    call from On 05/25/2023 l conducted an unannounced onsite inspection at . I met with the licensee/program director, Peggy Condon-Watson and an interview was conducted. On 05123/2023, Child A (female, arrived at 8:05 am and was signed out at 2:00 pm. Child A was not present 3 at the center on 05/22/2023 which was unusual as Child A never misses child care. The center uses video cameras in each classroom and hallway. She and watched the entire video surveillance from the date in question and at no time were there any accidents, injuries or incidents involving Child A. There was a 45 minute period where Child A was outside on the playground with staff that is not on video surveillance however no one reported there were any accidents, injuries or incidents involving Child A while outside. During outdoor play Child A was in a stroller or in the infant swing. On 05/23/2023, Child A was being cared for by who is the infant lead caregiver „' is not present a e center ue o being off today. Child A's mother called on 05/2312023 at 3:25 pm and reported she was taking Child A to the as there was something . Child A's mother has never reported to the center what the injuries were or where Child A was taken for medical treatment. On 05/24/2023, came to the center and notified them of the investigation and the Injuries. is also performing an investigation and will e at the center on 05/26/2023 to interview and download a copy of the video surveillance from 05/23/2023. hild A has been attending the child care center since age and there has never been any accidents or injuries. Child A has not returned to the center since the accident however Child A's sibling returned the next day. I then met with the and an interview was conducted. On 05/23/2023, Child A was picked upWa : m and she assisted in the pick up from the classroom when Child A's mother arrived. Child A was wrapped in a blanket by the lead caregiver, ttooookk Child A to the mother outside. Child A's mother placed i in a car seat right away. Child A was fussy and cried a little bit. Child A's mother said it was unusual for Child A to be crying when picked up and then she left. Child A did not have any visible injuries on 05/23/2023. I then met with child care staff member (CCSM) and an interview was conducted. ~~'Menworks in the pre-toddler room. On 05/23/2023, M was outside hild A was outside from approximately 10:30 -- 0 am. I did swing in the infant swing and was also walked around the playground in the infant stroller. Child A appeared to be fine and did not appear fussy or crying 4 white outside. Child A did not sustain any accidents, injuries, or incidents while outside. On 05/2512023, 1 obtained a copy of Child A's child information card and copies of her attendance records during the onsite inspection. On 05/25/2023, I attempted to contact Child A's mother by telephone and there was no answer. I was unable to leave a voice mail message as the mailbox was full. On 05/31/2023, 1 received an email from On 05/31/2023, 1 contacte~ by telephone and an interview was conducted. On 05/24/2023, watc a the video surveillance for the date in question and did not observe any notable injury or accident suggesting Child A was injured at the center. observed Child A moving her there was no indication of injury. CCSMs noted that Child A had been fussier than normal however there was no clear indication of any injury or reason why Child A was fussier than normal. did ask that licensing refrain from interviewing • until concludes his investigation.     On 4/17/23, I interviewed Child A's Father. On 4/14/23, Ms. Condon-Watson requested that Child A be picked up from the center because she was no longer going to provide care for him due to their complaints of Child A not being changed regularly. Ms. Condon-Watson pushed Child A's Mother and told her to get off the property when she picked Child A up from the center. 3 On 4/17/23. I went and conducted an on-site inspection. I interviewed Richard Watson, child care staff member and Ms. Condon-Watson's assistant. Mr. Watson explained that on Friday, 4/14/23, Child A's Mother dropped Child A off at the center and was very upset. This was the fifth time that Child A would have attended the center. He did not have contact with the mother previously. Melissa Fante, program director, was not working that day and Ms. Condon-Watson had not arrived at the center yet. Karen Rousse, administrative assistant/child care staff member, went to the door to let Child A's Mother into the building as she was dropping of Child A. Ms. Rousse came to him and told him that Child A's Mother wanted to talk to someone about her concerns regarding Child A's diapering. Mr. Watson went to talk with Child A's Mother. Mr. Watson had never talked with her before and had no idea that there were concerns previously regarding diapering. When he spoke with her, she was very upset and loud. She told him that she did not bring an extra set of clothes for Child A. She wanted to make sure that they checked on him more frequently, so he would not pee through his diaper. He explained to Child A's Mother that they change and check diapers and do toileting routinely. If they notice a child is soiled prior to these times the child care staff would change them. He asked if she would like to talk to the child care staff and she said that she had already talked to them previously. Child A's mother mentioned during the conversation that she should take Child A out of the child care if this could not be corrected. Mr. Watson informed Child A's Mother that he would inform Ms. Condon- Watson and Ms. Fante of her concerns. When Ms. Condon-Watson arrived at the center that day he informed her of his conversation with Child A's Mother. Ms. Condon-Watson called Child A's Mother and during the conversation Child A's Mother would not calm down and discuss the situation with her. Mr. Watson did not know exactly what was said during the conversation. He was told that Ms. Condon-Watson requested that Child A be picked up from the center. When Child A's Mother came to pick up Child A Mr. Watson and Ms. Condon-Watson took Child A to the door and gave him to his mother. During this time Child A's Mother was very upset and loud. Ms. Watson kept on asking her to leave the property. At one point Child A's Mother pushed the door back with her body as if she was going to charge into the building. She then yelled don't touch me, and you pushed me. Mr. Watson stepped forward and told Child A's Mother that no one touched her. Ms. Condon-Watson told Child A's Mother that she needed to leave the property. She did not touch Child A's Mother. They have cameras in the building and have the interaction recorded. I then interviewed Ms. Condon-Watson. On 4/14/23, Ms. Condon-Watson had a meeting in the morning. When she arrived at the center Mr. Watson informed her of his conversation with Child A's Mother that morning. Ms. Condon-Watson called Child A's Mother at about 10:00 a.m. Child A's Mother was very upset and yelling at her. She stated that no one was changing Child A's diaper and that she did not send an extra set of clothes today so that the staff will have to check him more often. Child A's Mother accused the child care staff of only changing Child A at scheduled times and not when he needed changing. Ms. Condon-Watson attempted to explain 4 to her that in the Pre-K room there are regular times that are built into the schedule for diapering and toileting.     On 4/14/23, 1 interviewed Child A's Father. Child A has been attending the child care center for four weeks. Child A attends the center all day. They have had concerns regarding lack of changing of Child A's diapers. He is coming home in an extra set of clothes because he is peeing through his clothes. Staff were questioned about the diapering and the change of clothes. The secretary, Karen Rousse, went and asked child care staff. They were told that they only change children when it is in the schedule. The day Child A came he must have missed the first diaper change/check at 8:00 a.m. and they did not check again until 8:30 a.m. Since the center provides diapers, they will not use more than three diapers a day. Child A has come home with diaper rashes because of sitting in a soiled diaper too long. I asked Child A's Father if Child A had a urinary tract infection due to sitting in soiled diapers. He was describing how Child A's penis would get red but then they would make sure and clean it and it would be fine. Child A did not see a doctor for a urinary tract infection or diaper rashes. Ms. Condon-Watson informed me that Child A was in the pre-k and the pre toddler classroom. This room has children from 30 months to four years of age. Child A attended the following days: 3/20/23, 3/24/23, 3/27/23, 3/31/23, and 4/14/23. Ms. Fante believes it was the third time that Child A came to the center that she discussed Child A's Mother's concerns with her when she dropped Child A off at the center. When she investigated the situation Child A was changed because he soaked through and got his pants wet during nap time. Ms. Fante confirmed for me when Child A was in the pre toddler room and the pre-k one room. This was done by reviewing the cameras in those rooms to see when Child A was present in the room. Ms. Rowell documented diaper changes for Child A on 3/20/23 and 3/24/23. On 3/20/23, Child A's first day Child A was in the pre-k room in the morning from about 7:30 a.m.-11:15 a.m. with Ms. Bleicher. Ms. Bleicher had eight children ages 30 months through four years of age. Child A then went into the pre toddler room late morning with Ms. Rowell and two other toddlers. Ms. Rowell documented two diaper changes while Child A was in the room. Those changes were at 11:44 a.m. and 2:50 p.m. and they were wet. Child A was picked up from the center at 4:10 p.m. A On 3/24/23, Child A was in the pre toddler room in the morning with Ms. Rowell and two other toddlers. Ms. Rowell changed his wet diaper at 9:43 a.m. and 11:49 a.m. He was in the pre-k room in the afternoon with Ms. Bleicher and seven other children ages 30 months through four years of age. Child A was picked up at 3:45 p.m. On 3/27/23, Child A was in the pre toddler classroom from 9:50 until 3:00 p.m. with Ms. Rowell and two other children. Child A was in the pre-k room until he was picked up at 3:05 p.m. There was not a diapering log kept for this day. On 3/31/23, Child A was in the pre-k room all day from 9:35a.m. until 3:10 p.m. There were two child care staff with seven children ages 30 months through four years of age. The diapering changes and toileting is built in the daily routine. This is not the only time that children's diapers are changed. The staff check and change children throughout the day also. If they notice smell or feel that a child is wet the children are changed. They do provide diapers unless they are the larger sizes then they would require the parents to provide them. Some parents do provide their own diapers. If the child is in pull ups the parents are supposed to provide them. The center has a supply of extra pull ups just in case a child needs them. They do not limit the number of diapers a child can use in a day. I interviewed, Karen Rousse, administration assistant/child care staff member. She is one of the staff people that will take children back and forth to the classroom to their parents for drop off and pick up. There was an occasion when Child A's

    On 4/11/23 and 4/12/23 I conducted a renewal inspection on- stie at the center. I observed the center to be clean and in good repair. I did not observe any safety concerns or hazards during any of my on-site inspections. Ms. Condon-Watson showed me the electrical system that had just been completely redone. The other improvements to the building included new flooring and a new rj roof in the past few years. There was an "A" rating fire inspection completed on 3/28/23 and an "A" rating environmental health inspection completed on 4/05/23. On 4/17/23, during the on-site inspection the center appeared well maintained and clean. I did not observe any safety concerns.
