On May 25, 2022, I received a telephone call from Cindy Hoover, Education Coordinator regarding an incident at the child care facility. Ms. Hoover reported that she was contacted by Child A `s Mother on May 24, 2022, to report that when Child A was picked up on May 23, 2022, Child A normally runs to her, she did not run, and could not walk. Child A's Mother was inquiring about what happened in the classroom regarding Child A. Child A's Mother was told by a child care staff member that Child A had 'ust worked up from nap and will still very groggy. After speaking with Family Engagement Coordinator, Ms. Hoover identified that Child A has tripped on a stool by the sink, on May 23, 2022, but was not injured. Ms. Hoover is not aware if Child A received any medical treatment. Child A's Grandfather called the Burton Police, and an officer came to the facility on May 25, 2022. Ms. Hoover did not have the officers name at this time and child care staff were spoken to by the Burton Police officer. On June 2, 2022, I completed an onsite inspection at the facility with Crecendra Brown, Area Manager. I interviewed was present in the classroom on May 23, 2022 and was acting as a substitute child care staff member. -indicated that child care staff member Linda Meyerhofer and child care staff member Courtney Brokaw were also present on May 23, 2022. The day was typically as far as activities. Child A was sleepy when she was dropped off and slept until 9:00 am. When Child A woke up, she had breakfast and participated normally in all classroom activities. Child A was walking and moving about the classroom normally. Child A had lunch around 11:45 am. After lunch Child A walked to the sink to dump her milk cup out. There is a small rectangular stool in front of the sink. Linda Meyerhofer walked Child A to the sink, Child A tripped placing her hands on top of the stool, spilling some of her milk. She had one leg on the stool and one leg on the floor. Child A did not cry. Linda Meyerhofer checked out Child A over for injuries. Child A walked normally to her cot and was not limping, crying, or expressing any discomfort. Child A laid down and took a nap. Child A's Mother picked her up around 2:45 pm. Child A was still asleep when her mother arrived. County Brokaw sat Child A down at the doorway by Linda Meyerhofer, who was helping take children to the front door. Parents are not permitted into the building to drop off or pick up. Linda Meyerhofer, grabbed Child A's backpack and said "Lets walk to mom," Child A walked about halfway down the hallway near the drinking foundation when Child A's Mother rushed down the hallway and picked up Child A. Child A's Mother said something to Linda Meyerhofer, did not hear what, and walked out of the door, carrying Child A. Child A's Mother spoke with Ms. Hoover on May 24, 2022, stating that Child A was having trouble walking. Child A's Mother indicated Child A would not be returning to the facility. Child A's Grandfather called Ms. Hoover. Ms. Hoover was unable to speak with Child A's Grandfather because he is not Child A's legal guardian. On May 25, 2022, Child A's Grandfather came to the facility and 3 indicated he wanted child care staff to call the police. Child care staff indicated to Child A's Grandfather that he could phone the police if he decided he wanted them contacted. Child A's Mother does not like for Child A to walk, she prefers her to be carried. Child A does not have any developmental delays or individualized educational plans. Child A can participate in all classroom activities normally, including running and playing outside. An incident or "boo, boo" report is provided when a child has a physical injury, mark, bruise, limping, extremely upset following an injury or has concerning behavior following an incident. Child A did not display any signs of injury or distress on May 23, 2022. does not have any concerns with the care and supervision provided by the facility. On June 2, 2022, I interviewed
