(1) At the time of a child’s initial attendance, a center shall obtain a child information card, using a form provided by the department or a comparable substitute, that is completed, signed and dated by the child’s parent and updated as changes occur. The center shall keep current hard copies of the child information cards at the center.

Ten child child information cards were reviewed. None had an admission date. Five were missing employer phone number. Two were missing medical treatment waiver. Three had missing allergy/special needs/special instructions sections.

R 400.8213(2) · R 400.8213(2) Staff; volunteer (2) All supervised volunteers shall receive a public sex offender registry clearance by the licensee against the Michigan State Police Sex Offender Registry or the Dru Sjodin National Sex Offender Public Website before having any contact with a child in care. A copy of this clearance must be maintained on file at the center or at the central office.

Six supervised volunteers were present at the time of the inspection and none had a Public Sex Offender Registry Clearance at the time of the inspection.
