On 08/16/2023, I talked to licensee designee Barbara Loper about the new incident report. Barbara Loper was not present at the center when the incident occurred, but she was present at the school. She assisted in getting Child F ice once she was informed of his injury. We discussed active supervision among child care staff members during outdoor play. On 08/16/2023, an incident report was received stating Child F (11-year-old, male) jumped off the top of the spiral slide. When Child F landed, he tripped and fell on his left wrist and right side. He told everyone to stay away from him. CCSM 1 took Child D inside the center where he was given ice and CCSM 8 contacted Child F’s Mother. Child F sustained fractures to both wrists. On 08/17/2023, I interviewed CCSM 1. On 08/16/2023, Child F jumped off the spiral slide and his foot caught on the railing when he jumped resulting in him falling on his side. She was by the slide when this happened. She had told him to get off the slide prior to him jumping, but he jumped down from the top of it. She reported that Child F has been jumping off elevated outdoor equipment recently. Child F has never jumped from the spiral slide at that height before. There have been a lot of children jumping off the slide and she feels that they should block it off. I asked which child care staff members were outside with her when the incident happened. She stated that she could not remember who was outside with her and she needs to check. She then stated that CCSM 3 was present during outdoor play. She did not know where CCSM 3 was when this incident happened. After Child F fell, she took him into the 14 center, and he was given ice. CCSM 8 contacted his mother. Child F sustained fractures to both of his wrists and she believes that he possibly broke one of his arms. Child F has not returned to the center. On 08/21/2023, I interviewed Child F’s Mother. On 08/16/2023, Child F jumped off a slide during outdoor play. He landed face first resulting in a fracture to both of his wrists. The center contacted her immediately after the incident. She picked Child F up and took him to the hospital. The center did call her back and ask whether she wanted the center to call for an ambulance due to the severity of Child F’s injuries. She told them no because she was close to arriving at the center. Child F did not sustain an injury to his neck, face, or arm. Child F has never been seriously injured at the center prior to this incident. She does not have any concerns regarding care and supervision at the center. The center staff had directed her son to get off the slide prior to him jumping off, but her “son is very defiant.” On 08/23/2023, I made an unannounced onsite inspection to the center I took several more pictures of the All Star Logger’s outdoor play area and I measured the height of the spiral slide. I climbed onto the elevated play structure with the attached spiral slide and measured the distance from the top of the metal railing to the ground. It was 11 feet high from the ground to the top of the metal railing. On 08/31/2023, I interviewed CCSM 4. She was not present at the center when Child F jumped off the slide. She has told Child F in the past not to jump off the spiral slide. Child F has had to sit by her on several occasions when he has attempted to jump off the slide. Other children have attempted to jump off the spiral slide in the past. On 08/31/2023, I interviewed CCSM 3. She was present inside the center when Child F jumped off the slide. She came outside to assist Child F when he was injured, and she brought him into the center. CCSM 1 was present outside with Child F when the incident happened. She does not know whether CCSM 1 was on her phone when the incident happened, but it is normal for CCSM 1 to be on her cell phone sitting on the bench or picnic table. She does not feel that children are consistently using the elevated outdoor play equipment improperly, but the center does see more improp

Barbara Loper did not ensure that appropriate care and supervision of children occurred at all times. On 08/16/2023, Child F jumped 11 feet off the top of the spiral slide. His foot became caught in the metal railing surrounding the top of the slide resulting in him falling face first and breaking both of his wrists. Interviews with staff and Child E indicated that proper use of elevated play equipment by children was an ongoing issue during outdoor play.

Barbara Loper did not ensure that all staff and volunteers provided appropriate care and supervision of children at all times. On 07/18/2023, Child N, Child O, Child P, and Child Q picked cone cap mushrooms during outdoor play and placed them in their mouth. Child O was the only child to ingest them. No child care staff members witnessed the children placing mushrooms in their mouth during outdoor play.
