The child care staff members are ensuring that the premises are maintained in a safe condition by ensuring that the accumulation of water, ice and snow are removed from stairs, walkways, ramps and landings to ensure there is a nonslip surfacing, as required. During the onsite inspection, I observed shovels and salt used by child care staff members used to shovel and salt the stairs, walkways and landings leading up to the center to ensure safety.

On 06/16/2023 I initiated the investigation with a telephone interview of Child A's father. Child A's father said it seems something is always happening at child care that they do not approve of. This happens when he or his wife pick up Child A from care. About a week ago Child A was being picked up by his mother from child care. He began to choke, and Child A's mother had to help Child A herself while staff did not do anything. This makes him mad. Child A is not returning to care at this center. I attempted to interview Child A's mother by telephone on 06/25/2023 and 06/27/2023. I have not been able to interview Child A's mother to date. On 07/17/2023 I conducted an onsite inspection at the child care center. I observed 12 preschool age children with one child care staff member napping on mats or cots. I observed eight toddlers napping with two child care staff members to supervise. I asked the program director for staff time sheets and attendance records for the weeks of 06/11/2023 through 06/17/2023 and 06/18/2023 — 06/24/2023. I requested Child A (1 year, 11 months/male)'s child information card. I observed in the entryway of the child care center a short counter with a large, bold printed sign taped down at the end of said counter and obvious to anyone that states "NO PARENTS BEYOND THS POINT." This sign is posted to prevent children from seeing their parents at pick up to eliminate children getting over excited. The program director knew immediately why I was there and provided Child A's child information card and the requested documents. Staff time cards were reviewed and I verified that the program director and a child care staff member (CCSM) 1 were working in Child A's room at the time that Child A started choking on his snack. Attendance records confirm that Child A was in child care from 05/09/2023 until 06/19/2023 when his parents pulled him from care. Child A's mother signed a child in care statement indicating that food would be provided by the center. The family was newer to the facility, they had been informed of the rule about going past the end of the counter before. The program director explained that Child A's mother had come to pick Child A up earlier than usual and Child A was seating at the table having a snack. Child A's mother barged into the child care room which she is not allowed to do, going past the point parents are not supposed to go beyond. When Child A saw his mother, he got excited and inhaled sharply, choking on his snack. CCSM 1 did not have time to react because Child A's mother had grabbed him from the chair and started patting him on the back. The program director observed this happening in a matter of moments. I interviewed CCSM 1 alone at the entrance of the child care center. CCSM 1 recalls that" Child A was seated at the red table eating his snack when his mom came in and he saw her and choked himself out." Child A's mother barged in and grabbed him and CCSM 1 did not intervene at that time because Child A was being held by his mother. 3 I verified with the program director that child care staff are trained in first aid and know what to do if a child does choke on something. On 08/01/2023 I interviewed licensee designee Davana Harrison regarding Child A. She said that Child A's parents were always unpleasant to staff and that they returned Child A to care the day after he choked before pulling him from the center a short time after that.

This happens when he or his wife pick up Child A from care. About a week ago when he picked up Child A he was sleeping on a cot on the floor and when he woke Child A up, he had food and saliva running out of his mouth and this makes him mad. Child A is not returning to care at this center. I attempted to interview Child A's mother by telephone on 06/25/2023 and 06/27/2023. I have not been able to interview Child A's mother to date. On 07/17/2023 I observed several children napping at the center under appropriate supervision. No infants or toddlers were observed to have bottles, beverages containers, or food in their sleeping equipment. I interviewed program director Tricia Hackett and CCSM 1 who both say they do not put children down to nap with food in their mouth. 4
