On February 11, 2022, program director Sarah Kosciuk called and told me that a five-year-old boy (Child A) was left unsupervised in the school library for about 10 minutes on February 9, 2022, she and sent an incident report. Child A attends the ABC afterschool program. It is mainly held in the school library. Patti Sterner was the sole staff member supervising children until 4:00 p.m. When I interviewed her on February 24, 2022, Ms. Sterner said that Child A and the other children were working with Perler Beads that day. Child A said he wanted to give his project to his kindergarten teacher, Ms. Wilson, and Ms. Sterner allowed him to leave the library for this purpose. Meanwhile, she began helping the other children put on their snow pants and other winter gear. She did not realize that Child A had not 2 returned. As the children left the classroom, she did a head count. She had signed in 11 children on the attendance record and that matched the number that she had counted. She later realized her count was off and that there were actually12 children in attendance, including Child A. She did not have an accurate attendance record. Ms. Kosciuk asked Ms. Wilson about the incident. Ms. Wilson responded in an email that Ms. Kosciuk shared with me. In it, Ms. Wilson confirmed that Child A brought the beads to her classroom with Ms. Sterner's permission. He wanted to chat, but she told him he needed to return to the library. She watched him walk down the hall and into the library. She later learned that his classmates had gone outside without him. Ms. Kosciuk said she viewed camera footage of Child A's movement that evening. She saw the ABC children heading outdoors at about 3:50 p.m. Children use the third-grade classroom to enter the playground. Ms. Kosciuk could see Child A enter the classroom afterwards. It appeared that he had confirmed that his classmates were outside and then went back to the library. Other staff members were present in the building during this time and were seen on camera in the hallways as Child A sat in the library. The second staff member, Kara Stanton arrived at 4:00 p.m. and found Child A alone. When interviewed, she told me that she had asked Child A where the others were. He told her that they were outside and that he did not want to go outside. She spoke to him for a few minutes and explained that he needed to join the others outdoors. As they were talking, Ms. Sterner and the children came back indoors. Ms. Kosciuk said she called Child A's father about the incident the following day and that Child A remains enrolled. Ms. Sterner indicated that she had spoken to Child A's father about the incident the previous evening at pickup. Child A's father responded to my phone call with a voicemail message on March 3, 2022. He said he has no concerns about the incident and that staff do a great job. .................... .................................. ................

On April 28, 2021 and consistent with R 400.8158(2)(a), program director Sarah Kosciuk contacted me when she learned Child A was briefly unsupervised on the playground the prior day. I made an unannounced inspection at the center on May 4, 2021 and interviewed Child Care Staff Member (CCSM 1). CCSM 1 was supervising Child A and seven other children, age 3 to 4 years, on the playground on April 27, 2021 at approximately 4:00 p.m. The classroom door leads directly to the fully fenced playground. When CCSM 1 and the children reentered the building, she miscounted and believed everyone had returned indoors. Within seconds after the door closed, another staff member who had just gone outside with her class spotted Child A and brought him to the door. CCSM 1 said nothing like that has ever happened. When she spoke to Child A, he told her he wanted to stay outdoors longer. She expressed remorse and said she takes full responsibility for the issue. She immediately notified Ms. Kosciuk of the incident. 2 Ms. Kosciuk reported that she contacted Child A's mother about the incident at 4:40 p.m. The child remains enrolled. I spoke to Child A's mother on May 4, 2021. She indicated that her son is fine and said she has no concerns.
