I interviewed four program staff members and the Licensee Designee who all confirmed that parents of infants and toddlers are provided with a written daily record that includes changes in the child's usual behaviors. I observed daily sheets being completed. A parent confirmed receipt of the daily care sheet on a daily basis.

Evidence shows that while the center has a written plan for how and when a parent is to be notified if program staff or volunteers observe an incident with a child the center failed to implement that plan when Child A appeared to two program staff members that his body was rigid and he had stopped breathing, for however brief a time. Center policy explicitly references calling parents immediately and calling 911 if a seizure is suspected. Based on the parent's report and program staff interviews Child A's Mother was not contacted immediately and was only informed after arriving at the center to pick Child A up for the day.

There is insufficient evidence to support a finding of inappropriate care and supervision. I observed children being fed a snack, having their faces cleaned and their hands washed. I observed program staff to wipe children's noses and wash their hands. I spoke to parents of other children participating in child care in this room and none had concerns about the care their children receive or the condition the children are in when they are picked up.

Child A's child information card did not contain information about the need for diaper rash cream. I observed children being diapered and the diaper changes being logged on daily care sheets that were then provided to parents. There is insufficient evidence to conclude that Child A's diapers were not checked frequently and changed when soiled or wet.

There is a written agreement on file at the center regarding Child A's Mother's agreement to provide milk and food. When Child A did not have food provided the center did provide food for Child A. All parents interviewed confirmed receiving daily care sheets that specify when and what their child ate on any given day.

At the time of the inspection, no odor of sewage was observed within the building. Interviews were conducted with CCSM 1, CCSM 2, CCSM 3, CCSM 5, and CCSM 6 who reported the hall by the gym and kitchen which leads to the infant room smells of sewage around every three-four weeks and the odor is present for one-two days. The PD and LD verified there has been a smell of sewage on an infrequent basis within the building. Child B's Mother stated she has observed the smell of sewage within the building.
