    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times. I

    I received an incident report via email from licensee designee and program director, Rebeka Hayes, on 04/14/2023. The report indicated Child A was left sleeping inside unsupervised for approximately 20 minutes. On 04/17/2023, I conducted an unannounced inspection at the center. I interviewed Ms. Hayes. The incident occurred on 04/13/2023. She was at the center but helping in the 3-year-old classroom because they had a child care staff member (CCSM) absent. CCSM 2 immediately notified her of the incident. Ms. Hayes said that CCSM 1 and CCSM 2 were getting ready to go outside. They counted the children when leaving the classroom. Usually there is a CCSM at the front and back of the line. When they were leaving the classroom, there were some behavioral issues in the middle of the line, so CCSM 1 moved to the middle of the line. Child A was at the end of the line. Child A stayed behind in the classroom. 2 Ms. Hayes advised the class went outside and stayed out approximately 20 minutes. When they were getting ready to come back inside, they counted kids and noticed they were missing one child. They realized it was Child A. CCSM 2 sent a message through the application "Group Me" the center uses that can send a text to the entire school. Everyone began looking for Child A. CCSM 2 went immediately to the classroom and found Child A sleeping under a bean bag. Ms. Hayes, CCSM 1, and CCSM 2 called Child A's Mother on speaker phone. They advised Child A's Mother of the incident. Child A's Mother did not seem concerned and explained that she was working late, and Child A isn't sleeping well so he is tired. Ms. Hayes, CCSM 1, and CCSM 2 also talked to Child A and encouraged him not to leave the CCSMs and if he is tired, talk to them. During the onsite inspection on 04/17/2023, I interviewed CCSM 1. She said that was a rough day. She and CCSM 2 were getting the children ready for lunch. There was a child with a nosebleed that they had to handle so they were late getting their lunches. They lined the children up. CCSM 1 was standing near the middle of the line because there is a child with behavioral issues, and she usually walks next to that child. CCSM 1 and CCSM 2 took the children to the lunchroom, got the food, and then stopped at the classroom on the way outside so CCSM 2 could get her hat and jacket. CCSM 1 stayed with the children at the door. They all went outside to eat their lunches. After eating lunch, they lined the children up to come back inside and were missing a child when they counted. There were only 14 children when there should have been 15 children. They determined it was Child A missing. The CCSMs began looking for Child A near all the play equipment and on the hill where children lay. They did not see him anywhere. There was another teacher outside with school-age children and they confirmed with her that she had not seen Child A. The took all of the children inside. CCSM 2 went into the classroom and found Child A asleep under a bean bag. They asked Child A what happened. He said he did not go to lunch. He left line and went back inside because he was upset that he did not get a toy based on his behavior that day. He planned to go back inside and steal a toy. CCSM 1 said Child A was unsupervised for approximately 15 minutes. Child A was in the classroom when CCSM 2 went in for her hat and jacket, but she did not see him because he was under the bean bag. When the CCSMs came back inside, CCSM 2 noticed the bean bag was not where it normally was and then one of the bean bags moved. They found Child A asleep under the bean bag. CCSM 1 denied Child A usually has any behavioral issues. They did talk with Child A's Mother, and she said he was tired. 3 While onsite on 04/17/2023, I requested child information cards. I was also provided and reviewed two incident reports written by CCSM 1 and CCSM 2. The report from CCSM 1 states "When going to lunch a kid left out of the line and went back to the classroom unattended." "We c
