On 04/06/2023, I completed an unannounced onsite inspection. I spoke with program director Wendy Monahan, licensee designee Nanci Canine, and Child Care Staff Members (CCSM) 1, 2, and 3. I spoke with Ms. Wendy Monahan, who denied any knowledge of infants being allowed to sleep in the swingers. She shared there are times when a child has fallen asleep in the swingers, but they are removed and placed in their cribs. The room has some children who like to "wind down" with the swingers, and for some children, they help to relax them. She shared, "I'm well aware of the safe sleep rules" and practicing safe sleep is something the staff are trained on and something she takes seriously. I spoke with licensee designee Nanci Canine who arrived at the facility during my onsite inspection. She was made aware of the allegations. She denied having any knowledge of staff allowing any of the children enrolled to sleep in unapproved equipment. She shared Ms. Monahan does a good job with the staff, and she did not believe she would have knowledge of safe sleep concerns and ignore them. I spoke with CCSM 1. She shared she's worked in the infant classroom for over a year. When asked about sleeping arrangements for the children she reported the children sleeps in their cribs. She denied ever witnessing any children sleeping in the swinger. There has been times when a child has fallen asleep in the swinger, but as soon as a staff see it, they will move the child to the crib. She shared some of the children enjoy sitting in the swingers and will fall asleep in it, but, again, shared this is when the children are removed. She denied observing any other staff allowing a child to sleep in the swinger without immediately removing them. I spoke with CCSM 2. She confirmed working in the infant classroom and shared children are only allowed to sleep in their assigned cribs. She shared there has been times when a child has fallen asleep outside of the crib, but they are always immediately moved. She denied ever observing any staff member allow a child to sleep in a swinger and shared that, even if she observed it, she would have immediately moved the child to the crib. I spoke with CCSM 3. She currently works in the infant classroom and denied that she has observed infants being allowed to sleep in unapproved equipment. She said she's aware of the safe sleep requirements, and as a staff member would address this as a concern if observed. On 04/06/2023, I completed an observation of the infant and toddler classroom. As I was walking into the room, CCSM 1 was picking a sleeping child up from the swing and placing the child in the crib. She shared the infant had just fallen asleep and was 3 immediately removed from the crib. There were 11 children present and 12 cribs. I observed the cribs to be in good condition and with fitted sheets. I did not observe any concerns regarding safe sleep. On 04/27/2023, I spoke with Child A's parent. When asked about safe sleep practices, she shared there have been times when she observed the room from the parent app, and it has looked like a child was sleeping in a swing. She denied ever observing this in person and said, "I do believe I've seen it on camera." On 04/27/2023, I spoke with Child B's parent. She denied any safe sleep concerns. She shared her child has been enrolled for several months, and during pickups and drop offs, she's never observed any children sleeping in swingers nor has she had any concerns. On 04/27/2023, I spoke with Child C's mother. She shared she's never had safe sleep concerns for her child; however, she did confirm on at least two occasions during her pickup of Child C, she has observed a child sleeping in a swinger. She shared her pickups are typically quick and she doesn't stay long, so she wasn't sure how long the child was in the swinger sleeping prior to her visit or if they remained when she left. On 04/06/2023, I spoke with Wendy Monahan about the allegation. She reported 13 children as being enrolled in the classroom and 12 cribs total. She shared there's typically 11 children that come daily and two children that come on opposite days and share the additional crib, which is changed between use. She shared her staff are well aware of this licensing rule requirement, and she feels confident that they've been following it. I spoke with CCSM 1. She shared each child has their own crib with the exception of two children who share a crib but attend on different days. She shared the sheets are always changed in between uses, and she's never observed any other CCSM not changing the sheets prior to the other child using the crib the following day. I spoke with CCSM 2. She shared she's worked in the infant room for a while and shared the bedding is always clean and changed if used by another child. The sheets are changed every Friday. I spoke with CCSM 3. She denied having any concerns with bedding not being changed between use by children. She shared the sheets are changed, and due to having a child enrolled in the infant classroom if she had a concern regarding this, she would have addressed it with Ms. Monahan. She went on to share, "I have no concerns with the way the infant room is being ran."

On 04/06/2023, 1 spoke with Ms. Monahan who walked me to each of the classrooms to confirm ratio. Each of the rooms were within ratio and she denied any of the rooms had mixed age groups. Ms. Monahan denied that the facility was even in operation during Spring Break. She shared the building (which was observed) operates with the younger children on one side and the older (preschool) on the other side. There is never a time when the two age groups would mix. Infants and toddlers are never mixed with preschoolers including at the start of the day and later during the end of business when the numbers have dropped. I spoke with Ms. Canine who also confirmed the facility did not operate during Spring Break and specifically provided March 27-March 31 as the dates they were closed. On 05/08/2023, I completed an exit conference with Ms. Nanci Canine. I informed Ms. Canine of the findings of the investigation and an opportunity for questions. Ms. Canine denied having any concerns or questions.

Sometimes staff in the early years, 6 weeks to 30 months, have a 1:5 or 1:6 ratio. I interviewed the owner Nanci Canine, the director Wendy Monahan, and caregivers Nakenya Williams, Aubrey Begley, and Cheryl Corrion, who all work with this age group. I interviewed them individually, but they all said essentially the same information. They all stated there are no ratio issues. They also stated if a classroom does go out of ratio, Ms. Monahan, Ms. Canine, or Ms. Corrion are notified and they come to help or they move staff or children around to ensure all rooms are in ratio. Ms. Canine stated staff arrive at the center at 7:00 am, 7:15 am, 7:30 am, 8:00 am, 8:15 am, 8:30 am, and 9:00 am to ensure all rooms have the correct staff coverage daily. Staff member Monica Thelen stated the baby room has been out of ratio a time or two, but not for very long. When this happens, she lets Ms. Monahan or Ms. Corrion know and they will come and help or move staff around. I observed the baby and transition classrooms (6 weeks to 30 months rooms), and they were in ratio. The baby room had 8 children and 2 staff. The transition room had 10 children and 3 staff.

They all stated that the toddler children do not climb on the shelves and that they do not touch the air purifier. They stated staff supervise the children to ensure those things do not happen. I did observe the air purifier on a lower shelf in the toddler room and had them move it while I was there. As for the barrier gate between the babies and transitions area, they have a nice wooden gate in that area. The staff stated it is relatively new within a month or so. I observed it to be easy to tip over. Ms. Canine, Ms. Monahan, and Ms. Corrion stated they have ordered weights that go with the gate, but they have not arrived yet. Ms. Begley stated the gate fell over once because a child pushed it. The child was not hurt, and they are waiting on weights for it to keep it from being able to tip easily.
