    On 5/12/23, I conducted an on-site inspection. I interviewed Clint Shope, child care staff member, who was working at the time of the accident. Mr. Shope did not observe the accident. Rhonda Rexford, child care staff member, observed the accident and then told him about it. Ms. Rexford said that Child A was on the monkey bars. He wanted to show Ms. Rexford how fast he could go across. While attempting this he fell onto his stomach with his arms under him. Mr. Shope called Child A's Father while Ms. Rexford went to get some ice for the injury. Child A's Stepmother arrived to pick Child A up from child care while he was on the phone taking to Child A's Father. On 5/12/23, I interviewed Child A's Mother. She had no concerns regarding the accident. She does not know if it could have been prevented due to the way he fell onto his hands. They are going to see an orthopedic surgeon for a follow up and she will contact me if she has any concerns. I observed the playground area. Wood chips are used for surfacing. The wood chips looked like they had been placed down this year sometime. There looked and felt as though there was plenty of surfacing as you walked under the equipment. On 5/15/23, I interviewed Ms. Rexford. Ms. Rexford was standing right by the monkey bars when Child A fell. Child A missed a bar as he was going across and when he fell, he twisted himself so that he landed on his stomach with his hands under him. She informed Ms. Shope what had happened and ran into the school to get some ice. Mr. Shope called Child A' s Father. When she returned with the ice Child A's Stepmother was there. I interviewed Wendi Witzke-Bixman, designee. She thought that an inspection was completed on that playground because it was just installed in the summer of 2022. She requested that one be completed even though it was not required for the school age program. The playground was newly installed in the summer of 2022. There were eighty cubic yards of surfacing placed down on 10/3/22. On 5/23/23, I spoke with Tim Sheffer, certified playground inspector. He completed an inspection of the playground, on 5/23/23, and did not have any concerns regarding surfacing or equipment. 3
