  Staff did not provide appropriate care and supervision of children at all times. Child A has special needs, is non-verbal, does not respond to her name when called, and does not have the ability to understand dangerous situations. The licensee designee and the teachers were aware that Child A was known to wander away from adults and that she required additional supervision. At the end of the program day on 9/10/2025, Child A exited the Pre-K classroom and school building without teachers noticing. She crossed through the busy parking lot and driveway and went into the wooded property adjacent to the school. When teachers noticed Child A was missing approximately 4 minutes after she exited, they did an “all call” to alert building personnel. Witness 1 found Child A unattended in the wooded area approximately 164 feet from the exit doors 10 minutes after she left the building.
