I conducted an unannounced visit at the facility on 10/08/2020. At the time of my arrival, the program director, Leah Glaze was present along with 2 child care staff child care staff members. There were 8 children present, age 14 months to 3 years of age. I conducted a walk thru of the 3 classrooms and observed an adequate amount of equipment and toys to accommodate the ages and number of enrolled children. Leah Glaze reported that they clean and sanitize the equipment daily and ongoing as needed. Most of the toys were not visible due to limitations during Covid 19. I attempted to obtain specific information from the complainant, but no response was provided. She later attempted to have the complaint withdrawn. The complainant alleges that all the concerns in the complaint was observed during a tour of the facility around 5:20 pm. while children were present. I interviewed the Licensee, and she reported that they have not allowed tours in the facility due to Covid 19, they have only allowed potentials to enter the facility and stay in the hall way but they are not allowed to enter into any of the classrooms while children are present.

At the time of my arrival, there were 3 kids with one 1 child care staff member in the preschool classroom. There were 3 kids with another child care staff member in the toddler classroom. There were 2 kids present with 1 child care staff member in the infant room. There was a total of 8 children present with 3 child care staff members ages 14 months to 3 years of age. I reviewed the sign in sheet for the previous 2 weeks. According to the attendance log, the maximum amount of kids present on any given day was 8 children at any given time, with 3 child care staff members. There are only 2 infants enrolled, 6 preschoolers enrolled, and 3 toddlers enrolled. There are 5 child care staff members currently working at the facility. I interviewed child care staff members Nina Glailan and Bree Hanselman during the onsite visit. Ms. Glailan reported that she is never out of ratio and that she currently only had 3 children enrolled in the toddler room at the time. She denies being out of ratio at any given time. Ms. Hanselman reported that she has 4 children that comes on a regular basis, but only had 3 children present at the time of visit. She also denied being out of ratio at any given time. Ms. Glaze was present with 2 infants. She denied being out of ratio at any given time. She reported that they have 2 additional child care staff members that are available to work. If there is a need to call another worker in to comply, they would do so. I spoke to Ms. White, the Licensee, and she also denied being out of ratio. She stressed that she has a total of 5 child care staff members, but the enrollment does not warrant them all being present at this time. She reports that she can call additional staff to come to work to prevent being out of ratio. I attempted to obtain additional and specific information from the complainant about the room that was allegedly out of ratio with mixed age groups, but she declined to provide any additional information.

I informed Ms. Glaze about the allegation of an infant being left in the dark in a crib. Ms. Glaze reported that the lights are turned off in the infant room during nap time. There are several windows in the room that allow light in the room. She is still able to see in the room during nap time and continue to provide supervision. I observed the lay out of the infant room. There was a crib in a corner, but I was still able to see into the crib from the doorway of the room as well as from the chair that was stationed in the room for the child care staff member. .........................................

Ms. Glaze denied the allegation of food items being left out that needed to be refrigerated. She reported that they only use the microwave to warm up any food, and after lunch whatever is not eaten is discarded. She admits that they may not clean up or throw the items away immediately after lunch due to getting the children prepared for nap time. She reported that any small juice items that are used are thrown away during clean up time. Ms. White reported that they do not refrigerator any leftover items after lunch. Whatever is left must be thrown away. She also stated that all juices that are left out will be thrown away during clean up time. I arrived at the center shortly after lunch time. I observed the tray in the hallway with plates and uneaten food and drink items. The items were stacked in a way that appeared it was for trash. All the child care staff members were allowing children to use bathrooms and prepare for nap. Ms. Glaze reported they would clean up the items later when they were finished with the children.

My visit to the center was unannounced. Upon my arrival, there was a cart outside of a classroom with left over food from lunch. It appeared that it was to be discarded according to how it was stacked in layers of used plates and uneaten food. I arrived shortly after lunch had been served. The rest of the facility appeared to be clean and orderly. The facility appeared to be very well maintained. No safety issues or concerns noted. I attempted to obtain specific information from the complainant, however, no response was received to further explain the allegation at the time of her "tour".

At the time of visit, the center was not dirty. It appeared well maintained. The complainant declined providing any additional information to support the allegation.
