On Ucted an onsite inspection to interview licensee Kristina Prevo. on 04/27/2023, Ms. Prevo was sitting in the gray rocking chair in her living room with Child A around 12:15 PM. Child A was sitting next to her on the chair. Child A's bottle dropped and when she reached for the bottle, Child A fell out of the chair and She denied having an arm around Child A or e held and comforted Child A after he fell. She observed that he could move his arms, sit, and stand with no issues, however Child A would whine when he was picked up or attempted to crawl. She contacted Child A's Mother to report Child A's fall. Child A's Mother stated that she would get him if needed. Since Child A was moving his arms, Child A remained in Ms. Prevo's care for the remainder of the day. Ms. Prevo held Child A until he fell asleep, and he slept until Child A's Father picked him up around 3:10 PM. asked Ms. Prevo if Child A had any restrictions due to his injury and she stated no. Child A's Mother took Child A to the urgent care after he was picked up on 04/27/2023. Urgent care initially assumed Child A had Child A 3 was inconsolable that night. so Child A's Mother took Child A to his primary doctor on 04/28/2023 where it was determined he had While we were speaking to Ms. Prevo, she placed Child A on on the floor in the corner of theoffivin room. Two other children began ig ing over magnet tiles next to Child A. and I went and stood in front of Child A in an effort to prevent the children ro hitting or falling on Child A. We discussed placing Child A in a safe location until his since we observed active children in her care asked s. Prevo to reenact the incident, placed a doll in a chair. Ms. Prevo pushed the baby doll on floor and the doll flopped on the floor, asked if Child A had hit his head when he fell, and Ms. Prevo stated that she assumed that he did. While onsite, I took two pictures of the gray recliner chair, child attendance, child information cards, serious accident injury plan, and emergency postings. I discussed the daily schedule and routine to assist with enforcing structure in the child care. Before leaving, Ms. Prevo confirmed that Child A's Mother is her first cousin and Child A is her cousin once removed. On 05/02/2023, and I interviewed Child A's Mother. On 04/27/2023, she received a call from Ms. Prevo around 12:30 PM informing her that Child A had fallen and hit his head. Ms. Prevo stated that Child A had fallen out of a chair, but she is not sure how that happened. She asked if she should pick Child A up, but Child A was moving his arm, so she did not go and pick him up. Child A consistently falls after pulling himself on objects and she was not concerned about his falling. Child A's Father picked Child A up from child care and took him to swimming. Child A ended up not swimming and she met them at the home where they proceeded to take Child A to the . At the emergency room, the doctor felt that Child A had a strained muscle. That night Child A would not sleep and continuously cried. He would only not cry when she held him, and she ended up sleeping on the floor that night. The next day on 04/28/2023, she called Child A's doctor's office around 8 AM to get Child A into their clinic. Child A saw and Pheere erred Child A for an x-ray at the hospital.
