On May 17, 2021, I received an incident report from Program Director Laura Anderson self-reporting that Child A had been left outside when a group of children was coming inside on May 14, 2021, at approximately 6:15PM. On May 20, 2021, I spoke with Ms. Anderson over the telephone to receive a more detailed explanation than the incident report provided. She stated that during the late afternoon of May 14, 2021, the children were outside playing. As more children were being picked up to leave for the day Child Care Staff Members (CCSM) were 2 condensing age groups by putting the remaining children together in one mixed age group. This was all taking place outside on the playground in the rear of the building. At approximately 5:20PM, CCSM Haley Day released what was left of her group to CCSM Julie Wysocki. Ms. Wysocki remained outside with the now larger group of children. Ms. Day left the group and went into the building. At approximately 5:30PM, Ms. Wysocki brought the children into the building to the preschool room. Around the same time Ms. Day came back to the preschool room and saw Child A through the window outside on the playground. Ms. Day immediately went out and brought Child A back into the building to the school aged room with Ms. Wysocki. Ms. Anderson stated that the parents were notified when they picked up Child A from care that same day. Ms. Wysocki was reprimanded and put on a final written warning. On May 25, 2021, I conducted an unannounced inspection to Childtime. Upon arrival I met with CCSM Team Leader Alexis Renock who repeated the details Ms. Anderson provided on May 18, 2021. Child A had been left outside on the playground when the group of children came back into the building. She said Ms. Anderson does not believe that Child A was out there for more than a few minutes but did not have an exact time. She was also unsure when or if Ms. Wysocki had completed name to face counts of the children before entering or when entered the building. There was a total of seven children left in attendance when the incident occurred. I interviewed CCSM Julie Wysocki in the office. Ms. Wysocki has been employed at the center for 15 years in the capacity of an assistant caregiver. Her work days and times vary. Ms. Wysocki explained her actions and thoughts about what had occurred on May 14, 2021. First, she said that because of the new procedures put in place because of the pandemic the center does not always run smoothly. On that day Ms. Wysocki went to the preschool class to close the door to the hallway. She then went outside to join the preschool and early preschool group. She stated those two groups were already joined together when she went outside. She does not remember anyone telling her how many kids she had at that time. The group was outside for another half hour from the time she came outside. She admitted she did not do a name to face check when she was outside when she first came out. During that time children in the group were being picked up by parents and leaving the group. She was in charge at that time because Ms. Day had left. Ms. Wysocki did not remember if Ms. Day gave her a list of children. During that same time the Pre-K children joined her group. Again, she did not remember if anyone gave her a total number of children present. She did have a list of children. She did not remember how she got the list. 3 Children were released when either Ms. Anderson or Ms. Renock came to the preschool door to get them and take them to the front door to their parent or whoever was picking up. Ms. Anderson and Ms. Renock were signing children out and could see the numbers in attendance. When it was time for the group to go into the building she had all the children line up against the wall in the first playground. She did a name to face count of 8 or 10 children, and then opened a closed gate that led to a second playground. She closed the gate behind the last child.
