On 08/24/2021, I spoke to Child A's mother via telephone, and she stated that Child A had started at Eagles Nest on 08/17/2021 and attended 08/17-08/20/2021 and then again on Monday, 08/23/2021. Child A's mother stated when she enrolled Child A, she read their handbook which stated all children must be fully potty trained while attending the child care. She stated Child A is fully potty trained at home and does not have accidents there. On Friday, 08/20/2021, Child A was sent home with a bag of soiled underwear in his bag and "poop" on his bottom. Child A's mother stated that she was informed at pick up that Child A had an accident during the day. Then on Monday, 08/23/2021, at pick up, Child A's mother stated she was informed that Child A had another accident. Child A's mother noticed Child A's pants were on backwards when they got home. She stated she contacted the staff members via telephone and was told that child care staff members are not allowed to wipe children on their private areas. Child A's mother was very concerned that she was not contacted at any time throughout the day to be informed that Child A had an accident, especially since it was such a mess and so severe that he had to change completely. Child A is no longer enrolled at the child care. On 08/26/2021, I made an onsite inspection and observed the classroom where Child A was enrolled. The classroom is a large room and had one staff member and one child. The bathroom is in the classroom and up three steps in a small hallway. The bathroom has a small toilet and sink for children to use. At this time no staff members were interviewed. On 08/27/2021, I spoke to Tammy Streeter, program director of Eagles Nest, and she stated that she was not informed of the incident on either days the incidents happened and was not informed by any of the child care staff members that a child had an accident. She became aware of the incident with Child A when the principle of Byron Schools, Bob Cassiday, informed her that he had been notified by Child A's mother via email on 08/23/2021. Mr. Cassiday then scheduled a meeting on 08/24/2021 with her and all three child care staff members: Anne Boilat, Sonya Streeter and Daniell Davis. The meeting was to discuss what had happened with Child A. Ms. Streeter stated she was informed at this time that on Friday 08/20/2021 Ms. Davis was working alone when Child A had an accident in his pants. Ms. Davis said she gave him new clothes and he changed himself. Ms. Davis stated that she watched him the whole time to make sure that he washed up and got his clothes on correctly. Ms. Streeter stated that on Monday 08/23/2021, it was Ms. Sonya Streeter and Ms. Boilat in the classroom with Child A when he had an accident. Ms. Streeter stated she does not know how long Child A was at the child care from the time he 3 had an accident and picked up by his mother. She stated she will provide attendance sheets via email today. Ms. Streeter stated that in the meeting she discussed with all three staff members that if a child has an accident, they are to assist them and make sure they are fully cleaned. She stated the staff told her they had been previously informed that they were not to touch a child during a bathroom accident. Ms. Streeter advised staff this is no longer the policy. Ms. Streeter stated she also talked to all staff that they must contact parents whenever any incident occurs in a classroom with a child. The center has an incident policy, and she was able to provide me a copy via email. I reviewed the incident policy, and it confirms that the center's incident policy states that whenever an incident occurs, parents are to be contacted immediately. She stated she will be updating their policy so staff are very clear when they are to contact parents in any type of incident. On 08/27/2021, I spoke to Anne Boilat and Sonya Streeter via telephone separately. First, I spoke to Ms. Boilat, who stated that she is the lead caregiver of the preschoo
