On 03/17/2021, I received a voice mail message stating that a child was hurt at the child care center and taken to emergency. The program director, Laura Holbrook, emailed a written incident report regarding the matter on 03/17/2021. The incident reported stated on 03/16/2021 at 10:35 a.m., Child A told staff that he reached through the wardrobe and it fell on him. The staff did not see the incident happen. The tape was viewed to determine what happened to Child A's finger. The video showed Child A was playing and clean up was announced. Child A went over to the wardrobe in the Dramatic Play area and took hold of the top edge 2 of the wardrobe. He pulled on the edge and the wardrobe tipped over his head backwards. Child A's head and shoulder went through the wardrobe opening, but his left hand was caught under the top of the wardrobe. Child A's hand was swollen, bruised, and scrapped. Ice was applied immediately to his hand and Child A's Mother was called. The swelling continued so they called Child A's Mother again and asked her to pick him up. Child A's Mother arrived quickly and took him to emergency. His head was x-rayed and a small fractured was located on the outside of his pinky finger. Child A's finger was put in a splint. An appointment was scheduled with a specialist on 03/17/2021 for further evaluation. On 03/19/2021, an amended incident report was emailed stating that Child A's two fingers were placed in a cast and needed to be reevaluated in two weeks. On 03/26/2021, I completed an unannounced onsite inspection at Karnak Creative Child Care Center. I spoke to Laura Holbrook, program director regarding the allegations. I observed the Turtle GSRP classroom, there were no children or child care staff members present due to the class being closed for break. I observed the wooden wardrobe that fell on Child A's hand. The clothing wardrobe was approximately three feet tall and three feet long in the Dramatic Play Area. The 1/2 1/2 wardrobe is free standing and is used to hang the children dress up clothing. The wardrobe was standing away from the wall and being used to divide the space. The attendance sheet from 03/16/2021 and video footage was provided. On 03/26/2021, the attendance sheet and video were reviewed. There were 13 children present with two caregivers on 03/16/2021, according to the attendance sheet. The video provided is a six minute, no sound video. In the first minute and a half, It shows the children in the Turtle room playing in various areas of the classroom. Ms. Tince can be seen on the video, sitting at a table interacting with two children and Child A is behind her in the dramatic play area. Ms. Malgorzata walks into the classroom and she says something to the children. They all stop look at her and then start to clean up. Child A goes over to the wardrobe and put both of his hands on top of the wardrobe while he is kneeling down. He swings on the wardrobe and looses his balance. Child A falls back on his hand and the wardrobe falls over him. Child A body goes through the opening of the wardrobe and it lands on his hand. Ms. Tince goes over and pick up Child A and bring him to the table. She talks to Child A, looks at his hand and rubs his back to comfort him. Ms. Tince and Ms. Malgorzata can be seen talking and then Ms. Tince takes Child A out of the classroom. Ms. Malgorzata picked up the wardrobe and the children continued to clean the classroom. On 03/31/2021, I spoke to Child A's Mother regarding the allegations. Child A's Mother stated that she received a call from the Karnak Creative Child Care center on 03/16/2021. She believes the call was from the center's receptionist. She was informed that Child A was injured, that something fell on Child A's hand while he was cleaning up. His hand was bruised but he could move his fingers. The child care called back about five minutes later, stating the swelling was getting worse and that 3 Child A needed to be taken to emergency. According to Child A'
On 03/16/2021, Child A (4-year-old, male) was injured and fractured his left pinky while at Karnak Creative Child Care Center.
