06114/20221 received an incident report from Gateway Country Day School. The incident report indicated Child A was standing by the climber in the classroom when she fell backwards to her bottom and began to cry. Child A was unable to place 2 On 06/16/2022 l conducted an unannounced onsite inspection. I interviewed Lead Toddler Prep Staff Member, Trudy Houle, Assistant Toddler Prep Member, Elizabeth Granda, and Licensee Lynn Piontkowski-Bilski. I was informed that Child A is enrolled in the Toddler Prep Room. This room is designated for children ages 12 months through 18 months. Ms. Houle works 8 am to 5 pm, Monday through Friday in the Toddler Prep Room. She stated on 06/13/2022 around 4:50 pm, she cared for 4 children in the Toddler Prep Room by herself. She observed Child A run towards the small climbing structure in the classroom. Ms. Houle stated that Child A lifted both her hands above her head and pushed them against the climber. She watched Child A fall on her bottom to the floor. Child A cried loudly. Ms. Houle ran to her and tried to lift her off the floor by gently placing her hands under Child A's armpits and lifting up. Child A bent her left leg up off the floor and would not apply pressure. Ms. Houle said she called the administrative office right away. This is always the procedure when a child is hurt at the center. A member of the administrative team arrived and brought Child A to Ms. Piontkowski-Bilski. Child A's Father arrived around 5pm and was informed of the incident. Video footage was not available. There are no cameras in the classroom. Ms. Houle stated that Child A is a very sweet and active child. She is the kind of child who is always moving around. Ms. Houle said Child A had no injuries to her left leg prior to the incident. She did not observe any falls or other incidents that may have caused Child A's leg to break. I interviewed Ms. Granda. She works 7 am to 4 pm, Monday through Friday in the Toddler Prep Room. She will work later if needed to meet ratio requirements. Ms. Granda stated she was not present during the incident with Child A. She had already left for the day. Ms. Granda said she did not observe any falls or injuries to Child A prior to her departure. Child A was walking around fine throughout the day. She did not notice Child A cry or appear to be in pain at any time. She said she has worked with Ms. Houle for the last several months and does not have any concerns with Ms. Houle's behavior or interactions with children. I spoke with Ms. Piontkowski-Bilski. She stated that Child A was brought to her on 06,11312022. She observed that Child A was unable to bear weight on her left leg. She held her and talked to her. When Child A's Father arrived, she informed him of the incident. The following morning Child A's Mother emailed the center. The email Ms. Piontkowski-Bilski said that Ms. Houle has worked with the center for years. She has not had any concerns regarding Ms. Houle's behavior or interactions with children. Ms. Houle has not had any disciplinary action in her time employed at the center. I reviewed Ms. Houle's employee file. She received training on child handling, child protection, and preventing and reporting child abuse and neglect through MiRegistry and signed an abuse and neglect statement on 0110712005. On 06116/2022 I spoke with Child A's Mother. She stated that Child A has been enrolled in the Toddler Prep Room since . She said Child A very active. She will climb on top of everything e is"noclosely observed. Child A Mother stated that Child A Uniia As mother statea that no one at the center was clear on how the injury occurred and felt here was some inconsistency with what occurred and the injury the child had. However, she had no concerns with the center and has been satisfied with the care provided. 4 On 08,3112022 1 informed Ms. Ms. Piontkowski-Bilski by email that there were no rule violations and the case will be closed.
