On 11/16/2021 Ms. Gentry contacted me to report an incident that involved inappropriate contact between child care staff member, Galina Petrova and Child A. She stated she was informed by Oakland Family Services Human Resource Department on 11/15/2021, that Ms. Petrova pushed Child A down onto his cot every time he moved at naptime, Child A was refused necessary toilet use, and was not allowed to get up from nap after the other children were awake. Ms. Elizabeth Thomas told Ms. Gentry that Ms. Petrova told Child A, "Pee on your cot. You're a baby. I am putting you (Child A) in a diaper." Ms. Gentry stated the incident occurred on 11/10/2021. On 11/17/2021 I received Child A's information record. On 11/18/2021 I received a written incident report, detailing the incident. 3 On 11/19/2021 I conducted an inspection at Children's Learning Center in Walled Lake. I interviewed Ms. Petrova and observed video footage of the incident. I was not able to interview Program Director, Kathryn Chinavare because she had left early for the day. Ms. Gentry informed me that on the date of the incident, classrooms 1 and 6 were combined due to low enrollment. Child care staff members Elizabeth Thomas (room 1, Lead Substitute) and Galina Petrova (room 6, Lead Substitute) worked together in Room 1. There were 9 children ages 3- and 4-years old present. When Ms. Gentry was informed of the incident, she requested video footage from the date of the incident. Ms. Gentry indicated that the footage clearly showed Ms. Petrova place Child A's blanket over his head, grab Child A's arm, reposition him on the cot, push his head down, and at one point, appears to smack him in the head. Ms. Gentry stated that Ms. Petrova's body language was threatening. When rest time was over, Child A remained on his cot. All the other children were putting their blankets away and choosing toys to play with, and Ms. Petrova continued to make Child A remain on his cot. The child was visibly upset. He cried as he watched other children play at the table. Ms. Gentry learned Child Care Staff Member, Elizabeth Thomas told Program Director, Kathryn Chinavare about the inappropriate contact between Ms. Petrova and Child A on 11/10/2021, however, Ms. Chinavare did not report the incident to Ms. Gentry. Ms. Gentry stated that Ms. Chinavare told her that she had not seen the part of the footage where Ms. Petrova physically handled the child and was upset by what she observed. On 11/16/2022 Ms. Gentry verbally informed the department of the allegations. On 11/18/2021 the department received written information regarding the incident. Ms. Gentry stated she contacted Child A's Grandmother on 11/15/2021, 11/16/2021, and 11/17/2021, but had no response. She received a return call on 11/18/2021 from Child A's grandmother. Ms. Gentry informed her of the incident. Child A's grandmother was upset but thanked her for letting her know. On 11/15/2021, Ms. Chinavare said Ms. Gentry informed her of the physical discipline allegations by Ms. Petrova and together they watched video footage of the naptime incident. After watching the video, Ms. Gentry informed Ms. Petrova that she will be placed on administrative leave pending an investigation. Ms. Petrova has not returned since. On 11/19/2021 I reviewed video footage of the incident. There was no audio available. Video Footage Preschool I 1:20 pm — all children laid on their cots. Ms. Petrova lifted Child A's cot up by the corner about 12 to 18 inches off the floor then dropped it to the ground. Child A moved his right arm up and down then turned to his side. Ms. Petrova is turned towards Ms. Thomas. It 4 appears the pair are talking. Child A tried to sit up. Ms. Petrova pushed him with her left arm. She pulled him by his arm to reposition him on his cot. His head hits the cot. 1:28 pm- Ms. Thomas left the classroom (for break) 1:31 pm- Child A tried to sit up. Ms. Petrova pushed the top of his forehead with her hand. He fell back to his cot. On 11/16/2021 I received information that Child A was left unattended in the parking lot. The complainant indicated that Child A ran out of the building and was in the parking lot alone, before being brought back into the building. This incident occurred sometime between October 2021 and November 2021. I was unable to contact the complainant. No contact information was provided. On 11/16/2021 I completed an unannounced onsite inspection. I interviewed Program Director, Ms. Chinavare. She stated on 10/28/2021, Child A ran outside of the building and was in the parking lot. However, Child A was not left alone. Ms. Chinavare said she was informed by child care staff member Alyssa Ms. Senczyszyn (Preschool 6 Lead Staff), that Child A walked out of the building and into the parking lot as the class got ready to go outside. As Ms. Senczyszyn and Ms. Nahas helped children get ready for outside time, Child A walked out of the building. Ms. Senczyszyn observed Child A walk out and followed her. Ms. Senczyszyn did not want Child A to bolt into the parking lot, so she used verbal directives to request Child A return to the building. Child A stood still while Ms. Senczyszyn spoke, but Child A started running away from her as soon as Ms. Senczyszyn took a step toward her. Ms. Senczyszyn chased her and grabbed her. She hugged her for a moment then returned her to the classroom. Ms. Senczyszyn told her that Child A was never left unattended and that she observed Child A walk out of the building and she went after her immediately. Ms. Chinavare stated Child A exhibits avoidance behaviors which had been escalating in the weeks prior. Child A struggles to manage her emotions at times but has never attempted to run out of the building. She has not attempted to run out of the building since. Ms. Chinavare stated she has spoken with Ms. Senczyszyn about the incident and developed strategies to help with Child A's avoidance behaviors, to ensure Child A does not attempt to leave the building again. Ms. Senczyszyn informed Child A's Mother of the incident on the date that it occurred. On 10/28/2021 Ms. Chinavare and the vice president of operations, Kerry Gentry, reviewed video footage of the incident. Both Ms. Chinavare and Ms. Gentry determined that Ms. Senczyszyn always maintained direct sight of Child A. Ms. Chinavare stated since the incidents she has spoken to staff about classroom strategies to ensure all children stay in the classroom. Strategies include moving the cubbies inside the classroom (so children do not need to go out in the hallway to get 3 something from their cubbies), verbal reminders to children that they need to stay in the room and always keeping a direct line of sight on Child A. On 11/16/2021 I interviewed Ms. Senczyszyn. She stated she worked in Preschool 6 on the 10/28/2021. Ms. Senczyszyn stated around 9:25 am on 10/28/2021 Child A left the building and ran into the parking lot. Children were in the hallway by the classroom door getting their coats on. Some children were ready to go outside, and others were not. Ms. Senczyszyn watched children get ready and observed Child A's hand slowly push on the door that led to the playground. Ms. Senczyszyn told Child A to wait until everyone was ready (to open the door). However, Child A pushed the door open anyway and went outside. Ms. Senczyszyn followed her outside. Child A stood in the drive between the playground and building. Ms. Senczyszyn told Child A to "stop," and "come back." Child A did not comply. Ms. Senczyszyn said she told Child A that she will count to 3, and if Child A does not return, she will call Child A's Mother and let her know that Child A is not making a safe choice. Ms. Senczyszyn counted, "1,2,3." When Ms. Senczyszyn got to 3, Child A ran away into the parking lot. Ms. Senczyszyn chased after her and stopped her. She brought Child A back into the building. Ms. Senczyszyn told Child A that she was very scared that she was not being safe. Child A cried. Ms

On 11/16/2021 I received information that Child B walked out of a classroom and was missing for 5 to 10 minutes before being found. Child B was found hiding inside a laundry room, where several cleaning chemicals are stored. This incident occurred sometime between the end of October 2021 and early November 2021. I was unable to contact the complainant. No contact information was provided. The second incident occurred on 11/01/2021. Ms. Chinavare stated Ms. Senczyszyn came into her office around 11:45 am and asked her if she had seen Child B. Ms. Chinavare told Ms. Senczyszyn she had not seen Child B and immediately jumped up out of her chair and began checking classrooms, hallways, and areas outside of the building including the parking lot, playground, a neighboring business, and the general perimeter. However, they could not find Child B. Ms. Chinavare observed Ms. Senczyszyn try and open the laundry room door, but the door did not open, it was locked. Ms. Senczyszyn knocked on the laundry room door and called Child B but did not get a response. Ms. Chinavare stated that she and Ms. Senczyszyn were joined by other staff and the search ensued for around 5 minutes, to no avail. Just as she got ready to call 911 to report Child B missing, she turned around and observed Child B standing in the hallway next to the laundry room. Ms. Chinavare stated she ran down the hallway to get him. Child B told her he was in the laundry room because he wanted to dry his shirt. Child B was reminded that he is not allowed to leave 5 the classroom by himself for any reason and he cannot go into the laundry room. Ms. Chinavare said Child B did not sustain any physical injuries. Ms. Chinavare stated the laundry room door is always supposed to be locked and she was not sure why it wasn't. Ms. Chinavare stated that it is not uncommon for Child B to hide in the classroom. He is an active child and hides in a playful manner. Ms. Chinavare said staff placed bells on the door knob so staff could hear if Child B ran out of the room to hide. Child B's Mother was informed of the incident that same day that it occurred. I interviewed Ms. Senczyszyn. She stated the incident with Child B occurred on 11/01/2021 during morning worktime around 11:45 am. She said Child B painted at the easel then started painting the drying rack and floor. Ms. Senczyszyn told Child B to paint on paper, not on the drying rack and floor. Child B helped her clean up. In the process of cleaning, Child B's shirt got wet. Ms. Senczyszyn told him to get a clean shirt from his cubby in the hallway. Child B walked out of the room. Ms. Senczyszyn went to the sink and washed and dried her hands. She observed that Child B had not returned to the classroom, so she walked out of the room to check the cubby area. She did not see him. Ms. Senczyszyn yelled for Child B and asked Ms. Nahas if she saw him anywhere. She had not. Ms. Senczyszyn began checking other classrooms and told Ms. Chinavare that she could not find Child B. Ms. Senczyszyn said that she, Ms. Chinavare, and other staff looked inside and outside of the center. She recalled knocking on the laundry room door and calling out to Child B but received no response. A short time later, Ms. Chinavare found Child B outside the laundry room and returned him to class. Ms. Senczyszyn stated that Child B is a "runner," and that it was common for him to hide in other rooms around the center. Ms. Senczyszyn stated that since the incident she moved the cubbies inside the classroom and has put up a gate to prevent Child B from escaping the room. Ms. Senczyszyn stated she told Child B's Mother about the incident the date it occurred. On 11/16/2021 I observed the laundry room at the center. The laundry room is a hallway adjacent to the preschool room 6 hallway. The laundry room hallway and preschool room 6 hallway meet about 10 feet from the preschool room 6 classroom.
