On February 23, 2022, I initiated the investigation with an unannounced site visit. Paige Pesci, program director and Sharon Loney, assistant director were present. Mrs. Pesci said the center's policy is to complete a written incident report as well as to notify the program director when a child is injured. When Child A was injured, the child care staff member in his classroom, Alexia Lee, did not follow this protocol and has been terminated because of it. Mrs. Pesci did not learn of the injury until Child A's Father notified her. She then reviewed the center's video footage. Per Mrs. Pesci, the footage from December 14, 2021, showed Child A trip "over his own feet" and hit his face on the table. The incident report was written by Mrs. Pesci after reviewing the video. Mrs. Pesci provided a copy of the incident report and their written accident/incident policy. On February 23, 2022, Sharon Loney stated that Child A had swelling and a bruise on his check from the accident. She also said that Ms. Lee, who was in the classroom when the accident occurred, was no longer employed because she did not follow the center's protocols. The protocols are written and provided to al CCSMs. On April 21, 2022, Child A's Mother said she does not have any concerns with the care that Child A is receiving. She said that when she arrived for pick up on the date in question, she was notified verbally about Child A tripping. Child A's Mother stated that when she followed up with Mrs. Pesci, she learned that Ms. Lee had not reported that accident to the director. Child A is currently doing well and has healed. Attempts were made to contact the complainant, Child A's Father, Alexis Lee, and an additional family for comment. At the time of this report, none of these people have responded.
