On 11/09/2022, I received a written incident report submitted to Mary Dufon, child care licensing consultant, by program director Carrie Ondersma. The incident report states that CCSM 1 carried Child A by the coat to the exterior door on 11/07/2022, where she sat Child A down in the doorway and let the exterior door close on her. Additionally, CCSM 1 was placed on non-disciplinary paid administrative leave while the district investigates the incident for potential misconduct. According to report, conducted an unscheduled visit at s "ami home on 11/12/2022. interviewed Child A's Mother. Child A's Mother stated that the center called her to inform her of the incident. Child A's Mother denied Child A having any marks or bruises. Child A's Mother also denied having any concerns with the center or this incident. Child A's Mother does not feel that CCSM 1 had any malicious intent. Child A's Mother stated that Child A throws herself down on the floor or throws a fit when she does not want to do something. conducted a visual observation of Child A. Child A was void of any RvisiPem"ar or injures. Due to her young age and her limited language skills, Child A was unable to be interviewed following the Michigan Forensic Interview protocol. Rs According to report, spoke with program director Carrie Ondersma by telephone on 1 . Ondersma told that she has spoken with Child A's Mother multiple times regarding the incident. Child A's Mother expressed to Ms. Ondersma that she fully trusts CCSM 1 and has never had any concerns related to the care Child A has received at the center. Ms. Ondersma advised that CCSM 1 is the lead caregiver in Child A's classroom, along with o paraprofessionals to assist. On 11/07/2022, CCSM 4 and CCSM 5 were working in CCSM 1's classroom, but they did not witness the incident and have no knowledge of it. Ms. Ondersma stated that CCSM 1 was by herself with three children when the incident occurred. Ms. Ondersma stated that CCSM 1 did not seem to be reacting out of anger or frustration during the incident, but her concern is if CCSM 1 has the 'common sense' to realize that her actions were wrong. Ms. Ondersma expressed concern to with how CCSM 1 may handle a child in a moment of frustration given how she handled Child A during a time when she was calm and not agitated. Ms. Ondersma advised that CCSM 1 is currently on paid administrative leave but would likely be terminated by the end of the week. CCSM 1 has worked at the center since 2006. According to report, received a voicemail from Ms. Ondersma indicating that CCSM 1 submitted her resignation on 11/14/2022. According to report, received a telephone call from Child A's Mother. Child A's Mother stated that er husband reviewed the video footage of the incident, and, while she does not agree with how the staff member handled Child A, she does not believe there was ill intent. Child A's Mother stated that she was surprised that th was contacted because she does not believe s actions rose to the level abuse or maltreatment. M. s . According to report, contacted Ms. Ondersma by telephone on Ondersma confirmed that she had video footage of the incident. On 11/22/2022, I completed an on-site inspection with 3." We interviewed program director Carrie Ondersma, CCSM 2 CCSM s ndersma stated that there has been a history of difficulties with CCSM 1's classroom transitioning from the classroom to the playground. Previously, the entire classroom would go out at the same time, and it was common for CCSM 1 to let the children run up to the door and wait for her. Ms. Ondersma discussed with CCSM 1 how the transition was creating difficulties and that it was not best practice. CCSM 1 agreed with Ms. Ondersma that the transitions have been hard and began splitting the children into smaller groups between her and the other two staff members for outdoor transitions. CCSM 1 is not conducive to the welfare of children. She placed Child A in the doorway and allowed the heavy exterior door to close on her. Child A was pinned for approximately 13 seconds before she freed herself. CCSM 1 stood and watched the door close on Child A before briefly walking away. CCSM 1 placed Child A at significant risk of harm when she allowed the door to close on Child A.

Appropriate care and supervision was not provided to Child A and Child B on 5/4/2021: • Child A and Child B were seconds away from getting over the top of a four-foot fence, which is located very close to Byron Center Avenue. The lack of supervision posed a threat to the health and safety of both Child A and Child B on 5/4/2021. • Child A and Child B were left unsupervised while outdoors for approximately five minutes on 5/4/2021. • Staff failed to count children after coming indoors on 5/4/2021. • Child B fell from the top of a four-foot fence on 5/4/2021 due to lack of supervision. • Child A and Child B's lead caregiver, Sara Hengst was supposed to take a 15-minute break and instead she took a break for approximately 36-40 minutes, during which time the incident occurred. • Child care staff members Carrie Hinton and Sara Hengst did not notice Child B was missing until after looking for Child A.
