On 3/23/22, I interviewed Child A's Mother. Child A has an acute fracture of his left collarbone. He will be in a sling for three to four weeks. When she dropped off Child A that morning, he was a little more emotional than usual and crying when she dropped him at the center. Child A's Mother handed Child A to Wendy Witzke-Bixman, program director. Ms. Witzke-Bixman took Child A and then Child A's mother left the center. When she picked up Child A from child care she was approached and given an accident report and was told that Child A was extra sensitive that day. She observed Child A sitting off playing by himself. He looked like he was favoring his right arm. Child A pointed to his arm when she greeted him. She went to pick him up underneath his arms and he cried. She took him to get looked at and that is when he was diagnosed with a fractured collarbone. Child A is not very verbal and was not able to tell her what happened and she believes it was an accident. She does not think he was hurt by a child care staff. She does feel that the child care staff should have figured out that something was wrong with him. They are with him three days a week. Child A's cousin is in the same classroom with him and they usually are running all around the room. This day Child A was playing by himself which is unusual. On 3/24/22, I went to the center and interviewed Ms. Witzke-Bixman, program director. She took Child A that day from his mother. He was having a hard time and was crying. Child A's Mother stated that his dad was home today after being gone for awhile and Child A wanted to stay with him. Ms. Witzke-Bixman took Child A to his classroom. Ms. Witzke-Bixman did go look at Child A after she was informed that he was crying. He was sitting at the table and he appeared to be fine at that time. I interviewed Jane Lockhart, lead caregiver and Child A's primary caregiver. There were 12 toddlers and three child care staff that day. Ms. Lockhart explained that they had just finished with art and she was cleaning up. Child A was sitting on the floor crying so she knew something occurred but did not see what. Child A was acting a little off all day but Child A was upset upon arrival to care. His dad was gone for three weeks and now he is home and he did not want to leave his dad this morning. He did fall asleep in the morning on a climber. Normally he is outgoing, rambunctious, and very active. Child A does have his moments where he is upset and over sensitive about things. She thought he was over tired that day. Sarah Wahr, child care staff member, wrote the accident report and Alissa Snyder stated that she saw the one child collide into Child A. I interviewed Ms. Snyder. She observed Ms. Wahr on the ground reading to some children sitting around her. She was cleaning tables and observed Child A and 3 another child collide and land in the foam pit. Child A did land on his shoulder in the foam pit. Ms. Snyder said that it was chaotic during that time. Child A was not acting out of the ordinary that day from what she witnessed. He would have times during the day where he was sensitive. Ms. Wahr came and asked her what happened so she could complete the accident report. On 3/29/22, I interviewed Ms. Wahr. She was sitting by the foam pit reading a book to a group of children. Child A was sitting near her but behind her. Ms. Lockhart was cleaning up from breakfast or an art project. A child ran by her and the next thing she knew Child A was laying flat on his back crying. He popped up right away. She picked him up and he cried. She thought he may have hurt his ribs. She comforted him and asked him what hurt he pointed to shoulder and then to his stomach. She got him a cool cloth for his stomach. She attempted to discuss Child A's accident with Ms. Lockhart. Ms. Lockhart would not listen to her and when Child A was crying told him that he was fine Ms. Lockhart did not answer her about completing an accident report. Ms. Wahr brought it to Ms. Witz
