On 01/26/2023, I completed an on-site inspection. I interviewed program director Briamarie Vasilion and Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), Child Care Staff Member 3 (CCSM 3), Child Care Staff Member 4 (CCSM 4), and Child Care Staff Member 5 (CCSM 5). Ms. Vasilion stated that CCSM 1 has fallen asleep twice while she was supposed to be supervising the children during rest time. She observed both occurrences by either the camera or by walking past the classroom. She does not recall if any of the children were awake when this occurred. The first time occurred on 11/28/2022 and the second time was approximately two weeks ago. She had a verbal discussion with CCSM 1 both times. She had no knowledge of CCSM 1 leaving children unattended in the classroom or bringing fecal matter into the facility on her shoes. CCSM 1 stated that she did not sleep the night before when she fell asleep while caring for children. She stated that she nodded off for approximately one minute during rest time sometime in November 2022. Ms. Vasilion talked to her about it when it occurred. She recalled falling asleep again during rest time sometime in December 2022 for approximately two minutes. She could not remember who spoke to her about sleeping the second time. She noted that she has a hard time sleeping 2 at night, which causes her to be sleepy during the day, especially during rest time. She could not recall ever leaving children unattended in the classroom for any length of time. She denied bring fecal matter into the facility on her shoes. She noted that she lives on an animal farm, but she has never tracked fecal matter into the facility. I did not observe any fecal matter on her shoes. CCSM 2 stated that she witnessed CCSM 1 fall asleep during rest time sometime in November 2022 for approximately two minutes. She woke her up when she noticed her sleeping. Since then, she makes it a point to engage with CCSM 1 to keep her awake during rest time. She has no knowledge of CCSM 1 leaving children unattended in the classroom. She has never witnessed CCSM 1 tracking fecal matter into the facility. She stated that CCSM 1 is always clean and does not smell. CCSM 3 stated that he has witnessed CCSM 1 sleep for approximately 20 to 30 minutes during rest time. He also recently witnessed her sleeping for approximately one hour in the green classroom. He stated that CCSM 1 has fallen asleep on more than one occasion in more than one classroom; however, she does not sleep every day. He stated that the program director has woken her up before. He has no knowledge of CCSM 1 leaving children unattended in the classroom. He has never observed CCSM 1 track fecal matter into the facility, but sometimes he can smell animal fecal matter. CCSM 4 stated that he has witnessed CCSM 1 sleeping during rest time at least three times, anywhere between 15 to 20 minutes at a time. Sometimes CCSM 1 will pull her hoodie over her head to conceal her face during rest time. He has also witnessed CCSM 1 sleeping when the children were not resting. He has witnessed CCSM 1 leave the classroom on occasion for a bathroom break, but there is always another staff member present. He stated that he has observed fecal matter on CCSM1's shoes. He noted that sometimes he can smell the fecal matter. CCSM 5 stated that she saw CCSM 1 sleeping when a parent arrived to pick their child up during rest time, so she called out to her to wake her up. Additionally, last Friday she heard CCSM 1 snoring loudly during rest time. She noted that CCSM 1 woke up a short time later. She has never observed CCSM 1 leaving children unattended in the classroom. She has never noticed anyone tracking fecal matter into the facility.

On 11/16/2022, I completed an unannounced on-site inspection. I spoke with Amy Fleurette who was in charge in the center director's absence. At the time the report was written, Ms. Fleurette is the approved program director. Ms. Fleurette reported that there are only two school age children currently enrolled so the school age room (where the bathrooms are located) is not currently in use. The center staff do the cleaning at the center. Center staff use both the school age bathrooms and an additional adult bathroom located in the hallway. Parents and children can also use the bathroom as there are doors in the hallway and in the school age classroom. No one has complained to the center administrators that the bathrooms were not clean. I suggested that the center hire a cleaning company to clean the center instead of having their CCSMs responsible for that task. I walked through the center to observe the maintenance of the premises. I observed the center to maintained in a clean and safe condition. I went in both school age bathrooms and found them to be clean and sanitary. On 05/02/2023, 1 spoke with licensee designee Vonda Daily who confirmed that the center staff are responsible for cleaning the bathrooms. No one has complained about the bathrooms not being clean that she is aware of. I spoke with CCSM 1 who reported that it is mostly the staff that clean the bathroom. There is not a cleaning schedule. CCSM 1 said that the toilet had been dirty in the past with a ring around the bowl, but it is currently cleaned regularly.

On August 5, 2022, program director, Bria Marie Vasilion submitted an incident report to child care licensing consultant, Rachel Arens stating that Child G pulled down Child H's pants while out on the playground. On August 12, 2022, I completed an on-site inspection at the child care center. I spoke with program director, Bria Marie Vasilion and child care staff members (CCSMs) Joseph Vanderzee, Caleb Mills, Daniel Kapenga. Bria Marie Vasilion stated she spoke with Child G on August 5, 2022, after the incident. She asked Child G what happened. Child G told her that they were playing house and someone told her to change Child H's diapers. She then pulled down Child H's pants. Child G said there was no touching or talking of private parts. Ms. Vasilion stated she has not reached out to Child H's Mother. I provided technical assistance to Ms. Vasilion about the new group size requirements. I asked Ms. Vasilion to send me child information cards for the children involved in the incident, attendance for children and staff for the day of the incident, and contact information for other staff that would have been present on the playground. Joseph Vanderzee stated he is an assistant child care staff member at the center. Mr. Vanderzee stated he was present on the playground during the incident on August 4, 2022. He did not witness what happened; another child told him that Child G had pulled Child H's pants down and that they saw her "private parts." Child H did not have underwear on that day. Mr. Vanderzee explained he went to Child H and asked her what happened. Child H told him, "Child G pulled down my pants." He asked her, "Was that ok?" Child H answered, "No." He said that Child H was not crying and did not seem visibly upset. He and Child H went and spoke with Child G. He said when he asked Child G about what had happened, she would not answer and kept looking down towards the ground. After asking her a few times, she did admit to pulling down Child H's pants. I inspected the playground with Mr. Vanderzee, he pointed out where he and the other staff were standing on the playground. Mr. Vanderzee stated that he was standing by the sandbox, another staff member was by the tunnel by one door into the building, and other staff member was by another door into the building. He said that staff will position themselves in different areas on the playground and move around to supervise the whole playground. The playground is an L shape with a potential blind spot at the side of the building. Mr. Vanderzee stated there were six or seven children in the area by the side of the building. During the time he was outside he would occasionally walk over to see what the children were doing in the blind spot. When he walked over to talk to Child G, she was sitting by the far corner by the fence. I provided technical assistance on positioning himself where he could view the blind spot and the center of the playground at the same time. I took photographs of the playground; these are on file with this report. 3 Caleb Mills stated that he is an assistant in the Yellow Room. He was not outside at the time of the incident but when he went outside, he was told about what had happened. He stated that staff tend to position themselves in an area on the playground and will move around in their areas to ensure supervision. Daniel Kapenga stated he is the lead caregiver for the school age group. He was on break during the incident but was told about it when he went outside. He spoke with Child G and asked her what had happened. Child G told him that Child I (female, age 9) told her to pull down Child H's pants. He asked Child I and she denied telling Child G to pull down Child H's pants. Mr. Kapenga said he spoke with Child G's Mother and told her what he knew about the incident. He let her know that Mr. Vanderzee had more specific information and she could speak with him. She let him know she would speak with Child G about the incident. He does not know if Ch

On July 12, 2022, , detective Joe Saladino from the Kent County Sheriff epa men , and made an unannounced on-site inspection at the child care center. We spoke with licensee desianee Vonda Dally and child care staff members (CCSM) stated that she has not observed Child D scream or cry during diaper "Canag"es he is aware that at drop off time Child D would cry and scream, "no, no, no" but would calm down eventually. 4 stated that Child D is very emotional and will cry at drop off time and then FroWugout the day. She has not observed Child D get upset during diaper changes. She has spoken with Child D's Mother about her crying. She said that Child D's Mother told her she thought it was a "phase." stated that Child D cries "mostly" at drop off. On July 13, 2022, Detective Saladino, and I spoke with CCSM on the telephone, stated she noticed that Child D "scream and cry all day." During diaper changes she would scream, "No, no, no!" and sit up and watch while being changed. stated she told Ms. ,, Vasilion about this and Ms. Vasilion told her, "to keep an eye on her. stated that she did not notice any bruises rashes, or injuries to Child 's area but did notice it was "a little red." stated she did not speak to the Child D's parents about the concern, stated that Child D only seemed comfortable and would not cry if she, ,or changed her diaper. Detective Saladino, and I spoke with program director BriaMarie Vasilion FaCcetoface. She does not have any concerns regarding Child D's crying and screaming. She denied that staff brought any concerns to her about this. On August 2, 2022, l made an unannounced on-site inspection at the child care center. and I spoke with program director, BriaMarie Vasilion. Ms. Vasilion denied being told by "to keep an eye on" Child D. She remembers being told that she was told that Child D would say "no, no, no" if some staff tried to pick her up but denies any other concerns were brought to her attention. On August 15, 2022, 1 spoke with Child D's Mother on the telephone. She stated that she is not aware of Child D having an issue during diaper changes but that she will cry at drop off time. This has recently improved because she has been able to be dropped off in the same room as her sibling.

We spoke with licensee designee Vonda Daily concerning this allegation. Vonda Daily stated she is the current licensee designee for the child care center. Ms. Daily stated that she has not heard of staff having any concerns with any specific child care staff members. There were concerns about staff being on their cell phones during naptime and spending time in the office "hanging out.`' She has not heard of any concerns from either staff parents about the program director BriaMarie Vasilion or staff member . Ms. Daily accessed staff files and stated there were no write ups or notes oin* e er Ms. Vasilion or files. July 13 2022 , Detective Saladino, and I spoke with CCSMs and on the telephone. i RM stated she was the previous lead caregiver in the Orange Room at the care center. She said she told not to give children her personal food due to concerns with potential allergies and children should not be "eating candy while in care." said she would have to tell to pick up and soothe crying children ecause she would let children cry. would take 20 minutes to complete one diaper change so she would change the diapers herself to ensure they were changed faster. There wEere occasions that would say she changed a child, but she had not. would take a on ime during her personal bathroom breaks. Ms. Finkbeiner told her she would let the program director, BriaMarie Vasilion know about this, said at the end of June 2022, she spoke with Ms. Vasilion in person about her concerns with ' She was worried that a child would get hurt because was Mno"porovi g appropriate supervision. She stated she felt as if she was the only one supervising the children. She informed Ms. Vasilion that should not be in the Orange room. She waited a week to see if anything addressed. She did not see any changes in behavior, said she .,1 "gave her two weeks' notice, but left part way through the second week. ■ stated she sent text messages to Ms. Vasilion about her concerns. and I requested she send us screen shots of the texts she sent Ms. Vasilion. She sent the texts to us during the phone call. Copies of the texts are included on file with this report. The texts contain conversation threads between _" and Ms. Vasilion. The texts messages in part, the following concerns: Text from to Ms. Vasilion on June 23, 2022," ]...can't change diapers ...she's on her phone constantly.. .if aPer"oo something she says "Okay" and then doesn't do it.. .watching all of these kids by myself... one of them is going to end up getting hurt really bad... shouldn't be in orange room at all." Ms. Vasilion's response was that there was no one to replace and that she was assessing to see if would be a better fit in as room for older children. She has not noticed any differences during diaper changes. Child B's Mother stated that is Child B's primary care iver and that she is "nice" and that Child Bèerns when he sees . She stated she does not have and concerns about the care and supervision Child B receives at the child care center. On July 11, 2022, spoke with Child A's Mother on the telephone. On August 5, 2022, and I spoke with her on the telephone. She said that Child A has been and saying during This has been happening for approximately the last month. She MaiPatat home she will tickle his belly and arm pit to distract him during diaper changes. She said that she will say, "tickle, tickle belly" and he will touch his belly. She denied that any staff have said anything to her about him saying, "tickle, tickle" at the center or her telling staff about this. On 12/14/2021 a complaint came to the Bureau which stated that the center is not being cleaned and poses a hazard to children in care. Different types of illnesses are spreading quickly, including hand foot mouth and COVID-19 because staff are not cleaning their rooms or equipment as often or well as they should. Staff are not sanitizing things between use by different children or separating children who are ill from the rest of the group in the infant/toddler rooms. On 12/14/2021 I completed an unannounced on-site inspection at the center. I interviewed Program Director Bria Vasilion regarding center policies for cleaning and disinfecting, particularly in the infant and toddler rooms. "After every diaper change, between children, we use a 3-step process. That includes soap, water, and sanitizing spray. The staff wash their hands, and they wash the children's hands." In the infant and toddler rooms they have a "mouth toy bin" where they place toys which children have put in their mouths, which staff clean at some point during the day. If a classroom closes down due to COVID-19 exposure, the staff put the toys 2 through a sanitizing machine in the kitchen. Anything that can't go through the machine the staff wipe down with sanitizer and a rag. The sleeping cots are assigned to specific children, and they are cleaned once daily at a minimum, more often if they are soiled. I asked Ms. Vasilion if the center employs a cleaning crew for after care hours and she said they do not, the staff clean themselves. When asked how staff are trained on cleaning policies, Ms. Vasilion said that it's part of their hiring orientation. "I have them open and close with another staff member who trains them on how to do properly clean and what to do." I asked Ms. Vasilion about classroom closures. She stated that they recently had some closures due to COVID exposure, and to Hand Foot and Mouth going around. The recent closures are as follows: • November 29 & 30: 2 classrooms closed due to COVID exposure/ Infant and Wobbler. • November 23: 2 classrooms closed due to COVID exposure/ 3's and 4-5year olds. • November 22: 2 classrooms closed due to COVID exposure/ Toddlers. • November 8: 1 classroom closed due to COVID exposure/ 2 -3 year olds. 1/2 • First week in November: 2 classrooms closed due to low enrollment with Hand Foot and Mouth/ Toddlers. Ms. Vasilion stated that the rooms close due to COVID anytime there is an exposure by a child or staff member. "As soon as there's one positive case, we close the room." When Hand Food and Mouth was going around the toddler rooms, the center didn't purposefully close the rooms, they just had so many children out at the same time, they closed them due to low enrollment. "We had approximately 15 children out throughout that week." The first cases of Hand Food and Mouth showed up over a weekend, and on Monday many families were calling in sick. Throughout the next few days many more children came down with it. Ms. Vasilion said that center policy is that if a child has 2 or more symptoms of illness or a very high fever, they call the parents. The child is sent home and has to be fever free for at least 24 hours without medication. If they are out due to COVID concerns, they might need a negative test before returning to the center. And with Hand Food and Mouth, a physician's note was required prior to the child returning. "If a child comes down with symptoms when they are already here, the teachers will call Admin to come down and check the child. If we need to call parents, often we will bring the child up to the office while we wait for parents to arrive. Then the teachers will clean in the room as much as they can. If I have an extra staff member, I'll send them in to clean." I asked Ms. Vasilion if the center is doing any cohosting of groups right now, to minimize spread of illness. She said they're "trying to as much as possible." They group two classrooms together as much as possible and don't move childr

On 12/01/2021 the Bureau received an allegation that Child A was injured several times at the center, and proper notification was not provided to Child A's parents. I spoke to the complainant on 12/01/2021. They stated that in late October Child A was picked up from the center one day, and she had a gash on her finger; however, no incident report was provided. Approximately 3 weeks later, Child A fell and cut her lip. The center did not properly notify the family of the details of the incident. I spoke to Child A's Father on 12/01/2021. He said no documentation was provided to them regarding the incident in late October and that child care staff did not seem to know where the gash on Child A's finger had come from. No follow up was provided to the family after they brought it to the attention of the child care staff. On 11/10/2021 Child A fell twice at the center. The center called Child A's parents to notify them that she had fallen but said "everything was fine." Four hours later, when Child A was picked up for the day, Child A's parents found that her two front teeth were chipped "in half." The child care staff had not noticed her chipped teeth, even though they had cleaned up her mouth after her fall. "No one knew anything about it." "We met with the Director and found out that for the second fall, she helped clean up Child A. I don't know how she could have done that and not noticed her teeth. We had to request they give documentation." It was only until we asked to have two separate incident reports filed to ensure it was documented that our child fell and broke her teeth." Child A's Father sent me a picture of Child A's mouth, which shows the two front teeth chipped. On 12/07/2021 I completed an unannounced on-site inspection at the center. I interviewed Program Director Bria Vasilion. She stated that she remembered the initial incident in October, where Child A was noted to have a cut on her finger. A child care staff member had been changing Child A's diaper and noticed a cut on her finger. He did not fill out an incident report, and Child A's parent told Ms. Vasilion about it the following day. She checked with the child care staff member about it. "It was a little cut, he didn't know he had to report it. He said it looked old, it wasn't bleeding, and he didn't know if it happened at the center. So he didn't fill one out. I told him next time he needed to do an ouchie report." She did not have him write one at that time, she just instructed him to do it in the future. Ms. Vasilion explained during the second incident on 11/10/2021, Child A tripped and fell twice in the same day. "She bit her lip, but no one noticed if she hit her face when she fell." At the time of the second fall, Ms. Vasilion was outside with Child A's class. "I saw it. I'm the one who had cleaned her up because she was bleeding. I didn't notice the chipped teeth. Maybe there was too much blood to notice the teeth." When asked when the incident report was written for the incident, Ms. Vasilion said, 3 "I wrote the report, 1-2 days later, when the parents came in to talk to me. They were concerned that no one had noticed that Child A's front teeth were chipped. I apologized profusely and said next time we would check more thoroughly." Ms. Vasilion said Child A's parents did request an incident report, "I was planning on doing it, I just hadn't yet. Then they asked me too." "They left and I wrote it and had Child A's father sign it. It took him a few days to sign it, I think he kept forgetting." Molly Vanderzee explained she is a child care staff member in Child A's classroom and was present for both incidents on 11/10/2021. "I didn't see her fall the first time, but I heard it. The kids were cleaning up and she was wearing some big, clunky shoes. She was running by some shelves and she tripped on one of them. I heard her fall, looked up and she was on her stomach with her arms out in front of her. It didn't look like she had hit her face. I picked h

On 0310912021 l received a voicemail from Program Director Lisa Folkersma stating that there had been an incident in the wobbler room at the center. I returned her call and Ms. Folkersma explained that around 2pm on 0310912021, child care staff member Danielle Thompson had been standing by the counter preparing a bottle for a child. She unplugged the bottle warmer in order to dump out the hot water, allowing the cord to hang down. Child A was standing right beside her and pulled the cord of the bottle warmer which caused hot water to spill onto from above. Ms. Folkersma said they contacted Child A's Mother right away an stripped Child A down. They saw burns on right shoulder and there were spots of redness on. head as well. On shoo er the skin had begun peeling. Child A's Mother came directly to the cen er and took Child A to the , along with Assistant Director Austin DeFrain, who rode along. On 03115/2021 l completed an on-site inspection with at the center. We interviewed Ms. Folkersma, who stated that at the time of the incident on 03/09/2021 she was working at the front of the building and was not in the classroom. Child care staff member Danielle Thompson brought Child A up to the front, and Child A was screaming. Ms. Thom son said Child A had ulled the bottle warmer over on _. "Austin and I took off shirt and we called mom. At first they didn't look too bad, but over the time o waitin for [Child A's other), the burns got worse. The skin had started peelin already on shoulder." Ms. DeFrain held the child and sang to trying to keep r calm un i hill A's Mother arrived. Child A's Mother and Ms. eFrain went toe er to the with Child A. Child A's Father arrived later that day to , and he spoke with Ms. DeFrain at that time, saying it was'just an accident.' I asked Ms. Folkersma why they called Child As Mother at the time of the incident and not and she replied "because that's our policy, and she said she was on her way. s. Folkersma reported that it took approximately 20 minutes for Child A's Mother to arrive, and Child A's skin "got way worse by then? Child A was "screaming the whole time." asked Ms. Folkersma about child care staff member Danielle Thompson. She said s. Thompson is a floater at the center and had been in the wobbler room at the time of the incident to fill in for another staff member who was on break. Ms. Thompson has been a good staff member. Ms. Folkersma explained she has not had any concerns herself, nor from other staff members or parents regarding lbs. Thompson's interactions with children or for supervision. Assistant Director Austin DeFrain stated that at the time of the incident she was working at the front of the building. "Danielle brought the child up and said, 'I don't know what to do.` She was panicked," They took off Child A's clothes and assessed They also tried to dry off some body, around the burns, because was s [ wet. On 11/5/2020, I completed an unannounced onsite inspection to the facility. During my inspection, I interviewed program director, Lisa Folkersma. She reported that classrooms always operate within the required child care staff member to child ratios and she schedules three extra staff members a day to help with extra cleaning during the pandemic and to ensure that ratios are maintained if a staff member misses work. Ms. Folkersma provided attendance records for child care staff members and children in care. Based off the attendance records staff to child ratios have been maintained. I also observed every classroom with Ms. Folkersma, every classroom was within staff to child ratio guidelines. I observed: • The Infant Room had 7 infants and 2 Child Care Staff Members (CCSM). • The Toddler Room had 12 toddlers and 3 CCSM. • The Wobbler Room had 4 children and 1 CCSM. • The 2 to 2 year old classroom had 8 children and 2 CCSM. '/2 2 • The 2 to 3 year old classroom had 7 children and 1 CCSM '/2 • The 3 year old classroom had 15 children and 2 CCSM. • The 4 year old classroom had 9 children and 1 CCSM. • The 5 year old classroom had 11 children and 1 CCSM. During my onsite inspection, I interviewed CCSM, Annette Finkbeiner. She reported that she has worked at the center for 15 years and she has never noticed a classroom that did not maintain the staff to child ratio. Ms. Finkbeiner stated that there are always extra staff scheduled to help with cleaning and breaks. During my onsite inspection, I interviewed CCSM, Zac Steffes. He reported that he has only worked at the center for a couple of months but has many years of experience working with children and is well aware of staff to child ratio guidelines. He stated that since he has worked at this center he has never experienced or observed a classroom out of ratio. During my onsite inspection, I interviewed CCSM, Alicia Kapenga. She reported that she has worked at the center for a little over a year and typically works in the 2 to 3- 1/2 year-old classroom. Ms. Kapenga stated that her classroom has never been out of staff to child ratio and she has never observed another classroom being out of ratios. On 12/22/2020, I spoke to Child A's Mother. She reported that her daughter was in the 3-year-old classroom. There are cameras in the classrooms which she often watches, classrooms are never out of staff to child ratio and there is always more than enough staff members in each classroom. Child A's Mother stated that the center is very strict about policies and she has never worried about her child's safety or health. She also reported that the staff members and Ms. Folkersma are wonderful, and she had no concerns with the center. On 12/22/2020, I spoke to Child B and C's Mother. She reported that they have attended this center for over 2 years, and she has had nothing but a wonderful experience. She stated that classroom staff to child ratios are always maintained, and staff members are very engaging.

I conducted an unannounced inspection on 9/8/2020. During my inspection, I interviewed program director Lisa Folkersma. She did not appear to be intoxicated. When I read to her the allegation regarding her being under the influence, she was shocked and said, "absolutely not." She reported that she had a bottle of wine on her mini fridge that she received as a gift from one of the child care families. I observed the bottle of wine, which had not been opened. During my onsite inspection, I interviewed Child Care Staff Member, Julie Ward. Ms. Ward started working for Adventures Learning Center in July. She reported that she has never noticed Ms. Folkersma being under the influence and has never heard any staff members having concerns regarding Ms. Folkersma being intoxicated. During my onsite inspection, I interviewed Child Care Staff Member, Reanne Tittsworth. Ms. Tittsworth reported that she started working at the facility in July. She stated that she has never noticed Ms. Folkersma being intoxicated. She stated that she has never heard Ms. Folkersma slur her speech or stumble to indicate that she may be under the influence. During my onsite inspection, I interviewed Child Care Staff Member, Dan Kapenga. Mr. Kapenga stated that he has been working at the facility for 9 years. During those 9 years he has worked directly with Ms. Folkersma for 6 or 7 years. When I read him the allegations regarding Ms. Folkersma being intoxicated, he said "no way!" He explained that he opens the facility every morning with Ms. Folkersma and works with her directly every day and has never had any reason to believe she was under the influence. On 10/26/2020, I spoke to Child A's Mother. She reported that has never seen Ms. Folkersma intoxicated or appear to be intoxicated. Child A's Mother added that she "has nothing but nice things to say about Ms. Folkersma and has no concerns." On 10/26/2020, I spoke with Child B's Mother. She reported that she has never had any reason to believe that Ms. Folkersma has been intoxicated. On 10/26/2020, I spoke with Child C's Mother. She reported that when she drops off her child every morning Ms. Folkersma greets them and never appears to be intoxicated.

(3) In each room or well-defined space, the maximum group size and ratio of child care staff members to children, including related to a staff member or the licensee, must be as shown in Table 4: d) For preschoolers, 4 years of age until school-age, there shall be 1 child care staff member for 12 children.
On 9/8/2020, when I completed my onsite inspection, I interviewed Ms. Folkersma. She reported that the classrooms always maintain the staff member to child ratios. She explained that due to the Coronavirus pandemic, they are operating with more child care staff members because they need more staff in the building to help with extra cleaning. I observed attendance records for child care children and staff for August 2020, which indicated that classrooms were operating within ratio guidelines. I also observed every classroom with Ms. Folkersma and every classroom was well within their staff to child ratios. I observed: • The Infant Room had 8 infants and 2 Child Care Staff Member (CCSM). • The Toddler Room had 8 toddlers and 2 CCSM. • The Wobbler Room had 4 toddlers and 2 CCSM. • The 2-2 classroom had 10 children and 3 CCSM. '/2 • The 2 '/2 3 year old classroom had 7 children and 1 CCSM. - • The 3 year old classroom had 10 children and 1 CCSM. • The 4 year old classroom had 8 children and 1CCSM. • The PreK classroom had 11 children and 1 CCSM. During my onsite inspection when I interviewed child care staff member, Julie Ward, Reanne Tittsworth and Dan Kepenga. They all reported that classrooms are always maintained at required staff to child ratio. On10/26/2020, Child A's Mother stated that she has not been in the building for several months due to COVID19, therefore she is unsure if the classrooms are within staff to child ratios. She reported that before COVID19 when she would drop off and pick up, she entered her child's classroom and never had any concerns regarding ratio within her child's classroom. On 10/26/2020, Child B's Mother stated that she is no longer allowed in the building due to the COVID19 protocol, however she never had a concern before COVID19. On 10/26/2020, Child C's Mother stated that she worked in the child care field and is very familiar with staff to child ratios. A couple years ago, she had concerns regarding ratios that that were addressed; she no longer has those concerns. CONCLUSION: VIOLATION NOT ESTABLISHED
