During the inspection I observed that the outdoor play structure with the "firemen's pole" had the pole separated from the main anchor pole with no barrier around it to stop children from playing on the structure. The cushioning material had a large hole, measuring in diameter more than 12 inches, that exposed hard packed dirt.
