    On 9/14/21, Ms. Kurchak notified me that Child A (male, 34 months) hurt his arm on the playground. The ambulance was contacted and he was transported to the hospital. Child A's Mother was present and accompanied Child A to the hospital. There were two staff and one volunteer present with fifteen children. requested the date of the last time that the center had replenished the surfacing on the playground. Ms. Kurchak submitted the invoice for surfacing that was dated 9/04/2019. Ms. Kurchak stated that they had wood chips in the parking lot and they were waiting for maintenance to disperse them on the playground. It should be done tomorrow. On 9/17/21, I interviewed Child A's Mother. She was at the center to pick Child A up from preschool when she was contacted by Ms. Kurchak. She informed her that Child A was hurt. Child A was so upset because he was in so much pain, she was 2 worried about comforting him and did not find out what exactly happened. They told her that he hurt himself on the playground. Child A is not very verbal and just told her that he fell off the stairs. She is guessing it was the stairs to the big piece of equipment. Child A had surgery on 9/14/21 and is now home recovering. The doctor did not have any concerns with the how the injury occurred. I went to the center and interviewed Ms. Kurchak, program director, and Melissa Courier, child care staff member. They took me to the playground and I observed the new surfacing had been placed on it. Ms. Courier explained that she was talking to Child A on the one side of the ship playground structure. She took a bouncy toy that he was trying to ride from Child A and set it aside. Child A started walking on the back side of the ship towards the fence. At this time volunteer, requested her assistance with a child that was disrobing themselves. Ms. Courier went towards the building and assisted the child with putting on his pants. Child A took off running the opposite way around the ship. A few seconds later Child A came walking to her crying holding his arm. Child A was extremely distressed and was in a lot of pain. He did not verbalize what had happened and none of the staff or volunteer saw what had occurred to Child A. Ms. Courier stated that it could not have been more than a minute from when she left Child A to assist with the child until the time that Child A came to her injured. Ms. Kurchak was in the doorway of the building because she was supervising a child that had to go into the building to use the restroom. She did not observe what happened to Child A. There is a camera that points to the playground area but when they asked to review the camera footage, they were told the camera was not in working order. I took pictures of the playground and they are included in the file. The outdoor play area is totally enclosed with a chain link fence. It consists of a big ship wooden play structure with a ramp, stairs, and slide. A swing set with one swing and a sandbox. The rest is open grassy area. The playground inspection was completed on 10/06/08 and all equipment was approved for use. On 9/29/21, I interviewed volunteer, regarding the accident.. arrived around ten o'clock the morning that Child A was injured. They had story time and then went to play outside. She was standing on the sidewalk close to the building and called for assistance because a little boy was taking off his clothes. Ms. Courier came to assist with the boy. A few seconds later Child A came from the other side of the ship by the front of the playground holding his arm and crying. She did not see what happened to Child A. He did not say what happened either as he was very upset and in a lot of pain. 3 ..................... .............
