On 12/08/2021, I conducted an unannounced onsite inspection at DeVries Daycare where I met with program director Ms. Bonnie Best-Pero and Ms. Katerina Britton. After being informed of the allegations, Ms. Best-Pero informed me that when new child care staff members (CCSMs) are hired, they are provided with an orientation by Ms. Best-Pero or Ms. Britton. The orientation provides new CCSMs with a tour of the child care center, an overview of the licensing rules, and DeVries 2 policies such as discipline, diapering, emergency plans, etc. CCSMs are then paired with an experienced CCSM to begin working with children. Ms. Britton added that she completes certain trainings such as safe sleep and blood borne pathogen trainings with all new employees during the orientation and prior to CCSMs caring for children. On 3/3/2022, I met with Ms. Britton and reviewed five CCSM files and Ms. Britton's MiRegistry account for required trainings. All CCSMs files reviewed contained all the required trainings except for Ms. Megan Tollenaar. I noted that Ms. Tollenaar had not yet completed MiRegistry Health and Safety Part 1 or Part 2. Ms. Britton confirmed that Ms. Tollenaar was hired as a CCSM at DeVries on 9/8/2021 and she began working directly with children on 9/20/2022.

A review of the DeVries Daycare licensing file was conducted and a summary of room capacities for the yellow and red rooms as established by the Original Licensing Study Report (dated 7/8/2008) and the Addendum to the Original 3 Licensing Study Report (dated 10/16/2008) are summarized in the table below. It should be noted that no other addendums were found. Ms. Britton confirmed that no other modifications have been requested. Ms. Britton also confirmed that the only rooms used for infants are the yellow and red rooms. On 12/8/2021, there were three children in the yellow room being cared for by one CCSM. In the red room, three children were being cared for by two CCSMs. On 3/3/2022, there were four children in the yellow room being cared for by one CCSM. In the red room, four children were being cared for by two CCSMs. Ms. Britton also provided me with printouts from attendance app (Bright-wheel) used for child attendance for the yellow and red rooms for the period of 11/1/2021 — 11/19/2021 (15 days of operation). These records show that during this time frame, the yellow room did not exceed 4 children and the red room did not exceed 6 children at any given time.

Multi-purpose tables must be thoroughly washed, rinsed, and sanitized before and after they are used for meals or snacks.
During my 12/8/2022 unannounced onsite, I spoke with Ms. Britton and Ms. Best-Pero regarding the process used to clean/sanitize equipment at the center such as countertops, tabletops, toys, and any food service item such as multiuse cups. Both described a three-step process that included washing, rinsing, and sanitizing items prior to use by children. While walking thru the center, I noted that all table and countertop surfaces appeared to be clean. I was also able to observe several cups air drying on a countertop. Ms. Britton explained that following the three-step process of cleaning and sanitizing, items are left to air dry. I returned to the center on 3/3/2022 and again observed tabletops and countertops to be clean. I also noted directions for the three-step process to be posted by the sink in which dishes are cleaned.
