Interviews conducted on 5/58/25 with Renee Pertile, LC, CCSM 1, and CCSM 2, indicated that CCSM 1 did not know that Child A was injured when he bumped his head. There were no prior concerns about CCSM 1's interactions or about the supervision she was providing to the children.

Interviews with Lead Caregiver and Child Care Staff Members on 5/15/25 indicated that staff were interacting with the children during time of the incident. Lead Caregiver saw the incident happen and responded to the child immediately. Review of children and staff attendance records on 05/15/25 indicated the program was in compliance with ratio requirements during the time of the incident. Phone interviews with Child B's Mother, Child C's Father, and Child D's Father indicated they had no concerns regarding the care and supervision children were receiving in the classroom.
