On 3/14/22, I went to the center. I interviewed Jennifer Shelby, program director. She showed me two of the bathrooms that the children use sometimes while at the center. I did not observe them to be dirty even though it was 2 the middle of the day. Ms. Shelby does not have a concern regarding the cleanliness of the bathrooms or the cleaning schedule. Ms. Shelby took me down another hallway in the child care center to speak with licensee/designee, Andrea Secrease. Ms. Secrease then showed me the two main bathrooms that the children use during care. The bathrooms were being used by several children while I observed for cleanliness. There were a couple of pieces of paper towels on the floor but it was clean otherwise. I did not observe any smell, mold, dried feces or urine on the toilets or the floors. Ms. Secrease said that the custodian works between two buildings. He is cleaning the bathrooms on Mondays, Wednesdays, and Fridays. On Tuesdays and Thursdays, the staff are taking turns in emptying trash cans and wiping down toilets and sinks. She does not have a concern regarding the cleanliness of the bathrooms. They are currently going through a change and a new custodian will be starting soon. The new custodian will be cleaning the bathrooms daily. I interviewed the following child care staff members: Nataliegh Adkins, Tabitha Fike, Melaine Sasse, Kammi Penny, Marylou Holsinger, Mattea Goffnett, and Kennedi Korpal and none of them were concerned about the cleanliness of the bathrooms.
The custodian is not keeping up with cleaning the bathrooms.

On 01/05/2021, I spoke with Executive Director, Andrea Secrease about the incident. She was onsite when two child care staff members left Child A (female, 3 years) on the playground, unattended for approximately 15 2 minutes. Another child care staff member found her outside within the fenced-in play area and brought her back inside. Jesse Taylor and Jaedyn Myers, child care staff members took their class outside yesterday afternoon, 01/04/2021. Both staff members inadvertently left Child A outside in the fenced-in play area and returned to their classroom. Another caregiver, Trevor Schrank heard her outside and brought her back inside of the building to her class approximately 15-20 minutes later. One of the immediate plans is to let both caregivers go. They will also follow up with executing plans to ensure appropriate supervision always takes place. On 01/06/2021, I had an onsite visit and met with Jennifer Shelby, program director. I received a copy of the attendance and child information cards. There was a total of 10 children present in the afternoon along with two child care staff members. They have already implemented some new changes to keep track of children, and have had a staff meeting to reiterate those changes. On 01/06/2021, I also spoke with Trevor Schrank, child care staff member. Mr. Schrank works with the after school program. He was in the cafeteria and cleaning up after snack. The window was cracked opened and he heard a child making noise. He did not see any staff outside but opened his door and saw Child A standing outside in the fenced-in play area. Mr. Schrank reports Child A was emotional. He took her back to her classroom. Mr. Schrank is not exactly sure of the time, but knows it was after snack, which is 3:30pm for his classroom so he estimates it was about 4:00 or a little later, when he saw her. On 01/11/2021, I spoke to Child A (female, 3 years) and Child B's (female, 1.5 years) mother. She did not have any issues or concerns. On 01/11/2021, I spoke to Child C's (female, 3.5 years) mother. On 01/12/2021, I also spoke with Child D's (male, 3 years old), mother. Neither parent had any concerns or issues. On 01/12/2021, I spoke with Jessie Taylor, past lead teacher. On the afternoon of 01/04/2021, she and Jaedyn Myers, another child care staff member had taken her classroom outside. When they were getting ready to come inside, they lined the children up to count them and Child A was lagging behind. Ms. Taylor was at the front of the line, while Ms. Myers was at the back of the line. After counting all the children, they walked into the building. Some of the children walked past her when she opened the door so she was trying to keep an eye on them. They stopped inside to take off their outdoor clothing and go to the bathroom. Ms. Taylor did not realize she did not count the children when she entered the building. Additionally, a child was picked up by their parents, and she became distracted. She also knew a child was in the bathroom with Ms. Myers. They went into the classroom and were there for a couple of minutes when Mr. Schrank brought Child A back to the classroom. He told her he found her alone outside. She had not been crying. Ms. Taylor thought Child A may have been outside about 15-20 minutes by herself. Ms. Taylor was unaware 3 that they left Child A outside. On 01/12/2021, I spoke with Jaedyn Myers, past child care staff member who stated she was one of the child care staff members present on 01/04/2021, when Child A was left on the playground. The lead teacher was Ms. Taylor. They had 10 children in care and had been outside on the fenced-in playground. They came inside around 4:00pm. They normally line up at the door and count. Child A was at the front of the line and then everyone went inside. Ms. Myers did have to speak with another child briefly because they were upset about having to come back inside. When they got inside, they took their coats and boots off and then had a bathroom
