Child A's Grandmother stated that on 06/16/2023, she picked up Child A from the center. She has never picked him up from care before. There was a male and a female child care staff member present when she arrived. The individuals did not ask her for her ID. Skyler Lipinski, program director, stated that Child A's Mother told him on 06/16/2023, that Child A's Grandmother would be picking Child A up from care. Mr. Lipinski stated that Child A's Grandmother "came in with an attitude" when she arrived. He asked Child A's Grandmother if Child A would be attending the following week as he would be in Child A's Father's custody. Child A's Grandmother replied that it was a 45-minute drive for her to get to the center. She used a cuss word as she spoke. Mr. Lipinski stated that he did not ask Child A's Grandmother for her ID. Child Care Staff Member 1 stated that when Child A's Grandmother arrived, it was the end of lunch time, and the children were being put down for nap. Child A was already asleep. He falls asleep quickly. Child Care Staff Member 1 was in the nap room helping children fall asleep when Child A's Grandmother called the center number. Child Care Staff Member 1 went to the office to let Mr. Lipinski know that Child A's Grandmother was there. Mr. Lipinksi and Child Care Staff Member 1 let Child A's Grandmother in. Mr. Lipinski asked to see her ID. Child A's Grandmother showed him her ID. Child A's Grandmother woke up Child A. Mr. Lipinski asked Child A's Grandmother if Child A would be at the center the following week and she replied "no" because the center is too far away. Child Care Staff Member 2 stated that she was in the classroom caring for the children on 06/16/2023, so she was not present when Child A's Grandmother was let into the building. She did not witness Child A's Grandmother being asked for her ID, 3 but she assumed that exchange occurred out in the hallway before Child A's Grandmother entered the classroom. Dana Lipinski, licensee designee, stated that the center was informed that Child A's Grandmother would be picking Child A up on 06/16/2023. Child A's Grandmother has never picked up before this date. Ms. Lipinski was not present when Child A was picked up. Child B's Mother stated that she has had other individuals pick up Child B from the child care center before, but she is not aware of whether the center checked the individual's ID. She always tells the center the name of the individual coming for Child B. Child C's Mother stated that no one besides Child A's Mother and Father have come to the center to pick up Child C. I made several attempts to interview Child A's Mother regarding the allegations but did not receive a return call. During the exit conference, I suggested to Ms. Lipinski that they have parents document in writing when alternative individuals will be picking children up form care. I also suggested that they make it a center policy that individual's IDs be checked to assure the correct person is picking up the child.

Child A's Grandmother stated that on 06/23/2023 she asked Child A if he goes outside and plays at the center, and he said no. Mr. Lipinski stated that the children in care go outside every day unless it is raining outside. 4 Ms. Lipinski stated that the children in care go outside every day. Sometimes the children go out twice a day. Child Care Staff Member 1 stated that the children all go outside every day. Typically, this occurs after snack time. They will send out a group of younger children and then switch and send out the older children. Child Care Staff Member 2 stated that the children in care go outside every day. Child B's Mother stated that she has witnessed the children in care outdoors occasionally when she has been at the center. She could not say for certain that the children in care go outside every day. Child C's Mother stated that she has never witnessed the children in care outdoors when she has been at the center. She has gotten activity alerts regarding physical fitness occurring, but she was not sure if that was indoors or outdoors. During the exit conference, I suggested to Ms. Lipinski that the center list outdoor play on Pro Care when it occurs so that parents are aware when their children play outdoors.

Child A's Grandmother stated that when she picked up Child A on 06/16/2023, lunch was in process. Child A was done eating. Child A told Child A's Grandmother that he was hungry. Child A's Grandmother added that Child A is not a picky eater. 5 Mr. Lipinski stated that the center follows all of the required food groups for the meals that they provide. They have a menu posted for parents to review. They are trying to get on the food program. Ms. Lipinski stated that the center follows the requirements for meals. Child A is a picky eater and needs a lot of redirection during meal time to get him to eat his food. Sometimes he is a good eater and other times he is not. After lunch time, Child A lays down for a nap. During the on-site inspection, I reviewed the menus posted in the center. They listed balanced meals that meet the requirements of the child and adult care food program. Child Care Staff Member 1 stated that the center provides balanced meals to the children in care. Child A is pickier about eating breakfast but does eat lunch pretty well. He has some texture issues, so that makes him not want to eat the food served sometimes. Child A also has a hard time sitting still for meals. He also really enjoys nap time so will often rush through lunch so he can go lie down. Child B's Mother stated that she has never checked the menus at the center to see the foods being provided. Pro Care indicates that the child ate and how much of the meal, but it does not specify what foods were eaten. Child B's Mother felt that Child B is provided with enough food at the center. Child C's Mother stated that she has looked at the menu posted at the center. Pro Care does not indicate what foods are eaten, only whether the child at the meal. Child C's Mother felt that Child C is fed an appropriate amount of food while at the center. During the exit conference, I suggested to Ms. Lipinski that the center include the food eaten when entering meals on Pro Care so that parents know what food their child has eaten.

Child A's Grandmother stated that on 06/16/2023 when she arrived at the center, Child A was in a large room with other napping children. There was no child care staff member visible in the classroom. Child Care Staff Member 1 stated that she left the room momentarily on 06/16/2023 to let Child A's Grandmother into the building and assist with Child A being picked up, but Child Care Staff Member 2 was able to supervise the children while she was out of the room. Child Care Staff Member 2 stated that on 06/16/2023, Child Care Staff Member 1 was in the nap room supervising the children while Child Care Staff Member 2 was in the lunch room with the remainder of the children. When Child A's Grandmother arrived, Child Care Staff Member 1 left the nap room to let her in. The nap and lunch room are attached to each other and connected by a large double door that remains open. Child Care Staff Member 2 was able to supervise the children in both areas while Child Care Staff Member 1 let Child A's Grandmother into the building. Mr. Lipinski stated that the children are not left unsupervised. Ms. Lipinski stated that there is always a staff member in the nap room. Children are not left unattended. Neither Child B's Mother nor Child C's Mother had witnessed children left unsupervised while at the center. Neither had any concerns regarding the care and supervision that the center requires.
