On 08/15/2022, Child A (3-year-old, male) was found naked in the playhouse during outdoor play by child care staff member Patricia Blackwell-Seese. Child C (6-year-old, male) was present in the playhouse. Child A reported that Child C directed him to take his clothes off. Child C disclosed to his guardian that he touched Child A's= during this incident. Additional allegations were received on 08/18/2022 stating that on 0811612022, Child A (3-year-old, male) was left outside unattended by child care staff member Shayla Clarke. Ms. Clarke ran outside and found Child A hiding behind a car crying in the parking lot.

On 08/15/2022, I received an incident report from Northern Explorers Child Development Center. It stated that Child A was found naked in the play house with Child C during outdoor play. Child A reported to child care staff member (CCSM) Ms. Patricia Blackwell-Seese that Child C directed him to take his clothes off. Child C later reported to his guardian that he touched Child A's= during the incident. On 08/18/2022, I made an unannounced visit to the Northern Explorers center from 09:33 AM to 12:02 PM. While onsite, I interviewed the licensee designee/program director Ms. Rachel McDonough-Grogan and CCSM Ms. Patricia Blackwell-Seese. Ms. McDonough-Grogan stated on 08/15/2022 around 04:30 PM, Ms. Blackwell- Seese found Child A naked with Child C in the playhouse located in the large outdoor play area. Ms. Blackwell-Seese reported the incident to both Child A, B, and J's Guardian and Child C's Guardian. Child C reported to his guardian that he had touched Child during the incident. Ms. Blackwell-Seese reported the incident to her Aon'sP/2022. I inquired about where Ms. Blackwell-Seese was when this incident occurred. Ms. McDonough-Grogan indicated that a child was in the process of being picked up by a parent, so Ms. Blackwell-Seese was letting the child out of the outdoor play area gate. Ten children were present in the outdoor play area with Ms. Blackwell-Seese when this incident occurred. No other child care staff members were present. She discussed Child A having a tendency of taking off his pants, underwear, socks, and shoes when he uses the restroom. She did not know if this was a factor into why he took his clothes off. Ms. Blackwell-Seese stated on 08/15/2022 around 04:40 PM during outdoor play, Child E (4-year-old, female) informed her that Child A is naked in the playhouse. She was standing by the gate talking to a parent who was in the process of picking up their child when Child E informed her of this. She found Child A completely naked in the playhouse. Child C was with Child A, but he was fully clothed. Child C was found sitting down on a log away from Child A in the playhouse. She asked Child A why he was naked, and he said that Child C told him to take his clothes off. Child C denied 7 telling this to her. She believes that Child A and Child C were in the playhouse for less than 5 minutes before she made contact with them. She talked to both Child A's Guardians and Child C's Guardian's about the incident at pick up. She too discussed Child A having a history of taking off his clothes when he has used the restroom. Ms. Blackwell-Seese denied an incident like this occurring before with Child A and Child C. I took pictures of all the child information cards in this classroom and then I took pictures of the child information cards in the school age classroom. On 08/25/2022, 1 made an unannounced visit to the Northern Explorers We interviewed CCSMs Ms. Aila Johnson, Ms. Madison ouch, and Ms. Emily Hinkley. Ms. Johnson, Ms. Rouch, and Ms. Hinkley were not present for the incident involving Child A and Child C during outdoor play. They had no direct knowledge or relevant information to share on the incident other than Child A was found naked in the outdoor playhouse during outdoor play. On 09,0112022 Clarke On 09;1412022, I interviewed Child A, B, and J's Guardian. The center"nnoot ified her that Child A was found naked on the playground in a wooden playhouse. When found by Ms. Blackwell-Seese, it was alleged that Child A was laughing with Child C. She clarified that she was unsure if Ms. Blackwell-Seese found Child A in the outdoor play house or if it was another child care staff member. The center informed her Child C disclosed to his guardian that he touched Child A's back and privates during the incident. On 09/14/2022, I interviewed Child H and I's Mother, Child K's Mother, CNld L, M, an s Mother, and Child P's Mother. They all denied having any concerns with the center's ability to provide care and supervision. On

On 08/18/2022, I made an unannounced visit to the Northern Explorers center from 00:33 AM to 12:02 PM. I met outside to discuss the investigation. While outside, we watched the Cedar c assroom transition to the large outdoor play area. We observed children running out of the classroom across the parking lot without child care staff members guiding them in a line. CCSM Ms. Patricia Blackwell-Seese was the first staff member to come out of the building. Children were ahead and behind Ms. Blackwell-Seese. She was not actively supervising the children during this transition because her back was to several children crossing the parking lot. CCSM Ms. Alla Johnson was standing by the door to the building looking in the building while children came out. At one point, neither Ms. Johnson nor Ms. Blackwell-Seese were looking at the children crossing over the parking lot to the outdoor play area. Ms. Johnson then started walking out towards the outdoor play area and there was a child behind her. Neither child care staff member observed this child walking across the parking lot. ~ Before leaving the center, I discussed transitions from the classroom to outdoor play with s. McDonough-Grogan. We discussed what we had seen earlier in the day with children running out of the classroom across the parking lot without child care staff members guiding the children. We specifically discussed how chaotic and unsafe the transition was and I provided guidance on how to organize these transitions to ensure child safety. Ms. McDonough-Grogan reported having no additional concerns regarding care and supervision at the center. On 0812212022, 1 interviewed CCSM Ms. Patricia Blackwell-Seese. On 08/16/2022, she was in the office with Ms. McDonough-Grogan working on the incident report involving Child A and Child C. They had just met with Child A, B, and J's Guardian. Child A. B. and J's Guardian came back to the office and informed them that when she went to pick up Child A, Child A was outside by himself crying by the back door. This incident occurred around 5:10 PM. Ms. Clarke was the child care staff member present when this occurred. Ms. Blackwell-Seese did not speak to Ms. Clarke regarding this incident. She stated that by the time she was done with the incident report, everyone had left for the day. I inquired about why she did not say anything to us during the interview when I inquired about any other care and supervision ii;] issues at the center. She stated that she was just so focused on the interview and the incident with Child F that she did not think about it. On 08/22/2022, I contacted Ms. McDonough-Grogan. She stated that she is aware of the incident involving Child A being left outside after his class transitioned into the center after outdoor play. Child A was left outside by the door leading to the classroom. She meant to speak to Ms. Clarke about the incident, but she did inform her that she needs to be counting children during transitions. Ms. McDonough- Grogan was in a classroom, and she stated that she could not hear me, so I asked her to call me later when she has a moment. I informed her that I need an incident report on this matter. On 08/22/2022, I received an incident report stating on 08/16/2022, Child A, B, and J's Guardian saw Child A crying outside alone. Ms. Clarke went outside and found Child A crying by the back door. On 08/25/2022, I made an unannounced visit to the Northern Explorers . We interviewed licensee designee/program director Ms. Rachel McDonough-Grogan, CCSMs Ms. Maddison Rouch, Ms. Alla Johnson, and Ms.Emily Hinkley and Child D. Ms. McDonough-Grogan stated she reached to Ms. Clarke to discuss her leaving Child A unsupervised on 08/16/2022, but she has not heard back from her. She identified Ms. Clarke as being the only staff member present with Child A during the incident. She had been meeting with Child A, B, and J's Guardian when the incident occurred. After the meeting, Child A, B, and J's Guardian returned
