On 04/24/2023, I interviewed the complainant. The complainant mentioned Justin Moffat recently served as child care staff member at Cheyenne's Mott's child care even though he is not a child care staff member. On 04/24/2023, I interviewed Child A's Mother. Justin Moffat has cared for the child care children for an hour or so when Ms. Mott has appointments. She unsure whether he is a child care staff member. On 04/25/2023, I conducted an unannounced onsite inspection at Cheyenne Mott's home. I also reviewed Mr. Moffat's paperwork to identify whether he met child care staff member requirements. I reviewed the allegations with Ms. Mott. She confirmed that Mr. Justin Moffat worked as a child care staff member on 03/28/2023 from 4:30- 5:30 PM. Ms. Mott did not document her absence on her absence log for 03/28/2023 and Mr. Moffat was not documented on staff attendance. Mr. Moffat also has not completed the health and safety trainings, infant safe sleep, and an assistant caregiver statement form. Ms. Mott stated that Mr. Moffat has a valid CPR and first Aid certification, but she did not have it onsite for me to view it. She agreed to send it to me. Ms. Mott reported that she normally closes on days she cannot be at the child care, but she thought Mr. Moffat could fill in for her on occasion. She denied any other adults working at the child care as a staff member. Before I left, I discussed her being truthful during the investigation and I went over the general guidelines of the special investigation. On 04/25/2023 and 04/26/2023, I interviewed Child B's Mother, Child C's Mother, Child D's Mother, Child E's Mother, Child F and G's Mother, and Child H's Mother. They acknowledged Justin Moffat worked as a child care staff member on one occasion at the child care. They reported that Ms. Mott had informed them of this and gave them the option of picking up their children early on that day. Child B's Mother, Child D's Mother, Child E's Mother, and Child F and G's Mother were aware of a woman working as a child care staff member at the child care last summer. Child B's Mother and Child F and G's Mother identified this woman as Cheyenne Mott's mother. Child D's Mother acknowledged Cheyenne Mott's mother being at the child care last summer, but she assumed she was just visiting the home. On 04/25/2023, I conducted a follow up interview with Cheyenne Mott. I asked her to identify which woman was working at her child care last summer. She said that she was planning to have a relative work at her home, but she ended up not getting her background check to work. I informed Ms. Mott that I need her to be honest with me. She confirmed that her mother, CCSM 1, worked at her child care last summer from July to August. She stated that she could not get CCSM 1's fingerprints scheduled on the Child Care Background Check system. I asked whether she had tried to contact the CCBC Help Desk and she stated that she did not. I informed her that she cannot have adults working in her home unless that have completed a comprehensive background check and meet the requirements of a child care staff member. I asked whether CCSM 1 had completed the health and safety trainings or any paperwork before working as a child care staff member. Ms. Mott stated that she would not even look because CCSM 1 did not complete any forms or training before working as a child care staff member at her child care. I inquired why she did not tell me about CCSM 1 when I was at her home earlier and asked whether any other adults had worked at the child care. She said that she did not even think about CCSM 1 working last summer. On 04/25/2023, I interviewed CCSM 1. She confirmed that she worked at Cheyenne Mott's child care from July to August 2022. She denied completing her fingerprints for the comprehensive background check. She thought that she had 90 days to complete the background check. She was not aware of needing to have certain trainings and paperwork. She works at the schools in
Cheyenne Mott provided Child A (14-month-old, male) with dairy products on at least two occasions even though Child A has a known dairy intolerance. Ms. Mott was informed of Child A's dairy intolerance in February 2023, but his child information was not updated and continued to identify that Child A had no allergies, special needs, or special instructions. ANALYSIS: Ms. Mott did not ensure to provide appropriate care and supervision of children at all times when she allowed Child A to have yogurt and ice cream when she was aware of him having a dairy intolerance. CONCLUSION: VIOLATION ESTABLISHED APPLICABLE RULE R 400.1945 Emergency; plan; drill. (2) The written plan must include all of the following: (i) A plan for how children with chronic medical conditions will be accommodated in all types of emergencies. MCL 722.115n Application for or renewal of license to operate child care center, group child care home, or family child care home; household member or child care staff member; criminal history check; requirements; duties of department. (1) Except as otherwise provided in subsection (13), when a person, partnership, firm, corporation, association, governmental organization, or nongovernmental organization applies for or applies to renew a license to operate a child care center, group child care home, or family child care home under section 5m and before a group child care home or family child care home allows an individual to be a member of the household, or a child care center, group child care home, or family child care home allows an individual to become a child care staff member, the department shall do all of the following: 7 (a) Review its database of individuals with previous disciplinary action within a child care center, group child care home, or family child care home or an adult foster care facility. (b) Conduct a search of the individual through the national sex offender registry. (c) Request a search of the individual through all state criminal registries or repositories for any states of residence in the past 5 years. (d) Request that the department of state police perform a criminal history check on the individual, child care staff member, or adult member of the household.
