    On 11/10/2022, I spoke wit 1 informed her of the correct address for Pecomplaint. We confirmed a date for our joint investigation for 11/14/2022, to interview staff who were identified to be involved with the incident. informed me that she was unable to complete a forensic interview with i A because he was unable to explain the difference between a truth and a lie. said Child A had a soft cast on his middle finger wrapped with a brace. Child A said his friend at school closed the door on his hand. He explained that he was not moving fast enough, and his friend had to use the bathroom and closed his hand on the door. He told that his hand hurt "really bad" when it happened, but it does not hurt now. He stated that the teachers were "really close" because one of his teachers was helping his other friends wash their hands for lunch. He explained that the sink is right next to the bathroom. He said his other teacher was getting lunch or snack ready. Child A did not know his friend's name who closed his hand in the door, and he did not know any of his teachers' name. He was unable to name any of his other friends at the child care center. On 11/14/2022, 1 completed an unannounced onsite inspection with said she interviewed Child A's Mother. She stated that Child A was taken o urgent care on the day of the incident. He was taken to Child A has a fracture of the phalanx on his finger-right hand, laceration e finger on the right hand, and laceration of the nail bed of the finger-right hand. He was provided with a finger splint. It is reported by Child A's Mother that Child A had two other incidents prior to this one. She reported that there was an incident on 0912812022. Child A was bit by another child. Child A's Mother also reported that there was another incident approximately three weeks later, where Child A was hit in the head. She could not confirm what date this incident happened. and I interviewed Ms. Vicki Dobbs, Assistant Program Director, and Child Care Staff Member at the child care center. They both stated that they were preparing lunch when the incident occurred. asked the children to wash their hands for lunch. They both stated that Paraprofessional was also with the children when the incident happened. She was with Child C (4-year-old, male). Child A was in the bathroom washing his hands for lunch, when he was walking out of the bathroom Child B (4-year-old, male) was walking in the bathroom. According 3 to Ms. Dobbs and =, they did not witness the incident, however, the bathroom is right around the corner from the area where the children eat. Child A came over to and his fingernail was coming off his middle finger. and his finger was bleeding. They immediately rinsed and wrapped his fingers and applied ice. Child A was crying at first, but when they wrapped his hand, he appeared to calm down. He stopped crying and was able to eat. Child A's Mother was called to inform her of the incident. According to Ms. Dobbs, when they began to tell her about the incident, she began to use vulgar language and told them, "' Every day she brings her child, and he gets hurt". She hung up and disconnected the call. According to , Child A started attending the child care center, 09/06/2022. She recalls one other incident where Child A was bitten by Child C, this incident was reported to Child A's Mother. stated that they make sure that all incidents are reported to parents. They both denied that there was a third incident when Child A was hit in the head. Ms. Dobbs stated that she would get both incident reports and email them to - and myself. Ms. Dobbs said does not have anything in her discipline file. She has never had any with inappropriate discipline with any of the children. U . proems is up to date with all her training and has never had a safety plan for any of the ildren enrolled in the child care center.
