On 03/23/2023, I completed an on-site inspection. I interviewed program director Tera Fracala, Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), and Child Care Staff Member 3 (CCSM 3). Tera Fracala filled in the classroom for each staff member so that I could interview them. Tera Fracala stated that it was just a rumor that Child A was left unattended on 03/10/2023. She spoke with Child Care Staff Member 4 (CCSM 4), CCSM 1, CCSM 2, and Child Care Staff Member 6 (CCSM 6) about the rumor and none of their stories lined up. She checked the transition tracking sheet and there were no discrepancies documented, all children were accounted for at all times. CCSM 1 stated that she has worked for the center for two years. She reported that Child A was left unattended in the Infant 1 classroom for less than two minutes on 03/10/2023 when the Infant 1 classroom was combining with the Infant 2 classroom at the end of the day. CCSM 2 was moving the Infant 1 children to the Infant 2 classroom while she was gathering the children's belongings. She completed the name to face tracking sheet once they were in the Infant 2 classroom and realized Child A was left in a crib in the Infant 1 classroom. CCSM 2 retrieved Child A from the Infant 1 classroom immediately upon discovery of him missing from their group. She stated that they are supposed to perform the name to face tracking sheet before and after each transition, but they failed to complete it before moving the children on 03/10/2023. She reported that the Infant 1 classroom currently does not have a lead caregiver. She noted that she and CCSM 2 do not qualify as lead caregivers, but she is "technically" the one in charge. She described the afternoon of 03/10/2023 as being "crazy." She usually does not skip the name to face procedure before leaving the classroom with children. She does not believe she has skipped the procedure any other time. She completed the name to face tracking sheet for all of the children as if the procedure was completed before transitioning the children to the Infant 2 classroom. I advised that by recording information after the fact indicating that the 3 name to face check was completed before moving the children to the Infant 2 classroom was falsification of the record, because that name to face check was never performed. She stated that she did not think about it in that way, but she now sees that by doing so, she recorded false information. She reported that CCSM 4 and CCSM 2 notified Tera Fracala and assistant program director Child Care Staff Member 6 (CCSM 6) about Child A being left in the Infant 1 classroom. She was never approached by Tera Fracala or any other administrators about the incident. CCSM 2 stated that she has worked for the center for three months as a child care staff member in the Infant 1 classroom. She and CCSM 1 were moving the children from the Infant 1 classroom to the Infant 2 classroom at the end of the day on 03/10/2023 when Child A was inadvertently left unattended in a crib in the Infant 1 classroom for approximately two to three minutes. She completed the name to face tracking sheet once they were in the infant 2 classroom and immediately realized that Child A was missing. She stated that she feels bad that they did not compete the name to face procedure before they transitioned the children to the Infant 2 classroom. She has definitely learned to always perform the name to face procedure before and after every transition, which became center policy approximately one month ago. She cares a lot for the children, and she understands the seriousness of the situation. She noted that they have two separate tracking sheets that must be completed for each transition, which can be cumbersome and overwhelming to do both sheets. On 03/23/2023, I completed an on-site inspection. I interviewed program director Tera Fracala, Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), and Child Care Staff Member 3 (CCSM 3). CCSM 3 stated that she was told by her coworkers that Child Care Staff Member 5 (CCSM 5) was working in the Infant 1 classroom with CCSM 1 on 03/21/2023 when CCSM 5 put her Child B's out of frustration. i" I spoke with Tera Fracala about the allegation against CCSM 5. She stated that she had no previous knowledge of the allegation. CCSM 1 affirmed that she worked with CCSM 5 on 03/21/2023. She recalled that CCSM 5 was trying to bottle feed Child B, but Child B was crying and refusing the bottle, which was typical for Child B because she is mainly breastfed. CCSM 5 became frustrated and placed her Child B's for one to two seconds. CCSM 3 stated that she did not confront CCSM 5 about her actions because she did not feel comfortable doing so. She could not immediately report the incident to Tera Fracala because she was not present in the center. She notified Tera Fracala of the incident on 03122/2023 when she relieved her for a bathroom break between 9am and 1O am. She also told Tera Fracala that she did not want CCSM 5 back in her classroom. I asked CCSM 1 if she felt that CCSM 5's actions towards Child B were abusive in nature. CCSM 1 stated that she did find CCSM 5's action to be abusive in nature. I advised CCSM 1 that she is a mandated reporter and asked her why she did not report her concerns to Children's Protective Services (CPS). She did no explanation as to why she did not report her concerns to CPS. I spoke with Tera Fracala for a second time. She stated that she did not recall having a conversation with CCSM 1 about CCSM 5 putting her Child B's . She did remember CCSM 1 telling her that she did" nowant CCSM back in her classroom, but she could not recall CCSM 1 giving her a reason why. On 04/03/2023. l completed a second on-site inspection with era Fracala was not present. We interviewed assistant irec or CCSM 6. She was not present in the center when the incident allegedly occurred, but CCSM 1 told her that CCSM 5 put her an infant's after the fact. She noted that CCSM 3 abruptly quit on 03/2712023, CCSM 1 was not present, and CCSM 5 was placed on administrative leave due to the allegations against her. She denied having any concerns with CCSM 5 inappropriately handling any child. She has recently worked in the infant and toddler classrooms with CCSM 5, and she has never witnessed anything of concern. She has never witnessed CCSM 5 be aggressive with the children. She believes that CCSM 5 does "pretty well" with the children. On 04/03/2023, and I interviewed CCSM 5 at her residence. She stated that she was told a 4, CCSM 1 and CCSM 3 brought false allegations against her, that she Child B . She stated that anyone who knows her knows that she would never harm a child. She spoke with Tera Fracala and CCSM 6 about the allegations. If a child refuses to take a bottle, she takes a break and tries again later. She noted that a bottle is good for one hour, so there is plenty of time to bottle feed a child. She knows to call Tera Fracala if she gets overwhelmed and needs a break from the classroom, but she hardly ever gets overwhelmed with the infants because they do not know any better. On 11/23/2022, I completed an unannounced on-site inspection and interviewed CCSM 1 about the incident. CCSM 1 reported that she is the lead caregiver in the Discovery Preschool classroom and has worked at the center for two months. She reported that the incident occurred at 3:25 PM. She was working alone with eight children and left the classroom with the group to go get supplies for another classroom. When she left the classroom, she completed the name to face process at the first threshold and when she completed it at the second threshold, she discovered that Child A was not with the group. He was walking in the middle of the group, so she does not know how he ended up not continuing with them and does not know if he turned around and went back or how he got back in the room. She explained that Child A was lined up and at the end of the line when they first walked down the hallway. Once she knew he was not with the group after 2 completing the name to face process at the Toddler 3 classroom, she went back to find him and he was in the classroom, was not crying, and seemed fine to her. She thinks that he was in "Ia la land" and was just standing there. She estimates that Child A was separated from the group for about 2 minutes. CCSM 1 showed me her classroom and the Toddler 3 classroom that she walked to. The Toddler 3 classroom was on the side of the gym and across the hall from the Discovery Preschool classroom. On 02/07/2023, I spoke with Child A's Mother who reported that shortly after the incident, she pulled Child A from the center because she had concerns that Child A had a concussion after falling in the gym. She did not take him to the doctor but he had a "goose egg" on his head. She said that CCSM 2 contacted her to report that Child A had been unsupervised for 2 hours. She said that CCSM 2 was "super upset" when she reported it to her and that they did not realize that Child A was missing. She said that he was playing in the classroom by himself, the door was left open and another child care staff member found him. Per their records, they had not checked on him so that is why they did not know he was missing. CCSM 1 told her a couple days later that Child A was only unsupervised for a couple minutes. On 02/07/2023, I spoke with program director Tera Fracala via telephone about the allegations. She reported that CCSM 1 was working by herself when the incident occurred. Ms. Fracala reported that Child A has an older sibling who still attends the center. On 02/07/2023, I spoke with CCSM 2 who said that she remembers calling Child A's Mother but denied telling her that Child A was unsupervised for 2 hours. She said that there were no other incidents of Child A being unsupervised, so she is not sure why Child A's Mother had the wrong information. The center submitted an incident report as required. The incident report details that when CCSM 1 completed the name to face process at the Toddler 3 classroom, she realized that Child A was not with her. She went back to the classroom, and he was standing in the classroom. The report stated, "Approximate time frame is under 2 minutes."

On 06/14/2022 I completed an unannounced visit to the child care center. I interviewed Program Director Tera Fracala. Ms. Fracala stated she was made aware that staff member Amber Taylor-Crysler might have slapped or hit Child A when she placed her hand on Child A's shoulder on 06/08/2022. Ms. Fracala stated she was not present when the incident happened, and it was brought to her attention. There were no marks, bruises, or injuries to Child A. Child A's Mother was notified that there was an accusation regarding Ms. Taylor-Crysler. Child A's Mother did not notice any injuries or have any concerns and stated she would follow up with 2 child care center staff if she needed to. Ms. Fracala stated she has never had any concerns or issues with Ms. Taylor-Crysler. I interviewed staff member Kylee Meyers. Ms. Meyers stated she is the assistant staff member in the infant room. She was working with Amber Taylor-Crysler on 06/08/2022. While in the classroom she heard what sounded like a dull clap noise and Child A was crying. She did not see what happened but only heard what seemed to be an aggressive touch. Child A was crying before the incident and the noise that she heard, and Child A continued to cry after the noise. Ms. Meyers explained the noise was not as loud as a slap but seemed to be louder than what would normally happen when you touch someone's shoulder. She has not had any concerns with Ms. Taylor-Crysler, but she felt that this incident seemed wrong. She denied seeing any red marks, any bruises, or any other types of scratches or marks on Child A. She feels that although she did not see what happened, it was still a concern. I interviewed Lead Caregiver Krystal Groendyk. Ms. Groendyk stated she has worked in the infant room since November 2021. She trained Ms. Taylor-Crysler. Ms. Groendyk stated she has never had any concerns regarding Ms. Taylor-Crysler and she feels that Ms. Taylor-Crysler provides amazing care to the children. Ms. Groendyk stated she does not feel that Ms. Taylor-Crysler put her hands on Child A in an aggressive manner and that staff member Kylee Meyers was overreacting to what happened. Ms. Groendyk stated she was not there when the incident occurred on 06/08/2022, but has never seen Ms. Taylor-Crysler become aggressive, be upset in the classroom, or become agitated towards any child in care. She feels that this incident might have been made up because one staff member did not like another staff member. Ms. Groendyk stated she would let Ms. Taylor-Crysler watch her own children at anytime and again, has no concerns about the care she provides. On 06/14/2022 I contacted staff member Amber Taylor-Crysler. Ms. Taylor-Crysler stated she was placed on leave from the center because someone had made an allegation about her. She has never been aggressive, upset, angry, or violent towards any child at any time. She feels that someone in the classroom is mad at her for some reason and made up an allegation against her. She never slapped, grabbed, or pulled Child A on 06/08/2022, or any other time. She has never touched any child wrong, ever in her life. Ms. Taylor-Crysler stated she feels she provides great care to the children in the classroom. On 06/14/2022 I contacted Child A's Mother. Child A's Mother stated she was made aware of the potential of something happening to her child at child care. Child A's Mother stated she does not think anything happened to her son. Ms. Taylor-Crysler is an amazing child care provider, and she has never had any concerns. Child A's Mother stated she did not notice anything marks, bruises, or red marks on her child on 6/08/2022. She has continued to take her child to the center and is hoping that Ms. Taylor-Crysler can return to work as soon as possible because she does such a great job. 3 On 06/14/2022 1 contacted Child B's Mother. On 04/15/2022 Tara Fracala, program director, contacted her assigned child care licensing consultant to report Child A, male age 2, had been left unattended in a classroom. She also emailed the incident form to her on this date. On 04/21/2022 I made telephone contact with Child A's Mother. She was aware of the incident and said Ms. Fracala had told her Child A was left in the classroom. The incident happened when the class was outside and during the transition inside to the activities that were taking place inside, "he got away from the group" and was found in his classroom. She had no concerns about the incident. On 04/25/2022 I conducted an unannounced on-site inspection to the facility. Ms. Fracala was not present at the center. I met with Julie Horjus, child care staff member (CCSM), outside the presence of others. She said she is the Discovery Preschool lead caregiver. On the day of the incident the center was having an Easter event. This included activities for parents to participate in with their children, refreshments, and other things which had people moving around the center. The class was coming in from the playground with the intent on going to the school-age room for activities taking place in that room. Ms. Horjus stated she had Child A in the school-age classroom and then was distracted with the activities when Hailey Wierchz, CCSM, came in with Child A stating she had found him in the Discovery Preschool classroom unattended. Ms. Horjus said she has no idea how Child A left and has no memory of him not being with the group. She acknowledged the face to name process was not done when the class moved from outside to the school-age classroom. I then met face to face with Krystal Greondyke, CCSM, outside the presence of others. She was walking down the hallway from the school-age room and saw Child A in the classroom unattended. She also said Child B, female age 2, was with Child A unattended in the classroom. While she was in the classroom Hailey Wierchz saw her in there and asked why she was there, and she told her Child A and Child B were unattended. I then met with Christina Felan, CCSM, outside the presence of others. She is the assistant in the Discovery Preschool and was working with Ms. Horjus on the day of the incident. She explained that when the class moved from outside to inside the center they went to the school-age room and then, "realized Child A was gone." She said Ms. Greondyke told her Child A was in the classroom unattended. She said 3 she and Ms. Horjus did not do the face to name process when they moved the children from outside to inside and then into the school-age classroom. She said the Easter event had a lot of people moving about the center and there were distractions that kept them from following through with processes and procedures they would normally do. She denied Child B was left unattended in the classroom. She did not know the reason other staff said this took place. I then met face to face with Hailey Wierchz, CCSM, outside the presence of others. She is an assistant in the toddler 2 classroom. On the day of the incident, she saw Ms. Greondyke in the Discovery Preschool classroom with Child A and Child B. She thought this was strange and asked her the reason. She said, she told her Child A and Child B had been left unattended in the classroom. On 06/07/2022 I spoke with Tara Fracala by telephone about this incident. She said she had arranged an Easter celebration for families on this date. This included an Easter egg hunt and other activities which had families and children moving about the center. She was in the school-age classroom overseeing activities for everyone and was told Child A had been left in a classroom unattended. She said she was never told Child B was found in the classroom with Child A. Since the incident took place, she has provided training for staff on supervision, the face to name process, and observed staff completing this process.
