100% of her whole wheat chicken tenders. This is a food on the center provided list of foods containing egg. She called the center and spoke with Ms. Hyndman who informed her that Child A was given and consumed a chicken tender. She had been prepared an alternate meal but was mistakenly given the chicken tender by a child care staff member in the room. Child A did not have an allergic reaction and did not have medical treatment. In August 2022, Child A had a possible allergen exposure to egg when she grabbed another child's ravioli off her plate. Child A did not eat any of the ravioli because the child care staff member removed the ravioli from Child A's hand. Child A was given turkey for lunch when this incident occurred. She believes the error with Child A may be a result of a lack of consistency among child care staff members who work in the classrooms. On 09/06/2022, I reviewed documentation that was emailed to me by Child A and B's Mother. On 08/26/2022, there was an email exchange between Child A and B's Mother and the center regarding Child A's possible allergen exposure and the center provided Child A and B's Mother with a list of food substitutions that Child A is provided when foods with egg are served to the children. On 08/22/2022, there was an email exchange between Child A and B's Mother and Ms. Tumminello explaining the reasons why there are different child care staff members who work in Child A's classroom which included, to cover breaks and to cover other child care staff member's absences. On 09/06/2022, I interviewed Ms. Hyndman and Ms. Tumminello at the center. Ms. Hyndman stated Ms. Chrostowski, who prepares lunch for the classrooms, prepared Child A an alternate meal for lunch that does not contain egg on 09/02/2022. However, Ms. Chrostowski forgot to send the alternate meal to Child A's classroom and Child A was fed whole wheat chicken tenders which contains egg. The center is aware of Child A's egg allergy. Every classroom has a list posted with the children in all of the classrooms who have allergies or require a special diet. The same allergy and special diet list is posted in the kitchen. There is a binder in the kitchen which includes a sheet for food the center serves that lists the ingredients and 3 allergens/warnings. Also, the Nurture app lists a child's allergy or special diet. The center has begun to brainstorm on how they can ensure that children are not exposed to possible allergens and are provided the correct special dietary meals e.g., using special place mats during meals which lists a child's allergy or special diet. On 09/06/2022, I interviewed Ms. Chrostowski at the center. She said she is a child care staff member who works mainly in the kitchen and is not assigned to work in a specific classroom. She prepared Child A a turkey and cheese wrap for lunch and the other children in the classroom were prepared whole wheat chicken tenders. She went to the classroom and served all the children, including Child A, whole wheat chicken tenders. She forgot to put Child A's meal in the bin and mistakenly fed her whole wheat chicken tenders. She indicated she is aware of the children's allergies and special diets because there are allergy and special diet lists posted in the kitchen and in the classrooms. She feels bad that she gave Child A the whole wheat chicken tender, and she has never made this mistake before. On 09/06/2022, I interviewed Ms. Connolly at the center. She said she is the lead caregiver in the Lyric classroom. The Lyric and Pedal classrooms were combined on 09/02/2022. Ms. Connolly did not serve Child A the whole wheat chicken tenders. Child A has an egg allergy with an allergy plan on file with the center. She is restricted from having any egg products. The center provides her meals and they have previously sent the parents a list of foods that contain egg and the substitutes that they provide. On 09/02/2022, Child A was served whole wheat chicken tenders. This is a food on the center provided list of foods containing egg. Child B may have been fed formula instead of breastmilk. He has a milk protein intolerance.

On 09/02/2022, I interviewed Child A and B's Mother by telephone. Child A and B's Mother expressed concern that her son, Child B, who is in Infant 1 classroom, may have been fed formula in July 2022 instead of his breast milk. He has a milk protein intolerance. The Nurture app stated he was fed formula but when she questioned the center about him being fed formula, she was told that the child care staff member made an error by entering formula in the app and that he in fact was fed his breast milk. On 09/06/2022, I reviewed an email exchange between the center and Child A and Child B's Mother on 07/27/2022 and 07/28/2022 regarding the documentation error in the Nurture app for Child B. On 09/06/2022, I interviewed Ms. Hyndman and Ms. Tumminello at the center. Ms. Hyndman stated Child B has never been fed formula and always drinks his breast milk. The child care staff member accidentally clicked formula instead of breast milk when she entered the food intake in the Nurture app. In addition to entering an infant's food intake in the Nurture app, child care staff members also write down the food intake information on a paper log form. Child B's child information card and allergy and special dietary needs form states that he has a milk protein intolerance and cannot be given milk products and formula. He is only to be given breast milk. I reviewed the written record log for food intake that Child B was given in July 2022. He was only fed breast milk.

On 01/14/2021 I completed an unannounced onsite inspection at Gilden Woods Child Development Center. I observed the number of child care staff members to children in all classrooms. The ratios during my observations are as follows: • Otis (school age children); Natalee Whitford (1) to (7) children • Pratt (4- to 5-year-olds); Jessica Mendelsohn and Gabriella Fresquez (2) to (14) children • Terance (3- to 4-year-olds); Tammy Schluchter and Taylor Meredyk (2) to (18) children • Anabella (30 months to 3 years-old); Nakia Ga'lyn (1) to (8) children • Pedals (24 months to 30 months); Shelbie Sutton and Victoria Tumminello (2) to (12) Children • Lyric (18 months to 24 months); Chelsey Elias (1) to (11) children • Farris (12 months to 18 months); Jamie Jonas and Miranda VanLanduyt (2) to (10) children • Infant 1 (6 weeks to 6 months); Madisyn Fultz and Rachel Sablatzky (2) to (6) children • Infant 2 (6 months to 12 months); Megan McKay-Pfeiffer and Aubrey Preiss (2) to (8) children I interviewed child care staff members Tammy Schluchter and Jessica Dixon. Both child care staff members work in the Terance room. Ms. Schulchter stated that staff are asked to take a break while children are sleeping without appropriate coverage. She said as soon as 10 children are asleep, either Ms. Dixon or herself will take a break, leaving the classroom with 1 child care staff member with 20 children. Ms. Schluchter said she was concerned that cots were not spaced adequately apart. She stated she tried multiple configurations to ensure cots were spaced at least 18 inches apart but was unsuccessful. She expressed concern that children and staff would not be able to evacuate during an emergency due to the close spacing of cots. 3 I interviewed Ms. Dixon. She said there is always two staff members in the Terance room during all parts of the day, including nap time. Ms. Dixon stated that there are never more than 20 children in the Terance room at one time. She was unsure about cot spacing and the required distance between cots. I reminded her that cots need to be at least 18 inches apart. I observed cot placement in all classrooms. I visually inspected the distance between cots. I measured cot spacing in classrooms that appeared to have cots that were less than 18 inches apart. To measure cot spacing, I used an 81/2 inch by 11 inch piece of paper. I measured cot spacing in the Terance and Pedal classrooms. Both classrooms had cots that were less than 11 inches apart. The Terance room had two cots that were less than 11 inches apart and the Pedal room had three cots along the wall that were less than 11 inches apart. I observed emergency exit egress in all classrooms. One classroom had a cot placed in front of an exit door. The emergency exit door in the Pratt Room was blocked with a child sleeping on a cot. I interviewed Ms. Davis. She said it is her general procedure to have child care staff members take their lunch break when children are sleeping. She thought once children were asleep that ratio requirements were not applicable. She stated she sends staff on break leaving classrooms out of ratio. This occurs in toddler, preschool, and school-age classrooms. Once I informed her that child care centers are required to maintain ratio during all parts of the day, including rest time, she said she will no longer send staff on break without appropriate staffing in place. Ms. Davis stated she will adjust the schedule immediately to ensure that ratios are always met. On 1/29/2021 I completed an unannounced onsite inspection. I observed child care staff to children ratios in all classrooms. All classrooms were found in compliance with ratio requirements. I reviewed the original licensing report dated: December 13, 2018. The report stated the following: Terance Room which measures 24' x 24' (minus 2' x 9'), 8' x 13' and 8' x 8', for a total of 726 square feet. This accommodates the requested capacity of 20 children over 2 % years old, at 35 square feet per child. The licensee understands that if there is a mixed age group in this room, that the ratio is based on the ratio requirements of the youngest child. On 1/14/2021 I interviewed Terance Room child care staff members, Ms. Schluchter and Ms. Dixon and Program Director, Ms. Davis. Both Ms. Schluchter and Ms. Dixon stated there are up to 20 children allowed in the classroom at one time. Ms. Dixon said that children will move to other rooms to keep capacity at 20 if needed, however that is not a regular occurrence. Ms. Schluchter said that 20 children in the classroom at one time makes it difficult for cots to be spaced appropriately. I interviewed Ms. Davis. She said that the room is licensed for 20 children and that the center never goes over that. If maintaining capacity is a concern, children can be moved to other classrooms if needed. On 1/14/2021, I received and reviewed 10 records for the Terance Room. Attendance records were obtained from 12/28/20 to 01/08/2021. The maximum number of children in this classroom during this time was 18. Two staff members were scheduled to work in the classroom every day.
