On 4/13/2021, I completed an unannounced inspection at the facility. During my onsite inspection, I interviewed program director Candice Punches. Ms. Punches reported on 4/12/202, children were transitioning from free play to lunch. The children go to the bathroom and wash their hands before lunch begins. Ms. Punches stated that there were 15 students that day. She took 8 children with her to the bathroom and shortly after Child Care Staff Member Jamie Alber took the remaining 7 students. All 15 students and 2 Child Care Staff Members were in the bathroom. Ms. Punches stated the 8 children that were initially with her finished first and were getting ready to leave the bathroom when 3 more students from Ms. Alber's group joined. Ms. Punches recalled that Ms. Alber stated she had the remaining 4 students. A couple minutes later Ms. Alber came into the room. They were preparing for lunch and that is when Child A knocked on the door. Ms. Punches stated that neither her nor Ms. Alber realized that he was gone. Ms. Punches reported that he was in the bathroom for a few minutes unattended. She added that Child A was not scared and thought he was being funny. During my onsite inspection, I interviewed Child Care Staff Member Jamie Alber. Ms. Alber showed me the bathroom that they use, down the hallway. She stated that they currently share the bathroom with other children in the school because the bathroom they used to use is currently not working. She stated that on 4/12/2021, before lunch, Ms. Punches took a group of students in the bathroom while she waited for the remaining students to line up to go to the bathroom. Ms. Alber reported that her group and Ms. Punche's group met up in the bathroom. Ms. Punches started taking back a group of 8 students when 3 more students joined. Ms. Alber stated that she knew she had 4 children left with her in the bathroom. When 3 of the students finished going to the bathroom, they lined up against the wall. She opened the door to every stall to ensure that she had every student. She asked, "is everyone in line?" when no one responded she thought she had every student. Ms. Alber added that she figured that Child A must have jumped in Ms. Punch's line and that is why she only had 3 students in the bathroom. Ms. Alber explained that when she got back into the classroom with her group of students, Ms. Punches was with a child, and she was busy getting other children settled: neither her nor Ms. Punches realized that Child A was missing from the group until he knocked on the door. Ms. Alber reported that Child A was in the bathroom unattended for about 4 minutes. On 5/24/2021, I spoke to Child A's Father. He reported that he was aware of the incident that occurred on 4/12/2021. Child A's Father reported that Ms. Punches called him immediately after the incident occurred. Child A's Father explained that his son can be mischievous at times and probably thought he was being funny by hiding in the bathroom. Child A was not afraid that he was alone in the bathroom and purposely hid from Ms. Alber, like he was playing a game of "hide and seek." Child A's Father reported that he has no concerns regarding the care that his child receives and added his child has learned a lot this school year. 3
Appropriate care and supervision were not provided when Child A was left in the bathroom unattended.

On 3/9/2021, I made an announced inspection at the facility. During my onsite inspection, I interviewed Child Care Staff Member, Lori Brunsting. She explained she is the assistant caregiver in the classroom. When I read Ms. Brunsting the allegations, she responded "oh no, she would never do that!" Ms. Brunsting reported that Ms. Larson is always showing love towards Child A and takes special care of him. She denied seeing Ms. Larson hitting Child A or any other student. She reported that he is very lovable and sweet. Ms. Brunsting stated that she worked in Kent City School District for 32 years and recently transferred to Cedar Springs. She reported that she loves this school and "Ms. Larsen is one of the best people I have ever worked with and is super patient." During my onsite inspection, I interviewed Ms. Larsen. After reading the allegations, Ms. Larsen stated "No, I would never do that." Ms. Larsen could not recall an incident where Child A could have interpreted that she was being mean to him. Ms. Larsen stated that Child A is very young and displays some behavioral problems. Ms. Larsen reported that she will typically try to redirect Child A when he is misbehaving and squat down to talk to him about the situation to correct his behaviors. When Child A is misbehaving, he will sit in a chair which he typically choses to do himself. Ms. Larsen explained that Child A's behaviors are worse on the days that he has family visits which are Tuesdays and Fridays. During my onsite inspection, the children were arriving to school off the school busses. Children were patiently waiting in the hallway for all the children to get off their busses so they could all go into their classrooms. I observed Child A greeting Ms. Larsen with a hug. Child A did not appear to be afraid of Ms. Larsen and appeared very comfortable around her. On 4/12/2021, I spoke to the complainant. She stated that she has been working with Child A for almost a year. The complainant stated that on 2/23/2021, Ms. Larsen told her Child A had a rough day. Once Ms. Larsen left, Child A told the complainant that Ms. Larsen is "mean and bumped my butt on purpose." Child A was unable to give more details of what happened and what led up to the alleged incident. Child A kept repeating "Ms. Larsen bumped my butt." The complainant reported that Child A is "very creative and makes up imaginative stories." She stated that Child A said he felt safe at school when asked. She has never had any concerns with Ms. Larsen and she will see Child A holding Ms. Larsen's hand when they are walking. Child A does not appear to be afraid of Ms. Larsen. On 4/12/2021, I spoke to Child A's Guardian. She was made aware of the situation by the complainant, on the evening of 2/23/2021. She and her husband asked Child A about the alleged incident. Child A never disclosed that Ms. Larsen was mean or hit him on the butt. Child A has never said anything negative regarding Ms. Larsen and they have a great relationship. Child A's Guardian reported that Child A was removed from a child care center after 6 weeks due to his behaviors and then they decided to send Child A to Cedar Springs GSRP. She noted that Ms. Larsen has not called once to have them pick up Child A and has done such a great job keeping 3 him in school and controlling his behaviors. Child A's Guardian stated that Ms. Larsen does a great job communicating, she has no concerns and is "100% confident things are going well." On 5/3/02021, I received a copy of the discipline policy. The Great Start Readiness uses positive methods of discipline which encourage self control, self esteem and cooperation. Hitting, spanking, and other forms of corporal punishment are prohibited. The GSRP creates a curriculum that is age appropriate, with age- appropriate expectations, with a daily routine.
