On 06/07/2023, 1 interviewed the complainant. On 06/07/2023, I obtained five photos from the Little Duckling's Facebook page showing infant children sleeping on unapproved sleeping equipment. Photo 1 is dated 05/12/2023, and it shows Child A (7-month-old, male) and Child B (9-month- old, male) sleeping on a blanket. Child A is lying on his back asleep while Child B is face down lying on his stomach in yellow mustard. Photo 2 is dated 04/28/2023, and it shows Child A sleeping outside on a pink mat. Photo 3 is dated 05/12/2023, and it shows MHM 2 (9-month-old, female) sleeping face down outside on a blue mat. Photo 4 and Photo 5 are dated 04/28/2023, and they show Child A in the back ground in an infant swing. The photo is not clear enough to show whether Child A's eyes are open or closed. On 06/08/2023, an unannounced onsite inspection was conducted at Ms. Keagan Rainey's child care home. Ms. Rainey admitted to allowing infants to sleep in a playpen and beach hut during outdoor play. Ms. Rainey thought she was allowed to let them sleep that way when outside for outdoor play. She denied allowing infants to 3 sleep in the infant swing and she was adamant that she moves them right away when they fall asleep in the swing. She denied allowing infants to sleep on their stomachs and she always places infants on their back to sleep. She discussed Child B (8-month-old, male) rolling front to back on his own so, he will occasionally sleep on his stomach. I took pictures of the infant swing located in the living room and the three pictures of the outdoor play area, and pictures of the child information cards. I showed her the three pictures I had of children sleeping outside and the two identical pictures of an infant in an infant swing. She identified the children in the pictures, and she stated that the infant in the infant swing was awake. On 06/12/2023 and 06/26/2023, I Interviewed Child A and H's Mother, Child B's Mother, Child C's Mother, Child D's Mother, and Child F and G's Mother. They denied having concerns with care and supervision at the child care family home. Child A and H's Mother stated that she has some photos of Child A sleeping on a mat. She agreed to send me the photos. Child B's Mother has witnessed Child B sleeping outside in unapproved sleeping equipment. Child C's Mother, Child D's Mother, and Child F and G's Mother denied having concerns with infant safe sleep. On 06/12/2023, I received four photos from Child A and H's Mother showing Child A sleeping in unapproved sleeping equipment. Child A and H's Mother received these photos from Keagan Rainey. The first photo was taken on 04/20/2023 at 3:52 PM by Keegan Rainey, and it shows Child A sleeping on gray carpet. The second photo was taken on 03/23/2023 at 2:35 PM by Keegan Rainey, and it shows Child A sleeping in a red sled. The third photo was taken on 04/15/2023 at 3:08 PM by Keegan Rainey, and it shows Child A sleeping on a pink mat. The fourth photo was taken on 05/10/2023 at 4:29 PM by Keegan Rainey, it shows Child A sleeping on a pink mat.

On 06/07/2023, I interviewed the complainant. On 06/08/2023, an unannounced onsite inspection was conducted at Ms. Rainey's child care home. Ms. Rainey denied allowing any children to remain in soiled diapers at her child care. She checks children's diapers every hour and normally changes all children's diapers every two hours even if they are soiled or not. She has never had any major issues with diaper rash at her child care. There has only been one child, Child D, who had a minor diaper rash on one occasion. Ms. Rainey has a diaper changing table in the bathroom and sometimes she uses a diapering pad on the floor in the bathroom. I observed the changing table and pad along with three bottles used for the three-step method. Ms. Rainey reported that she three-steps the diapering pad and surface around it after every diaper because it is disgusting. I informed her that one of the allegations was that she does not three-step after every diaper which she denied. She showed me that she uses Steramine to sanitize and disinfect for the third step. I observed a diaper change while onsite and Ms. Rainey properly cleaned, sanitized, and disinfected the diapering station. I took pictures of the diapering station in the bathroom, and I took pictures of the three bottles used to clean, sanitize, and disinfect the diapering station. On 06/12/2023 and 06/26/2023, I Interviewed Child A and H's Mother, Child B's Mother, Child C's Mother, Child D's Mother, and Child F and G's Mother. Child A and H's Mother disclosed that Child A and Child H both had diaper rash while attending there. She clarified that Child H is prone to diaper rash though. She could not recall picking up Child A and Child H with soiled diapers. Child C's Mother reported that there were a couple of times she picked up Child C with a soiled diaper A which she feels is normal since she would pick up sometimes right after naptime. Child B's Mother, Child D's Mother, and Child F and G's Mother denied having any concerns with diapering and diaper rash at the child care.
