On 8/14/2024, CCSM-1 appropriately cared and supervised Child A and Child B. When Child A attempted to touch Child B in an inappropriate manner, CCSM-1 immediately stopped the incident and reported it to the PD. Child A had no past history of inappropriate touching.

On 12/12/2022 I conducted an unannounced on-site inspection. I interviewed Tammy Bailey (program director/license designee). Ms. Bailey was privately interviewed in the staff breakroom. Ms. Bailey stated that on 12/08/2022, she was approached by Ashley Smith (lead toddler caregiver), who informed her that Child A's mother had told her that on 12/02/2022, Ms. Childs had hit Child A after Child A had struck her in the face. Ms. Bailey spoke with Child A's mother on 12/08/2022. Child A's mother told her that during pickup time on 12/02/2022, Ms. 2 Childs told her that Child A had just hit her and that she in turn hit Child A back. However, when Ms. Bailey spoke with Ms. Childs, Ms. Childs denied hitting Child A and stated that she merely "waved her hand away" when Child A hit her that day. Ms. Bailey stated that she was very surprised that Ms. Childs would have done anything like this, and that she had no past concerns with Ms. Childs' behavior in the classroom. Ms. Bailey placed Ms. Childs on administrative leave and then terminated her employment later that day. Ms. Bailey stated that all staff are trained on the center's discipline policy, which prohibits the use of any form of corporal punishment. The two staff members that were in the classroom at the time of the incident (estimated to be around 5:00pm), were Ms. Childs and Amber Ellery (lead 3-year-old caregiver). At that point in the day the 3-year-old and 4-year-old classrooms were blended due to the number of children in attendance. Ms. Ellery and Ms. Smith were not present at the center during the on-site inspection. While on-site I viewed the preschool classroom which is a large room split into two separate areas, a 3-year-old classroom and a 4-year-old classroom. I viewed the center's discipline policy, which states that no form of physical discipline can be utilized and that they only use positive discipline techniques in the classrooms. Ms. Bailey stated that she retrained her entire staff on the center's discipline policy on 12/09/2022, after the reported incident had been reported to her. Ms. Bailey will continue to monitor the classrooms and assist caregivers with any children that exhibit challenging behavior. Ms. Bailey stated that she left a voicemail and emailed an incident report to her assigned licensing consultant on 12/08/2022. On 12/12/2022 I spoke with Child A's mother who stated that on 12/02/2022 at around 5:40pm she arrived at the preschool classroom to pick up Child A. Ms. Childs approached her and stated that Child A had punched her in the face, so she "reacted" and "bopped" her back but did not specify where she had struck Child A. Ms. Childs apologized to Child A's mother and said she did not mean to do it but that it was just a reaction when she was hit in the face. Child A's mother got upset and told Ms. Childs to never to touch Child A again. Child A's mother stated that she had never had past concerns with Ms. Childs but was concerned that she felt like she could physically hit her child. Child A's mother spoke with Ashley Smith on 12/08/2022 about the incident and was advised that she should speak with Ms. Bailey. Child A's mother met with Ms. Bailey on 12/08/2022 and informed her of the incident and her concerns. Child A's mother said that she attempted to speak to Child A about the incident, but due to Child A's age and speech delay, she was not able to get a clear understanding of what had occurred. Child A's mother stated that she did not see any marks or bruises on Child A after the incident on 12/02/2022. On 02/08/2023 I interviewed Ashley Smith (lead toddler caregiver) via telephone. Ms. Smith stated that she was approached by Child A's mother on 12/08/2022. Child A's mother told her that on 12/02/2022 Ms. Childs had struck Child A because Child A had hit her in the face. Ms. Smith informed her that she needed to report this to Ms. Bailey immediately. Ms. Smith stated that she had never had any past concerns with Ms. Childs and denied
