On 10/22/2020 I had an email correspondence with the Reporting Person. The Reporting Person informed me that an employee at their business was able to identify a woman on the company's surveillance system as Ms. Lynn Kehoe. The Reporting Person then forwarded me a copy of the video surveillance footage, recorded on 10/21/20 at 1:28 PM, showing Ms. Kehoe with at least 14 children without the presence of a child care staff member. The Reporting Person also noted that in the video clip he would be emailing, there was also a moment where Ms. Kehoe left the stroller that she was pushing unattended to pursue a toddler that had wandered off. However, upon receiving and reviewing the 19 second surveillance footage, I did not observe Ms. Kehoe leaving the child she was pushing in a stroller to pursue any other children. On 10/22/2020 I made an onsite inspection at Ms. Lynn Kehoe's child care facility. Upon arrival, I observed 10 children present in the home and Ms. Kehoe was the only adult present. Of the 10 children present in the home, 6 of the children were child care children; while the remaining four were related children. However, of the related children, three were under the age of 6 and were counted in the ratio. Ms. Kehoe informed me that the child care staff member, Ms. Danielle Kehoe, was working but left the home approximately 10 minutes prior to my arrival. I then asked Ms. Kehoe about the previous day, 10/21/2020 and how many children she had present in the child care facility. Ms. Kehoe responded that she only had six children present. I then inquired into whether Ms. Kehoe was present on the White Pine Trail on 10/21/2020 with her child care children, which she acknowledged. I informed Ms. Kehoe that I had been provided a video of her walking along the White 3 Pine Trail with 14 children without any other child care staff member present. Upon disclosing this information to Ms. Kehoe, I again asked if she had 14 children, including child care children on the White Pine Trail on the previous day. She responded that she "may have" had that many children present. I reviewed Ms. Kehoe's child information cards and it was discovered that there were only four children enrolled that are related, which would mean Ms. Kehoe had at least 10 child care children on 10/21/2020. It should be noted that prior to the conclusion of the visit, I informed Ms. Kehoe that she needed to contact the child care staff member, Ms. Danielle Kehoe, and instruct her that she needed to return to the child care facility to bring the facility within ratio. On 11/24/2020 I left a voicemail message for the child care staff member, Ms. Danielle Kehoe. On 11/24/2020 I spoke to Child A's Mother. Child A has been attending the child care facility for approximately three years. Child A's Mother has been "very satisfied" with the level of care Ms. Kehoe has provided. Child A's Mother denied ever being concerned regarding the health and safety of Child A. She's never paid attention to the amount of children that have been present at the facility during pickups, but noted that during drop-offs, Child A was typically one of the first children to arrive at the facility. Despite being presented with the allegations, Child A's Mother reasserted that she had no concerns regarding Ms. Kehoe's ability to provide care for Child A, and that he's never been injured or harmed under Ms. Kehoe's supervision. On 11/24/2020 I left a voicemail message for the child care staff member, Ms. Danielle Kehoe, and Child B's Mother, Child C's Mother, Child D's Mother, Child E's Mother, and Child F's Mother. On 11/25/2020 I spoke to Child B's Mother. Child B's Mother stated that Child B has been attending the child care facility for approximately two years. She denied having any concerns regarding Ms. Kehoe's child care. Child B's Mother advised that she was the primary parent that did drop-offs and pickups. She was unaware of the appropriate capacity and ratios but stated that Ms. Keho

On 10/22/2020 during the onsite inspection, Ms. Kehoe was observed to have a ratio of 10 children to 1 staff; however, 3 of the related children present were under the age of 6, and thus, counted against the ratio.

Upon review of the video surveillance footage obtained by the Reporting Person, Ms. Kehoe was never observed leaving the children unaccompanied to pursue a child wandering off. Additionally, during my conversations with the child care parents, there were no concerns noted suggesting there were issues regarding the level and quality of care provided by Ms. Kehoe.
