On 9/16/22, I made an on-site inspection with child care consultants, Tania Allard and Takiyah Jackson. I interviewed Amanda Hill, program director. Ms. Hill was outside in the outdoor play area with the preschool children on 9/13. The young and older toddlers were also outside in the area during this time. They had all decided to come into the building at the same time because it was almost lunch time. Ms. Hill and Ms. Floyd were in front of the line with the children lining up behind them. Ms. Hill took her preschool children into the building and the toddler room was still outside. The pastor's wife, Christina Hug, found Child A outside unsupervised and notified the toddler room. The staff went and retrieved the child from the outdoor play area. I interviewed Ms. Floyd. Ms. Floyd was a substitute in the toddler room that day. She does not usually work in that room. She and Ms. Geitman were outside with eight children. The younger toddlers and the preschool classroom were also outside during this time. It was time for lunch so all three groups of children were going inside the building. Ms. Floyd was at the beginning of the line. Ms. Hill had to give her the key to get into the building. She proceeded to go into the building with some of the children. They were taking the children's shoes off and then the pastor's wife was knocking on the window and told them there was a child outside. Kaylee Reimer, child care staff member, who was working with younger toddlers said is it Child A. It was Child A who was outside and Ms. Floyd was not sure how Ms. Reimer knew it was him. Ms. Floyd brought Child A into the building. She thought he may have been outside unsupervised for about 30-45 seconds. Ms. Hill provided me with children and staff attendance for the day of the incident 9/13/22. There were five preschool children and 12 toddlers with five child care staff. This is a total of 17 children outside in the outdoor play area. I observed the outdoor play area. The area is enclosed and located next to the building. The parking lot is adjacent to the building and outdoor play area. On 9/23/22, I interviewed Kaylee Reimer, child care staff member. Ms. Reimer was outside with her younger toddler group along with Samantha Smith, child care staff member. She was watching the younger toddlers carefully because they tend to put things in their mouths. The older toddlers and the preschool classroom were outside in the area also. They decided it was time to go in to get ready for lunch. She took two of the younger toddlers by their hand and assisted them inside. The pastor's wife knocked on the window while they were still getting the shoes off the children. She informed them that a child was outside on the outdoor play area unattended. She thought maybe Child A was outside for one to two minutes before they were notified. 3 I interviewed Diane Geitman. Ms. Geitman stated that the older toddlers were out in the outdoor play area first. The younger toddlers initially went for a walk then came in the outdoor play area. The preschool classroom came out to the outdoor play area after their classroom had been out there for a little bit. It was time to go in for lunch and all three groups decided to go in at the same time. Ms. Geitman had two of the children from the older toddlers that she was assigned and Ms. Floyd was assigned to Child A. Ms. Floyd usually does not work in the toddler room and was just filling in for another child care staff member who left early that day. They all went into the room and were taking off the children's shoes. There was a knock on the window. It was the pastor's wife informing them that there was a child outside unsupervised. Ms. Floyd then went outside and retrieved Child A. I interviewed Samantha Smith, child care staff member. Ms. Smith was with the younger toddlers in the outdoor play area. The older toddlers and the preschool children were also outside during this time. It was almost lunch time so they all w
On 9/13/22, child care staff members Erianna Floyd and Diane Geitman left Child A (male, 2 years) outside in the fenced in play area. They were unaware of it until a member of the church notified them that a child was outside unattended.
