On 08/05/2022, I completed an unannounced onsite inspection at the center. I interviewed program director Terrie Stratton, child care staff members (CCSM's) Alexandria Henson, Taniyah Swazer, and Riley Scobey. Ms. Stratton stated that she did not recall a time that an infant under one year of age that was served baby food, without parental permission. She confirmed that there is only one infant in attendance at the center that is under 6-months of age and provided me with Child A's child information card. She also provided me with the names and contact numbers of all of the CCSM's that work in the infant room. 2 During my interviews with Ms. Henson and Ms. Swazer, they confirmed that Child A is the only infant that the allegations could pertain to. Ms. Henson never cared for Child A and was not aware of any other CCSM feeding baby food to her. Ms. Swazer has cared for Child A, only fed her a bottle and was not aware of any other CCSM feeding baby food to her. During my interview with Ms. Scobey confirmed that Child A is the only infant that the allegations could pertain to. She has cared for Child A and only fed her a bottle. She did recall approximately two weeks to one month ago that CCSM Stacy Briggs accidently fed Child A orange baby food. She found out as the following morning Child A's Mother arrived at the center and notified them that Child A was vomiting up the food and was constipated. I reinterviewed Ms. Stratton, who stated she did not recall the incident and reviewed her text messages. She located the string of text messages with Child A's Mother on 07/13/2022 which confirmed the incident occurred. She said it was not made into a big deal, so that must have been why she did not recall the incident. If she were made aware of it occurring, she would have definitely notified Child A's Mother the day that it occurred. On 08/17/2022, I left a voicemail message for Ms. Briggs. She returned my call and confirmed that she accidentally fed Child A the baby food carrots, as she got her mixed up with another child that is on baby food. After she realized what she did she threw the food away and made note of it. She was unsure if anyone notified Child A's Mother and didn't think it was an issue, as it was only two bites that she fed to Child A. She thought Child A's Mother brought it up the following morning and that Ms. Stratton spoke with her. I clarified the issues of an infant receiving baby food and potential allergies. 10/07/2022 I spoke with Child A's Mother who reported that her daughter, at the time of the incident, potentially had severe food allergies as her other children and Child A's Father do. She recalled Child A vomiting up orange and being severely constipated. She called the center and spoke with Ms. Stratton, asking her if any of the CCSM's had fed Child A anything other than her bottled formula. Ms. Stratton told her she was not aware of such. She notified Ms. Stratton of Child A's symptoms and Ms. Stratton told her she would look into it and get back with her. Ms Stratton later told her that Child A was fed level three baby food carrots. She confirmed that Child A did not receive medical treatment. 3

On January 21, 2022, I was contacted by program director Terrie Stratton, who indicated that she received a text message from Child A's Mother the night before alleging that Child A was sexually abused at the child care center in December 2021 or prior to that. She alleged that Child B exposed himself to Child A and now he has to go to therapy for it. Child A's Mother then posted this information on their center's social media site about Child A being abused in the center. I requested a copy of an incident report detailing the allegations. On January 22, 2022, Ms. Stratton forwarded me the text messages, social media post and the incident report. The text message from Child A's Mother stated "so we have a real big problem all of you knew Child B made Child A look at his dick? No wonder he was acting out you guys were mad but took it out on my child." The social media post alleged that "y'all are down bad, the whole staff knew my child was sexually abused not only hid it but expelled my child to cover it up." The incident report from the center documented the allegations made and Ms. Stratton's contact with the family and staff. On January 22, 2022, I interviewed Ms. Stratton via telephone in regards to the incident report. She stated that she has no idea where these allegations are coming from. Nothing has ever been mentioned to her prior to yesterday about Child B exposing himself to Child A. Since the social media post, the center's Facebook page has been blowing up from Child A's Mother's friends and they are making death threats (all of which have been deleted). Prior to the allegations, Child A was having several negative behaviors at the center in December 2021. Ms. Stratton and her staff addressed this both verbally and in writing to Child A's Mother. On December 15, 2021, they issued a discharge notice to Child A's Mother after he had several physical outbursts at the staff that day. He was throwing chairs, making threats, and biting the staff. It had come to the point that the other children and staff were not safe around his behaviors. They told her he could come back only after receiving counseling. His younger sibling continued to attend the center up until now. This was the first indication that Child A's Mother had any concerns with the center. Ms. Stratton interviewed all her staff and no one knows what she is talking about. On January 25, 2022, I interviewed Child A's Mother. She stated that Child A attended the center up until December 15, 2021. That is when they handed her a packet of papers telling her about incidents that she had no idea about and that he was expelled. Since that time, he has been peeing his pants and hiding it. He never had accidents in his pants before this. He started attending the center in April 2021. He was doing great and loved it, but at the end of October he started peeing his pants for no reason. Then last Thursday, while he was at Relative 1's house, he told Relative 1 that Child B had 3 showed him his private parts on his last day at the center. Relative 1 asked when this happened and he stated that "while in the kitchen area of the center Child B showed me his pee-pee." At the time he immediately yelled and that drew the teacher's attention. All of the teacher's knew about it and were in the office yelling about it. Child A's Mother has not talked to him about it. She did not want to ask any more details or get him confused. She has contacted an attorney and the Burton Police Department to make a complaint against Child B and the center. She texted Ms. Stratton once she found out but she denied knowing anything about this, which she does not believe. She never wanted to send her children to the center but she had no choice. The staff are always on their phones when she picks them up. Her younger son attended the center too up until last week. She did not have any concerns regarding his care at the center.
