    On 3/4/2026, Teacher 1 failed to implement the center’s discipline policy when, after Child A threw a block at her, she responded by threatening loss of participation in “Boom Sticks” rather than using positive guidance strategies such as redirection or distraction, and then turned her back without intervening, at which point Child A threw a chair that struck Child B, resulting in an injury requiring medical treatment.

    Ten child information cards were reviewed. Half were from 2015 and outdated. Four out of ten were missing the “Release to” section, five out of ten were missing employer phone number, three out of ten were missing physician phone number, and one was missing the center’s name for medical treatment.

    R 400.8125 · 1 R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 11/02/2022, 1 completed an on-site inspection. I interviewed program director Danielle Smith, program director Brandy Wasick, Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), Child Care Staff Member 3 (CCSM 3), Child Care Staff Member 4 (CCSM 4), Witness 2, Witness 3, and Witness 4 CCSM 1 identified herself as the lead caregiver in Child A's and Child B's classroom. She stated that Child A began attending her classroom in April 2022 and Child B began attending her classroom in June of 2022. Child A and Child B both attended the center's early preschool classroom prior to her classroom. She often combined her classroom with the early preschool classroom during breakfast and recess when Child A and Child B attended the early preschool classroom, so she was familiar with both boys before they started attending her classroom. She is not sure if Child A or Child B ran from the early preschool staff, but they frequently run away from her 2 and other staff members in the classroom. She stated that Child A and Child B ran out of the classroom during clean up time on 10/27/2022, which was the first time they ever ran out of the classroom. She was in the classroom with Child A, Child B, and three other children, while CCSM 2 was in the bathroom with eight other children. She described clean up time as a difficult transition for Child A and Child B. Child A and Child B were running around the classroom during clean up time. Just as she got them to help hang up the dress up clothes, Child B bolted out of the classroom. Child A hesitated but then he also ran out of the classroom. She immediately took the other three children to CCSM 2 in the bathroom, which is located in the hallway just outside of the classroom, and then ran in the same direction as Child A and Child B. She could initially see Child A down the hallway, but she did not see Child B. She thought maybe they were headed towards the gym (located in the middle school within the same building) that they often use after clean up time, so she headed there first. She ran into Witness 2 and Witness 3 on her way to the gym. She told them she was looking for Child A and Child B. They joined her in the search for Child A and Child B. They followed the directions of others who saw the boys to know which way to go. Child A and Child B were not in the gym, so she made her way back to the classroom. CCSM 5 was walking back with the boys just as she returned to the classroom. CCSM 1 stated that she had a meeting with administrators and CCSM 2 to discuss strategies to prevent any child from running from the classroom. Some of the strategies discussed included making clean up time more of a game to engage children who struggle with cleaning up and for staff to be more mindful of their proximity to the door. Witness 4 stated that she is the program director for the school-age child care center at the same location, but she sometimes provides staffing support to the preschool child care center. On 10/27/2022, CCSM 1 came in the office she (Witness 4) shares with the preschool program directors and told her that Child A and Child B took off running and asked for help to locate them. Witness 4 stated that she went left and checked other classrooms before going with CCSM 1 to the middle school. They asked other adults along the way if they saw which way Child A and Child B went. They encountered two middles school students who found Child A and Child B who had already handed them off to CCSM 5. Witness 4 talked to Child A and Child B to determine the reason for them running. Child B told her that they were trying to see how fast they could run with their Halloween masks they made earlier that morning. Child B was wearing the Flash superhero mask he decorated while she talked to him. Witness 4 has no knowledge of previous running incidents with Child A or Child B. Witness 2 stated that she was in the office on 10/27/2022 when CCSM 1 stopped in and asked Witness 4 for assist
