On 1/22/2021 I completed an unannounced on-site inspection at the child care center. I interviewed child care staff member Casey Wertz. Ms. Wertz said she has not seen any "random" people come to work at the child care center. The same few people work every day. Ms. Wertz said licensee, Darlene Brooker, was not currently present at the child care center. I interviewed staff member Terri Sebring. Ms. Sebring identified three full time staff members and one part-time staff member who regularly work at the child care 2 center. Ms. Sebring could not recall any "random" people coming to the child care center to work. On 2/16/2021 I completed an unannounced on-site inspection at the child care center. I observed licensee, Darlene Brooker, in her car in the parking lot of the center. Ms. Brooker said the center is closed for the day due to severe weather. Ms. Brooker said she came to the center to verify that the parking lot had been plowed. I reviewed the allegations with Ms. Brooker and interviewed Ms. Brooker in the parking lot. Ms. Brooker said she does not have "random" people come work at the center. Ms. Brooker and her regular staff members are the only people who care for children at the center. On 2/18/2021 I completed an unannounced on-site inspection at the child care center. The staff members present were Ms. Wertz, Ms. Sebring and Ms. Brooker. There were no "random" adults present. I reviewed staff member files and found the staff members who are listed as employees to have the required comprehensive background check and trainings to qualify them as child care staff members. On 3/26/2021 I placed a telephone call to Child C's Mother. I left a voice message requesting a return telephone call. On 3/26/2021 I spoke to Child A's Mother by telephone. Child A's Mother said Child A is no longer attending the child care center due to Ms. Brooker making the decision to close her infant and toddler room. Child A's Mother said there were two of the same staff members who were at the child care center daily. Child A's Mother said there was one staff member who Child A's Mother only saw a couple of times at the child care center. On 3/26/2021 I spoke to Child B's Mother by telephone. Child B's Mother said the same staff members were present daily when she dropped Child B off at the child care center in the morning. Child B's Mother said sometimes there would be a different staff member at the center when she arrived to pick up Child B. Child B's Mother said she believes all the staff members she interacted with were regular child care staff members. On 3/36/2021 I completed an exit conference with licensee Darlene Brooker. 3
Random people are working at the center. There is a lack of space for the number of children enrolled. The center is short staffed.

On 1/22/2021 I completed an unannounced on-site inspection at the child care center. I interviewed child care staff member Casey Wertz. Ms. Wertz was able to identify the overall licensed capacity of the center and the capacity limits for each classroom. Ms. Wertz said the child care center follows the capacity limits in each classroom and overall capacity at the center. I interviewed staff member Terri Sebring. Ms. Sebring said she mainly works in the infant and toddler room. She rarely has more than four children in the classroom. I observed both classrooms and found both to be within the licensed capacity. I reviewed attendance records and found that for the past two weeks each classroom had an appropriate number of children signed in to meet the licensed capacity of the center. On 2/16/2021 I completed an unannounced on-site inspection at the child care center. Ms. Brooker said she has been maintaining the licensed capacity for both the classrooms and the overall center capacity. On 2/18/2021 I completed an unannounced on-site inspection at the child care center. I observed both classrooms to be at the licensed capacity. On 3/26/2021 I spoke to Child A's Mother by telephone. Child A's Mother said the child care center is very small overall. Child A's Mother said it does not seem like the facility is big enough for a lot of children. Child A's Mother has never counted the total number of children at the child care center. 4 On 3/26/2021 I spoke to Child B's Mother by telephone. Child B's Mother said although the facility is small, she believes there is enough space for the number of children who attend the center.

On 1/22/2021 I completed an unannounced on-site inspection at the child care center. I interviewed child care staff member Casey Wertz. Ms. Wertz said there have been problems in the past where staff members were left with too many children causing them to be over ratio. A different child care licensing consultant came to address the problem in the past. The ratios have been better since then. I interviewed Ms. Sebring. Ms. Sebring said she is usually the only staff member with four children in the infant and toddler room. Occasionally there is an overlap in scheduling where there will be two staff members with four children. If there happens to be more than four infants and toddlers there are always two staff members in the classroom. Ms. Sebring said she had not seen the other classroom to be over ratio. I observed both classrooms and found both classrooms to be within the appropriate ratio for the age groups of the children in each classroom. On 2/16/2021 I completed an unannounced on-site inspection at the child care center. I interviewed Ms. Brooker. Ms. Brooker said she has had problems with maintaining ratios at the center in the past, but she has been following ratios exactly as she should since a recent special investigation. On 2/18/2021 I completed an unannounced on-site inspection at the child care center. I observed both classrooms to be within the appropriate child care staff member to child ratios. 5 On 3/26/2021 I spoke to Child A's Mother by telephone. Child A's Mother said she never counted the number of children in the infant and toddler room. Child A's Mother said it was usually at least two or three child care staff members in the classrooms when she dropped off Child A. On 3/26/2021 I spoke to Child B's Mother by telephone. Child B's Mother said she believes there are enough child care staff members present to care for the number of children at the center.

On 10/13/20, I conducted an unannounced onsite inspection to the center. Upon arrival at the center, Ms. Casey Wertz was engaged in outdoor play with a mixed age group of 11 children ranging in ages from 1 years old to 5 1/2 years old. Within a short time, an additional staff member, Ms. Rachael Peterson, came outside with an additional child whom she had to take into the center momentarily for a bathroom break. There was now a mixed age group of 12 children ages 1 years old to 5 years old with two staff members. Based on the mixed age 1/2 group of children an additional staff member was needed to comply with the staff member to child ratio requirement of 1:4. 3 I introduced myself and asked the whereabouts of Ms. Darlene Brooker, licensee designee. Ms. Wertz informed me that Ms. Brooker had just left momentarily to run errands and was expected to return to the center shortly. I separately interviewed Ms. Wertz and Ms. Peterson and informed them of the complaint. Both staff members acknowledged being out of compliance with the child care ratio requirements. They reiterated that Ms. Brooker just left the center momentarily to complete some errands but should return shortly. Ms. Wertz informed me that she has been employed at the center for approximately three weeks and predominately works with the infant and toddler children. Outside of today, Ms. Wertz denied the center operating in non-compliance with the child care ratio requirements and said since she has been working, the center has been fully staffed each day and operating within the correct staff member to child ratio requirements. Ms. Wertz denied the infant and toddlers not being cared for properly. Ms. Wertz stated she worked in the infant toddler classroom on 10/12/20 and said she was within the correct staff member to child ratio requirements. Ms. Peterson has been employed at the center for approximately two weeks and works in the infant and toddler classroom. Ms. Peterson reiterated Ms. Wertz's statement regarding the center being fully staffed and operating within the correct staff member to child ratio requirements. Ms. Peterson also acknowledged the center not being in compliance during my onsite visit as Ms. Brooker left the center momentarily to complete errands. Ms. Peterson also denied the infant and toddlers not being properly cared for and said on the days she has been working, the center has been fully staffed. Within 10 minutes of my arrival at the center, Ms. Brooker returned from her errands. I informed Ms. Brooker of the allegations and that the center was currently operating in non-compliance with the staff member to child ratio requirements as there were only two staff members present with a mixed age group of 12 children ranging in ages from 1 years old to 5 years old. An additional caregiver was needed to be in 1/2 compliance with the staff member ratio requirement of 1:4. Ms. Brooker admitted to the center being temporarily in noncompliance with the ratio requirements as she needed to leave momentarily to get "some drinks and coffee." She asked me what she was supposed to do when she only had herself and two other staff members working. I informed Ms. Brooker when a staff member needed to leave for any reason, the center would need to either find an additional staff member to come into work or have parents come and pick up their children early to ensure the center maintains the correct staff member to child ratio requirements at all times. I further explained to Ms. Brooker the importance of maintaining these requirements to ensure appropriate care and supervision can always be provided to the children as the lower staff to child ratios help staff members know the whereabouts of children and the activities they are engaged in at all times. 4 Ms. Brooker and I reviewed the center daily attendance records. Ms. Brooker informed me that she is currently working on developing an automated attendance record system (Budget wheel) but does not have all

On 10/26/20, the department received an additional complaint alleging that the children are not being taken care of properly as on several different occasions the children have been given the wrong blankets, bottles, and pacifiers. This allegation was added into the initial intake (intake# 175434) to be addressed. On 10/28/20, I conducted a second unannounced onsite inspection to the center. I informed Ms. Brooker of the new allegations. Ms. Broker denied the children not being properly cared for and denied the children being given the wrong blankets, bottles and pacifiers. Ms. Brooker informed me that the children bring their own blankets, meals, and bottles of premade formula to care each day. Ms. Brooker acknowledged that she keeps extra blankets at the center for children to use if they forgot to bring one from home. Ms. Brooker said the blankets are never shared 7 among children and are cleaned and sanitized daily. Regarding bottles of formula and cups, Ms. Brooker reiterated that the parents provide the children's lunches and premade formula bottles each day. Ms. Brooker and I went inside of the center for her to show me where the blankets and children's meals are kept. Each child has their own cubby space where all of their personal belongings are kept. Any perishable food items are placed in the refrigerator. Ms. Brooker informed me that when the toddlers or preschool aged children have a drink, it is done in a designated area. Ms. Brooker showed me an area in the infant toddler room where there were several sippy cups sitting. On 10/29/20 and 11/10/20, I separately interviewed Ms. Casey Wertz and Ms. Rachael Peterson, both who are staff members at the center via telephone. I informed both staff members of the new allegations. Both staff members denied the child care children not being cared for properly. Both staff members also denied the children being given the wrong blankets, bottles, and pacifiers. Both staff members reiterated Ms. Brooker's statements regarding the children bringing their own blanket, bottles, and pacifiers from home. Ms. Wertz and Ms. Peterson also acknowledged the center having blankets they keep on hand if a child forgets to bring one to care. Both staff members said the blanks are cleaned and sanitized daily and no child uses another child's blanket for nap/rest time. Regarding the pacifier, both staff members said the children only use their pacifier during nap/rest time. Currently, both staff members said there is only one child who uses a pacifier. On occasions, the other children like to give that child their pacifier as they know the pacifier belongs to that child. Both staff members also acknowledged that some of the children's food and drink items not being labeled with the child's name and date each day when they arrive at the center. I informed both staff members of the importance of labeling the children's food and drink items each day to ensure that children receive the food that was intended for them and prevents the chance of cross-contamination. The dating of beverages and food allows for the monitoring of spoilage and prevents foodborne illness. Both staff members expressed an understanding to this rule requirement and said they would be more diligent in ensuring the items are labeled each day. On 11/10/20, I interviewed Ms. Veronica Edgars, supervised volunteer, via telephone. Ms. Edgars has volunteered at the center over the past year. During that timeframe, she denied ever seeing any child not being properly cared for and said she has not seen children share blankets, pacifiers, and bottles. Ms. Edgars said her own children have attended care at the center for the past 1 years. Ms. '/2 Edgars said all of the staff members have been attentive to the children needs and noted that the children bring their own blankets, bottles and pacifiers to the center each day.
