I interviewed the complainant. Regarding Child A, prior to March 2020, Child A was crying and wanted his pacifier. Ms. Allgood wrapped him up in a blanket and shoved a blanket in his mouth. Child A laid on the floor and cried. It is not known how long the blanket remained in his mouth. Child A was not injured and he was able to breathe. Regarding Child B, during the week of October 6th, Ms. Allgood placed on a pullover bib on his head before lunch at 10:50am. Child B was reminded several times to pull the bib down, but he did not. No one pulled the bib 4 down for him and he sat like that through lunch. Child B ate after 2:00pm when he finally pulled the bib down, despite lunch starting at 11a m. The complainant provided me with a picture of Child B sitting on the floor with a bib on his head with a text message at 12:49pm that said "He just took his bib off.....and kept trying to put it on and did." The alleged response from Ms. Allgood was "K I'm in the same hurry he was Imao." Regarding Child C, on 10/13/20, Ms. Allgood took him by the arm from napping on the floor to a crib after he would not remain still and fall asleep. Ms. Allgood "grabbed" him by the arm and walked him to the play pen. Child C's feet were not on the ground as Ms. Allgood was holding him up by his arm. Child C was not injured. Regarding Child D, on 10/14/2020, Child D was crying and wanted her mother. Ms. Allgood told her to stop crying. Child D started gagging and vomited on the floor near the kitchen area. Ms. Allgood yelled at her to stop, she took Child D to the bathroom, and told Child D to stay in the bathroom. Ms. Allgood said that Child D could stay in the bathroom all day and that she did not care. Child D stayed standing in the doorway. Ms. Allgood told her that she needed to stand over the toilet. Child D was later told to sit at the table and that she needed to calm down and relax. Child D was told that she could get up from the table once she composed herself. I questioned the complainant on why he or she did not report their numerous concerns individually while they were happening, as well as why the complainant chose to wait more than eight months to report some of the allegations. The complainant replied that he or she "loved" the children and does not want to see them treated this way. During the investigation, I completed two on-site inspections. I did not observe any concerns. During those two inspections, I observed the children engaged in meal time, circle time, freedom of choice learning, and quiet time. The children received appropriate care and supervision during both visits. I observed Ms. Allgood actively and positivity engaged with the children. She also positively and effectively verbally redirected one child. I did not observe any children wrapped in blankets with blankets in their mouths. I did observe the children wearing bibs at meal time. The children were all able to put the bibs on themselves. I observed children being carried in an appropriate manner. I did not observe any of children be yelled at, as they were all spoken to in a calm and patient manner by Ms. Allgood. In addition, I observed all the children actively approach Ms. Allgood for care and attention. None of the children expressed or exhibited any concerns. I interviewed Ms. Allgood. She adamantly denied the allegations. She stated that she has been providing child care since 2010, and she "loves" her job "very much." She believes that she treats all the children "well" while providing a "positive structured learning environment." The children have never been injured, and she would "not do" the things that she is being accused of doing. Ms. Allgood described all the children in her care favorably. She used descriptive words such as "funny," "super-sweet," and a "good kid." She has no concerns 5 regarding the children's behaviors, and she enjoys having all of them at her child care home. Ms. Allgood does not provide the children with appropriate care. Prior to March 2020, Child A (age 2 years) was crying and wanted his pacifier. Ms. Allgood shoved a blanket in his mouth and wrapped him in a blanket. Child A was not injured. During the week of 10/5/2020, Child B's (age 3 years) bib was hanging over his face and not pulled down. Ms. Allgood would not pull the bib down and told Child B that if he did not pull the bib down, he would not eat. Lunch started at 11:00am. Child B did not eat until after 2:00pm. On 10/13/2020, Child C (age 1 year) was looking around during naptime and was getting up. Child C had just moved from napping in a playpen to napping on a blanket on the floor. Ms. Allgood grabbed his hand and took him to a playpen. Child C did not have his footing and was hanging by the arm. Child C was not injured. On 10/14/2020, Child D (age 4 years) vomited. Ms. Allgood grabbed Child D by the hand and forcibly took her to the bathroom. Child D stood in the doorway of the bathroom crying with vomit coming out of her nose. Ms. Allgood would not let her leave the bathroom. Ms. Allgood yelled at Child D to stop crying. Ms. Allgood uses inappropriate discipline with the children. Prior to March 2020, Ms. Allgood yelled at Child A. Ms. Allgood grabbed Child A's hair and whipped his head back for him to look at her. She then whipped Child A's head forward when she was done yelling at him. Child A was not injured. This happens often. Prior to March of 2020, the children were instructed to pick up toys. Ms. Allgood made Child B pick up a Lego toy and walked him to a basket. When Child B would not let go of the Lego, Ms. Allgood put his hand on the edge of the basket and left him sitting there for several hours. Child B was not injured. Prior to March 2020, Ms. Allgood would smack the high chairs with her hand if children were sitting in the high chairs crying. The children "have fear on their face." A couple of weeks ago, Ms. Allgood "slammed" Child E (age 1 year, 10 months) in the time out chair. He would not stop crying and he vomited. He was not injured. Children have to eat everything that is served. They are not allowed to leave food behind on their plates.

I interviewed the complainant. Regarding Child A, if Ms. Allgood were "annoyed" with Child A, she would pull his hair back and make him look at her. 10 She would yell at him and then push his hair head forward. This has occurred since prior to March of 2020 and "happens all the time." Child A has not been injured. Regarding Child B, he got into trouble when he was supposed to be picking up toys after an activity. Child B picked up a Lego toy but did not want to drop the toy into the basket when Ms. Allgood told him to let go of the toy. Ms. Allgood told him to sit there until he dropped it. Child B sat by the basket from 9:40 — 2:43pm. This occurred prior to March of 2020. Regarding the high chairs, Ms. Allgood would use an open hand to smack the high chair tray if children were in the high chairs crying. This occurred prior to March of 2020. Regarding Child E, on an unknown date a couple weeks ago, Child E did not want to sit in a chair and kept getting up and down. Ms. Allgood "slammed" him down in the time out chair and he cried until he vomited. Child E was not injured. Child E would sit in a chair from 8:00am until 10:30am. I questioned the complainant on why they did not report their numerous concerns individually while they were happening, as well as why the complainant chose to wait more than eight months to report some of their allegations. I did not observe Ms. Allgood strike any of the highchairs. I observed Ms. Allgood pick up the children and hold them in a caring and loving manner. I also observed Ms. Allgood speak to all the children in a calm and patient tone. In addition, I observed all the children actively approach Ms. Allgood for care and attention. None of the children expressed or exhibited any concerns. I interviewed Ms. Allgood. She adamantly denied the allegations. She believes that she treats all the children "well" while providing appropriate structure and discipline. Discipline consists of all the children being spoken to and redirected. Children over 3 years of age may be placed in a time out, which consists of taking a seat at the table in the main child care room, but that happens rarely. Children under 3 years old may be redirected to another activity or asked to sit closely to a staff member while they continue to play and learn. Ms. Allgood believes that she is "patient" with the children and that she has appropriate behavioral expectations. She stated that she is "happy" with her job, and that she is "nurturing and loving." The children have never been injured, and she would "not do" the things that she is being accused of doing. Ms. Allgood described all the children in her care favorably. She used descriptive words such as "funny," "super-sweet," and a "good kid." She has no concerns 11 regarding the children's behaviors, and she enjoys having all of them attending her child care home. Regarding Child A, Ms. Allgood denied the allegation. She stated that she did not yell at Child A, she did not grab his hair, and she did not force him to look at her. I interviewed the complainant, who stated that the children have to eat everything that is served. The children are not allowed to leave food behind on their plates. During the investigation, I completed two on-site inspections. I did not observe any concerns. I observed the children engaged in meal time. The children actively ate what was served. I observed that some of the children did not finish all the food on their plates. Ms. Allgood did not express or exhibit any frustration during meal time, or regarding any of the uneaten food. I did not observe the children express or exhibit any concerns regarding meal time. I interviewed Ms. Allgood. She denied the allegation. She stated that the children eat at the same time, except for the babies that eat on demand. Lunch is served starting at 11:00am. Ms. Allgood described all the children in care as "good eaters." She encourages the children to eat and try new foods, but they are not required to eat all their food. I interviewed Ms. Henderson. She has no concerns regarding meal time for the children, saying that all the children "eat well." The children are encouraged to try food, but they are not forced to finish the food on their plates. I interviewed Ms. Boatman. She does not believe the allegations. She stated that when she worked alongside Ms. Allgood, the children in care would "eat well." She "never" had any concerns, saying meal time was not a stressful time. The children were not forced to finish the food on their plates. I interviewed six parents including Child A's Mother; Child D and Child J's Mother; Child F's Mother; Child G, Child H, and Child I's Mother; Child K and Child L's Mother, and Child M and Child N's Father. None of the parents expressed any concerns regarding meal service by Ms. Allgood. The parents described their children as being good eaters. None of the children have expressed or exhibited any concerns.
