During the onsite inspection on 02/14/2023, the child care staff members did not ensure that the children wash their hands before their snack time.

During the afternoon SACC program, Child A had asked multiple times to use the restroom because she had to use the toilet and was denied access by the child care staff members because the children were still in a "time out." As a result of depriving Child A from using the toilet, Child A accidently urinated in her Dants.

Child A was denied access to the restroom when she asked multiple times and had an accident where she urinated in her pants in front of her peers. Child A was upset and felt embarrassed that she had this accident in front of her classmates. This accident could have been prevented if child care staff permitted Child A access to the restroom.
