On 02/09/2022, I completed an unannounced inspection. I spoke with Ms. Cruse, and she stated that allegations are in reference to Child A (10- months-old, female). Child A is no longer enrolled in her child care home. She explained that Child A was exposed to COVID-19 and she told Child A's Mother that Child A had to quarantine. Child A did not return after the quarantine period. Ms. Cruse stated that she has never left Child A alone by herself without being monitored and supervised. Ms. Cruse stated that Child A is placed in her crib to take a nap, she is not left in the crib all day with the doors closed unless she is taking a nap. She stated that she does close the door when the infants are sleep and she checks in on them. Ms. Cruse explained that the children also rest in the living room, there are three cribs set up in the living room. She had a total of three infants enrolled in the child care home. Ms. Cruse stated that Child A was fussy her last day in attendance. Ms. Cruse spoke with Child A's Mother regarding Child A being fussy and Child A's Mother told her that Child A never liked cribs or porta cribs. Child A's Mother told her that Child A never slept in a crib. She stated that there was one afternoon during naptime when Child A was very fussy, and she let her rest upstairs. Ms. Cruse stated that she monitored Child A the entire time that she was upstairs. During the unannounced onsite inspection, I did not observe any children in cribs while they were awake. I reviewed the attendance for the day, all the children who were signed in on the attendance sheets were present in the child use space. There were no children in the sleep rooms with the doors closed. On 02/24/2022, I spoke with Ms. Christina Nesbitt, Child Care Staff Member. She said that her last day working at the child care home was 01/30/2022. Mrs. Nesbitt explained that she resigned because she was relocating. She stated that Ms. Cruse would tell her to put Child A in the crib room from approximately 09:00 a.m.- 12:00 p.m. She stated that Child A would be fussy and fall asleep. Ms. Nesbitt stated that when Child A would be fussy after breakfast, Ms. Cruse would put her in the crib room alone if she was fussy for more than 5-10 minutes. She stated that she would check in on Child A, a couple of times when she was in the sleep room. Ms. Nesbitt stated that Child A did not like being in the crib. Child A's Mother did not use a crib, porta crib, or playpen at home. She stated that Ms. Cruse would set a timer when Child A was fussy. When the timer went off then Child A was removed from the area. On 02/24/2022, I spoke with Child A's Mother. She stated that she found out the information from the Witness. The Witness said Child A is placed in a room alone. She stated that she would call Child A's Mother for approval to give Tylenol, because Child A was teething and very fussy. Ms. Cruse stated that when she received a verbal approval to give Child A the Tylenol her mother would sign the medication slip when she arrived to pick Child A up. Child A's Mother said Ms. Cruse would call every day stating that Child A was fussy and to get permission for Child A to receive Tylenol. She stated that Child A has been walking since she was 8- months-old and she believes that Child A was fussy because she was being left in the play pen for hours. On 03/30/2022, I requested the medication log for Child A. On 03/31/2022, I received the medication log and child care contract via email from Ms. Cruse.
